Background: Both competitive and recreational sports are considered a positive practice that is good for human health and quality of life, so it is difficult to understand how young, energetic and apparently healthy people can die while playing their usual sports. When these events occur in elite athletes during monitored events, they normally receive considerable media attention. Objective: To describe the case of sudden death in a young professional athlete who was successfully aided and resuscitated outside our hospital, and to review the literature about sudden death in sports. materials and method: We searched for Sudden Death (according to the International Classification of Diseases (ICD-9 and ICD-10)) in our hospital but found no other relevant cases. We performed a literature search on the state of the art in sudden death (Google Scholar, Pubmed, LILACS, Scielo and the Revista Española de Cardiología). Results: Although we observed that the prevalence of sudden death is not very high, it still represents a potentially avoidable event. An adequate medical examination prior to regular sporting practice can decrease the incidence of sudden death in athletes under the age of 35. Data available in the literature support medical check-ups that include history-taking, physical examination and electrocardiogram. The findings of this evaluation may also prompt further assessment (electrocardiograph, ergometry). Conclusions: We could not identify standard, obligatory guidelines for screening possible heart diseases causing cardiorespiratory failure and sudden death. As a measure for primary prevention, we propose a protocol for reducing the risk of sudden death in athletes.

Caregiver Burden in the management of Frail Elderly Patients with Diabetes in Internal medicine

Aim: Advanced age and fragility often lead to dependence, making caregiver assistance necessary for performing the activities of daily living. Patients with diabetes are characterized by an elevated presence of comorbidities and subsequent frailty—a relevant consideration when making decisions regarding treatment goals. Health professionals may overlook the caregiver experience when assessing the circumstances surrounding frail elderly patients with diabetes, but this factor can have a large impact on relevant caregiver and patient outcomes. methods: An observational, retrospective study was carried out to assess the impact of improved management in diabetes on caregiver burden. We assessed patients’ performance of activities of daily living using the Barthel scale. Our primary outcome was caregiver burden, as assessed using the Zarit scale. Results: At baseline, 81.4% of the caregivers reported severe overburden (>55 points on the Zarit scale). At three months, the proportion of caregivers reporting this level of burden had dropped to 76.7% and remained stable at six months (77.1%). In contrast, 8.5% of the caregivers reported moderate overburden (47 to 55 points) at baseline, compared to 13.3% at three months and 12.5% at six. The proportion of caregivers reporting no overburden (Conclusions: Caregiver burden remained persistently high, decreasing only slightly after six months.

Introduction: Post-hospital syndrome (PHS) is defined as a period of vulnerability during the first 30 days after a patient is discharged from hospital, in which multiple factors come into play. Hyponatremia is the most frequent hydroelectrolytic disorder in hospitalized patients and may be related to the appearance of PHS. Objective: The objective is to estimate the prevalence of PHS that is assessed as the rate of readmissions in the first 30 days after discharge, in patients with hyponatremia. material and methods: It is a descriptive observational study of patients with hyponatremia who were discharged from 1 September 2010 to 2 February 2020 at the Internal medicine Service of the Hospital University of San juan (Alicante, Spain). Results: Of the 25 included patients, 5 (20%) were readmitted within a month of discharge, after a mean of 11.4 days (standard deviation [SD] 5.1). The overall mortality of the study was 20% (n = 5), with one case of death in the first 30 days post-hospitalization (4%). In 12 patients (48%) the origin of the hyponatremia was undetermined. The most frequently recorded etiology for the condition was pharmacological (n = 7, 28%), and there was pronounced variability in its clinical and laboratory study. The most widely used corrective measure was drug withdrawal, in 16 patients (64%). Water intake restriction was the most common treatment after discharge (5 patients, 20%), followed by urea (2 patients, 8%), while tolvaptan was not used. Conclusion: Hyponatremia may be the cause of PHS, which could increase the rate of early readmission. Hyponatremia is an underdiagnosed and undertreated entity, so it is necessary to apply an appropriate system to optimize its management and, in future studies, to assess its impact on PHS.

Effect of Noisy Fitness in Real-Time Strategy Games Player Behaviour Optimisation Using Evolutionary Algorithms

This paper investigates the performance and the results of an evolutionary algorithm （EA） specifically designed for evolving the decision engine of a program （which, in this context, is called bot） that plays Planet Wars. This game, which was chosen for the Google Artificial Intelligence Challenge in 2010, requires the bot to deal with multiple target planets, while achieving a certain degree of adaptability in order to defeat different opponents in different scenarios. The decision engine of the bot is initially based on a set of rules that have been defined after an empirical study, and a genetic algorithm （GA） is used for tuning the set of constants, weights and probabilities that those rules include, and therefore, the general behaviour of the bot. Then, the bot is supplied with the evolved decision engine and the results obtained when competing with other bots （a bot offered by Google as a sparring partner, and a scripted bot with a pre-established behaviour） are thoroughly analysed. The evaluation of the candidate solutions is based on the result of non-deterministic battles （and environmental interactions） against other bots, whose outcome depends on random draws as well as on the opponents＇ actions. Therefore, the proposed GA is dealing with a noisy fitness function. After analysing the effects of the noisy fitness, we conclude that tackling randomness via repeated combats and reevaluations reduces this effect and makes the GA a highly valuable approach for solving this problem.

This paper analyzes the network of passes among the players of the Spanish team during the last FIFA World Cup 2010,where they emerged as the champion,with the objective of explaining the results obtained from the behavior at the complex network *** team is considered a network with players as nodes and passes as(directed) edges.A temporal analysis of the resulting passes network is also done,looking at the number of passes,length of the chain of passes,and to network measures such as player centrality and clustering *** of the last three matches(the decisive ones) indicate that the clustering coefficient of the pass network remains high,indicating the elaborate style of the Spanish *** effectiveness of the opposing team in negating the Spanish game is reflected in the change of several network measures over time,most importantly in drops of the clustering coefficient and passing length/speed,as well as in their being able in removing the most talented players from the central positions of the ***'s ability to restore their combinative game and move the focus of the game to offensive positions and talented players is shown to tilt the balance in favor of the Spanish team.

Quasi-in vivo corrosion behavior of AZ31B mg alloy with hybrid mWCNTs-PEO/PCL based coatings

This study investigated the effects of multi-walled carbon nanotubes(mWCNTs) and polycaprolactone(PCL) on the quasi-in vivo corrosion behavior of AZ31B mg alloy treated by plasma electrolytic oxidation(PEO). Thin(~2 μm, PCTPCL4) and thick(~60 μm, PCTPCL6) PCL layers were applied only onto the mWCNTs-PEO coating(PCT) as it showed better corrosion performance. Findings reveal that incorporation of mWCNTs induced several structural and functional modifications in the PEO coating, such as increased roughness, a thicker inner barrier layer, and reduced ***-in vivo corrosion testing was carried out under controlled temperature, p H, and fluid flow in simulated body fluid(SBF) by electrochemical impedance spectroscopy(EIS) and hydrogen evolution experiments. EIS results revealed that, after 48 h immersion, a diffusion process controlled hydration of the ceramic coatings. Comparison of the collected hydrogen after 15 days of immersion in the quasi-in vivo environment revealed that the PEO and PCT ceramic coatings decreased hydrogen generation by up to 74% and 91%, respectively, compared to non-coated ***6 coating exhibited the lowest amount of collected hydrogen(0.2 m L/cm^(2)). The thick PCL layer delayed the onset of substrate corrosion for at least 120 h, reducing the corrosion rate by 85% compared with the PCT.

Future constructions in the context of the industrial wastelands reuse may be exposed to Vapor Intrusion(VI).VI can be evaluated by combining in-situ measures and analytical models to evaluate exposure risk in future indoor ***,the assumptions in the existing models may reduce their accuracy when they do not meet the characteristics of real *** estimations of indoor concentration levels may lead to inappropriate solutions against *** this context,new semi-empirical models(SEm)are proposed in order to better specify pollution scenarios and thus increase the accuracy of VI *** development is based on a parametric study(numerical CFD)and a dimensionless analysis combined to existing VI models that consider a continuous source distribution in the *** expressions allow to better take into account the source position in the soil(*** and lateral source/building separation),soil properties(air permeability,diffusion coefficient of the pollutant,…)and building features(building foundation,indoor pressure,air exchange rate,…)in the estimation of indoor concentration *** obtained results with the proposed SEm were compared with a numerical CFD model and available experimental data,showing good accuracy in the estimation of *** the advantages of these new models,they can provide better precision in the health risk assessments associated with ***,these expressions can be easily integrated into building ventilation codes allowing to consider air exchange rate and indoor pressure variations over time.
