OBJECTIVES Elderly patients show a higher incidence of ischemic and bleeding events after percutaneous transluminal coron-ary intervention(PCI).We sought to investigate outcomes in elderly patients treated with antithrombotic strategy guided by bleeding and ischemic risks after revascularization with last generation everolimus-eluting stent(EES).METHODS Prospective multicenter registry including patients over 75 years revascularized with EES and antithrombotic ther-apy guided by clinical presentation,PCI complexity and PRECISE DAPT ***-primary safety endpoints were:(1)composite of cardiac death,myocardial infarction and stent thrombosis and;(2)bleeding(BARC 2-5).Primary efficacy endpoint was target lesion revascularization.A matched group of patients revascularized with current drug-eluting stents and no such tailored antith-rombotic therapy was used as *** Finally,1064 patients were included in SIERRA-75 cohort,80.8±4.2 years,36.6%women,71%acute coronary syn-dromes(ACS)and 53.6%complex ***-primary safety endpoint of major adverse cardiovascular events was met in 6.2%,co-primary safety endpoint of bleeding in 7.8%and primary efficacy endpoint of TKLR in 1.5%.The multivariable adjusted model showed no significant association of the prescribed short/long dual antiplatelet therapy(DAPT)durations with any endpoint suggesting a well tailored *** stent thrombosis reported in the subgroup with 1-3 months DAPT *** compared to control group,bleeding BARC 2-5 was significantly lower in SIERRA-75 group(7.4%vs.10.2%,P=0.04)as well as the net safety-efficacy endpoint(14.3%vs.18.5%,P=0.02).CONCLUSIONS In elderly population,the application of this risks-adjusted antithrombotic protocol after revascularization with last generation EES seems to be associated with an improved prognosis in terms of ischemic and bleeding outcomes.

BACKGROUND Currently,there is insufficient data about the accuracy in the diagnosing of pancreatic cystic lesions(PCLs),especially with novel endoscopic techniques such as with direct intracystic micro-forceps biopsy(mFB)and needle-based confocal laser-endomicroscopy(nCLE).AIM To compare the accuracy of endoscopic ultrasound(EUS)and associated techniques for the detection of potentially malignant PCLs:EUS-guided fine needle aspiration(EUS-FNA),contrast-enhanced EUS(CE-EUS),EUS-guided fiberoptic probe cystoscopy(cystoscopy),mFB,and *** This was a single-center,retrospective *** identified patients who had undergone EUS,with or without additional diagnostic techniques,and had been diagnosed with *** determined agreement among malignancy after 24-mo follow-up findings with detection of potentially malignant PCLs via the EUSguided techniques and/or EUS-guided biopsy when available(EUS malignancy detection).RESULTS A total of 129 patients were included, with EUS performed alone in 47/129. In 82/129 patients,EUS procedures were performed with additional EUS-FNA (21/82), CE-EUS (20/82), cystoscopy(27/82), mFB (36/82), nCLE (44/82). Agreement between EUS malignancy detection and the 24-mo follow-up findings was higher when associated with additional diagnostic techniques thanEUS alone [62/82 (75.6%) vs 8/47 (17%);OR 4.35, 95%CI: 2.70-7.37;P < 0.001]. The highestmalignancy detection accuracy was reached when nCLE and direct intracystic mFB were bothperformed, with a sensitivity, specificity, positive predictive value, negative predictive value andobserved agreement of 100%, 89.4%, 77.8%, 100% and 92.3%, respectively (P < 0.001 comparedwith EUS-alone).CONCLUSIONThe combined use of EUS-guided mFB and nCLE improves detection of potentially malignantPCLs compared with EUS-alone, EUS-FNA, CE-EUS or cystoscopy.

Analysis of the Behavior of a Turbomachine Driven by a Compressed Air System

Different experiments in high speed turbomachines require expensive and sophisticated infrastructure to implement their impulsion and reach high regimes. In developed countries, this can be achieved by using aeroderivative gas turbines or DC motors of about 400 HP. This paper presents an experimental system designed and built with the intention of performing behavior studies in test turbomachinery. The proposed installation uses compressed air as driving fluid, which allows the turbomachinery to reach high rotational speeds where very important phenomena occur. An analysis is carried out considering the rotational speed behavior of an internal combustion engine turbocharger of the Perkins series when it is driven by pressures ranging from 4.2 kg/cm2 to zero. Additionally, another experiment couples an automotive electrical generator with the turbine to observe the system operation when a load is applied. The behavior of the pressure is analyzed when it is in function of the time of air discharge that goes from a compressed air storage tank to the turbocharger for its impulsion. This is an experimental system that can be designed and constructed economically within the bounds of any public university.

Effects of cellulase and xylanase enzymes mixed with increasing doses of Salix babylonica extract on in vitro rumen gas production kinetics of a mixture of corn silage with concentrate

An in vitro gas production （GP） technique was used to investigate the effects of combining different doses of Salix babylonica extract （SB） with exogenous fibrolytic enzymes （EZ） based on xylanase （X） and cellulase （C）, or their mixture （XC; 1：1 v/v） on in vitro fermentation characteristics of a total mixed ration of corn silage and concentrate mixture （50：50, w/w） as substrate. Four levels of SB （0, 0.6, 1.2 and 1.8 mL g-1 dry matter （DM）） and four supplemental styles of EZ （1 μL g-1 DM; control （no enzymes）, X, C and XC （1：1, v/v） were used in a 4×4 factorial arrangement. In vitro GP （mL g-1 DM） were recorded at 2, 4, 6, 8, 10, 12, 24, 36, 48 and 72 h of incubation. After 72 h, the incubation process was stopped and supernatant pH was determined, and then filtered to determine dry matter degradability （DMD）. Fermentation parameters, such as the 24 h gas yield （GY24）, in vitro organic matter digestibility （OMD）, metabolizable energy （ME）, short chain fatty acid concentrations （SCFA）, and microbial crude protein production （MCP） were also estimated. Results indicated that there was a SBxEZ interaction （P〈0.0001） for the asymptotic gas production （b）, the rate of gas production （c）, GP from 6 to 72 h, GP2 （P=0.0095）, and GP4 （P=0.02）. The SB and different combination of enzymes supplementation influenced （P〈0.001） in vitro GP parameters after 12 h of incubation; the highest doses of SB （i.e., 1.8 mL g-1 DM）, in the absence of any EZ, quadratically increased （P〈0.05） the initial delay before GP begins （L） and GP at different incubation times, with lowering b （quadratic effect, P〈0.0001 ） and c （quadratic effect, P〈0.0001 ; linear effect, P=0.0018）. The GP was the lowest （P〈0.05） when the highest SB level was combined with cellulose. There were SBxEZ interactions （P〈0.001） for OMD, ME, the partitioning factor at 72 h of incubation （PF72）, GY24, SCFA, MCP （P=0.0143）, and pH （P=0.0008）. Floods affect more people globally than any other type of natural hazard. Great potential exists for new technologies to support flood disaster risk reduction. In addition to existing expert-based data collection and analysis, direct input from communities and citizens across the globe may also be used to monitor, validate, and reduce flood risk. New technologies have already been proven to effectively aid in humanitarian response and recovery. However, while ex-ante technologies are increasingly utilized to collect information on exposure, efforts directed towards assessing and monitoring hazards and vulnerability remain limited. Hazard model validation and social vulnerability assessment deserve particular attention. New technologies offer great potential for engaging people and facilitating the coproduction of knowledge.

Processor for Measuring Radio Network Design Quality

In this paper we present the design and prototyping of an arithmetic processor based on reconfigurable technology, whose purpose is to determine in a parallel manner the quality of the solution in a radio network design optimization problem. This problem consists in the search for an optimal set of locations in which to place radio antennas in order to obtain the maximum possible coverage, for a given terrain and antenna characteristics. The original computational contribution of this work is to use programmable logic devices to avoid the high cost of computing the evolutionary algorithms required to tackle this optimization problem. This is achieved by means of reconfigurable processors working in parallel. On the basis of the results obtained from the prototype, it may be considered a parallel architecture capable of achieving a great acceleration in the calculations.

The Taylor Rule and the Sandpile: The Taylor Contribution and Other Matters

The author of the Taylor Rule has provided new evidence about the application of the Sandpile model to his rule. The same findings of the Sandpile model are described in the Taylor paper in agreement with the conclusions of the Sandpile model. That is, that keeping interest rates too low for too long penalizes the economic recovery. On top of that the Sandpile also provides a metric for the severity of the crisis. The same law (Power Law) applies to the size and the duration of the crisis just modifying the order of the distribution paving thus a way for measuring the size of the crisis. According to the NBER data, the length is already determined for the US crisis, if the model holds on, we can also assess the severity.

Sandpile phenomena in dynamic systems in the vicinity of criticality always appeal to a sudden break of stability with avalanches of different sizes due to minor perturbations. We can view the intervention of the Central Banks on the rate of interest as a perturbation of the economic system. It is an induced perturbation to a system that fare in vicinity of criticality according to the conditions of stability embedded in the equations of the neoclassical model. An alternative reading of the Taylor Rule is proposed in combination with the Sandpile paradigm to give an account of the economic crisis as an event like an avalanche, that can be triggered by a perturbation, as is the intervention of the Central Bank on the interest rate.

Assessing radiation dose for postoperative radiotherapy in prostate cancer: Real world data

Conductivity of the phloem in mango (Mangifera indica L.)

Mango(Mangifera indica L.,Anacardiaceae),the fifth most consumed fruit worldwide,is one of the most important fruit crops in tropical regions,but its vascular anatomy is quite *** studies examined the xylem structure in the stems of mango,but the anatomy of the phloem has remained elusive,leaving the long-distance transport of photoassimilates *** combined fluorescence and electron microscopy to evaluate the structure of the phloem tissue in the tapering branches of mango trees,and used this information to describe the hydraulic conductivity of its sieve tube elements following current models of fluid transport in *** revealed that the anatomy of the phloem changes from current year branches,where it was protected by pericyclic fibres,to older ones,where the lack of fibres was concomitant with laticiferous canals embedded in the phloem *** was present in the sieve plates,but also in the walls of the phloem sieve cells,making them discernible from other phloem cells.A scaling geometry of the sieve tube elements-including the number of sieve areas and the pore size across tapering branches-resulted in an exponential conductivity towards the base of the *** evaluations in mangofit with previous measurements of the phloem architecture in the stems of forest trees,suggesting that,despite agronomic management,the phloem sieve cells scale with the tapering *** pipe model theory applied to the continuous tubing system of the phloem appears as a good approach to understand the hydraulic transport of photoassimilates in fruit trees.
