BACKGROUND: This study aimed to compare the rates of open and laparoscopic cholecystectomies and outcomes in patients with or without type 2 diabetes mellitus (T2DM) in Spain from 2003 to 2013. METHODS: We collected all cases of open and laparoscopic cholecystectomies using national hospital discharge data and evaluated the annual cholecystectomy rates stratiifed by T2DM status. We analyzed tendency for in-hospital mortality (IHM). We also analyzed the impact of T2DM on IHM in patients who underwent cholecystectomies. RESULTS: We identiifed 611 533 cholecystectomies (71.3%laparoscopic) in the patients, in whom 78 227 (12.8%) patients had T2DM. The rates of open cholecystectomies were 3-fold higher (130.0/105 vs 41.1/105) in patients with T2DM than in those without T2DM, and the rate of laparoscopic cholecys-tectomies was almost 2-fold higher (195.2/105 vs 111.8/105) in patients with T2DM. The annual rate of laparoscopic pro-cedures showed an 11-year relative increase of 88.3% (from 117.0/105 to 220.3/105) in T2DM and 49.2% (from 79.2/105 to 118.2/105) in patients without T2DM (P CONCLUSION: The rate of cholecystectomy was higher in patients with T2DM, and laparoscopic cholecystectomy was p

在地中海国家扩大灌溉土地的面积,应当考虑气候与土地利用变化对水资源的影响。本文利用降水和温度资料,分析了20世纪中叶以来西班牙比利牛斯山脉中部流域径流的变化趋势。降水与径流之间关系的年变化表明,径流在所研完期间的后半时段相对较低,这一趋势与土地利用变化一致。在月水平上,降水量在10月、4月和7月明显增加,而在3月则有下降:温度在1月和2月有所增加,而在4月则趋于下降。但过去50年间径流在大部分月份中呈显著下降趋势,土地利用和植被覆盖的变化是解释年平均径流下降大约30%的惟一非气候因素。

Simulation, Construction and Characterization of a Piezoelectric Transducer Using Rexolite as Acoustic Matching

This article describes the simulation and characterization of an ultrasonic transducer using a new material called Rexolite to be used as a matching element. This transducer was simulated using a commercial piezoelectric ceramic PIC255 at 8 MHz. Rexolite, the new material, presents an excellent acoustic matching, specially in terms of the acoustic impedance of water. Finite elements simulations were used in this work. Rexolite was considered as a suitable material in the construction of the transducer due to its malleability and acoustic properties, to validate the simulations a prototype transducer was constructed. Experimental measurements were used to determine the resonance frequency of the prototype transducer. Simulated and experimental results were very similar showing that Rexolite may be an excellent matching, particularly for medical applications

Assessment of Present and Future Radioactive Contamination at Global Scale

As a result of more than two thousand nuclear tests performed since 1945, two war actions and few accidents in nuclear reactor, it does already exist a radioactive contamination at global scale. This contamination has been accumulated mainly in marine sediments, because sea is about 80% of planet surface, and solid fission products released by nuclear explosions are transported by wind first and then by rain to liquid and solid portions on earth, while gaseous fission products are diffused in the atmosphere. In this way, heavy, high yielding fission products, such as 137Cs, get marine sediments, where they are found mixed with heavy radioactive natural elements such as U, Th and their also radioactive decay products. But since alkaline are so abundant on earth, it is also found natural radioactive isotope 40K, 0.0118% of isotopes forming K element, with half life 1.28 × 109, in the time range of planet age and elements origin. So, one easy way to assess both the importance and evaluation of radioactive contamination at global scale should be to establish a RCF (radioactive contamination factor) as a percentage of 137Cs contaminant radioactivity in marine sediments, compared to 40K natural radioactivity present from the very beginning of earth, both expressed as Bq per gram of sediment. In the frame of these rather general considerations, a research project has started in Mexico whose varied purposes are: to characterize sea regions in both vast littorals in the country, by its natural 40K radioactivity present in sea salts, as well as to determine if 137Cs traces produced by recent radioactive contamination have reached their coasts. Also, to assess the 137Cs traces already present in marine sediments as a result of more than two thousand nuclear test explosions performed till now, rather than contamination produced by four main accidents in nuclear reactors during the last 60 years. Efficacy of Alzer<sup>®</sup>and Diamel, Nutritional Supplements, in the Prevention of Severe Diabetic Macular Edema

Introduction: Macular edema is the main cause of low vision in diabetic patients. Laser photocoagulation continues to be the treatment of choice in conjunction with the use of steroids and anti-angiogenics, but these treatments include possible ocular complications. The nutritional supplement Alzer (whose primary active ingredient is Ginkgo biloba, a powerful antioxidant that acts on vascular factors and oxidative damage, which are two of the mechanisms implicated in the pathogenesis of diabetic macular edema), which has been used on other non-diabetic macular conditions, along with the Diamel nutritional supplement has been shown to be effective on glycemic control and could represent a treatment alternative for mild to moderate macular edema by reducing the thickness of the macular retina and preventing the progression of other more advanced clinical presentations that are harder to treat. Objective: Identify the effect of Alzer along with Diamel in reduction of the thickness of the macular retina among patients with mild to moderate diabetic macular edema. Materials and Methods: A phase II double-blind clinical trial was conducted in 64 patients with non-severe diabetic macular edema over the course of non-proliferative diabetic retinopathy, who attended the ophthalmology service of the Institute of Endocrinology of Havana from January 2016 to December 2016. Ultrasonic Sensors with Mechanical Couplers： Simulation and Validation

The use of ultrasonic sensors has varied applications, but the sensor operation frequency limits the operating distance. An easy way to increase this distance is to couple a mechanical element (horn), but it is necessary to characterize this technique. In this paper the results obtained in a study of the behaviour of mechanical elements coupled to an ultrasonic sensor using finite element techniques are presented. These results have been obtained using Comsol Multiphysics modelling. Also, the effect caused by the sensor size on the radiation acoustic pressure has also been evaluated. In other way, in this paper it is presented the results obtained in the laboratory measurements. First, it is studied the influence of a straight horn attached to the ultrasonic sensor. Later, it is presented the variation in the sound pressure on the radiation axis when the sensor varies its size. In the final part of the paper, the experimental validation of the simulations is presented.

Conceptual Framework for an Integrated Method to Optimize Sustainability of Engineering Systems

It is necessary to change the current dynamic of growth, to assure that future generations can satisfy their needs. Sustainable development and global sustainability are two concepts that have achieved great importance in almost every sector of activity. There is a wide range of methods and models for sustainability assessment. Nevertheless, it is necessary to go beyond evaluation, looking for sustainability optimization. In spite of this, little work has been done on the latter field. The authors present here a conceptual proposal for an integrated method to optimize sustainability of engineering systems, based on the MIVES (integrated value method for evaluating sustainability) method. For a better understanding of the method, its essential steps for optimizing an energy sub-system are summarized (shell-and-tube heat exchanger).
