New real-time-PCR method to identify single point mutations in hepatitis C virus

AIM To develop a fast, low-cost diagnostic strategy to identify single point mutations in highly variable genomes such as hepatitis C virus(HCV).METHODS In patients with HCV infection, resistance-associated amino acid substitutions within the viral quasispecies prior to therapy can confer decreased susceptibility to direct-acting antiviral agents and lead to treatment failure and virological relapse. One such naturally occurring mutation is the Q80 K substitution in the HCV-NS3 protease gene, which confers resistance to PI inhibitors, particularly simeprevir. Low-cost, highly sensitive techniques enabling routine detection of these single point mutations would be useful to identify patients at a risk of treatment failure. Light Cycler methods, based on real-time PCR with sequencespecific probe hybridization, have been implemented in most diagnostic laboratories. However, this technique cannot identify single point mutations in highly variable genetic environments, such as the HCV genome. To circumvent this problem, we developed a new method to homogenize all nucleotides present in a region except the point mutation of interest. RESULTS Using nucleotide-specific probes Q, K, and R substitutions at position 80 were clearly identified at a sensitivity of 10%(mutations present at a frequency of at least 10% were detected). The technique was successfully applied to identify the Q80 K substitution in 240 HCV G1 serum samples, with performance comparable to that of direct Sanger sequencing, the current standard procedure for this purpose. The new method was then validated in a Catalonian population of 202 HCV G1-infected individuals. Q80 K was detected in 14.6% of G1 a patients and 0% of G1 b in our setting. CONCLUSION A fast, low-cost diagnostic strategy based on real-time PCR and fluorescence resonance energy transfer probe melting curve analysis has been successfully developed to identify single point mutations in highly variable genomes such as hepatitis C virus. BACKGROUND Hepatocellular carcinoma(HCC) is the most common type of primary liver cancer. Current guidelines for HCC management recommend surveillance of high-risk patients every 6 mo using ultrasonography. Serum biomarkers, like alpha-fetoprotein(AFP), protein induced by vitamin K absence/antagonist-Ⅱ(PIVKA-Ⅱ) and lectin-reactive AFP, show suboptimal performance for detection of HCC, which is crucial for successful resection or treatment. Thus, there is a significant need for new biomarkers to aid early diagnosis of HCC. Studies have shown that the expression level of human microRNAs(miRNAs), a small, non-coding RNA species released into the blood, can serve as an early marker for various diseases, including *** To evaluate the diagnostic role of miRNAs in HCC as single markers, signatures or in combination with known protein *** Our prospective, multicenter, case-control study recruited 660 participants(354 controls with chronic liver disease and 306 participants with HCC) and employed a strategy of initial screening by two independent methods, real-time quantitative PCR(n = 60) and next-generation sequencing(n = 100), to assess a large number of miRNAs. The results from the next-generation sequencing and real-time quantitative PCR screening approaches were then combined to select 26 miRNAs(including two putative novel miRNAs). Those miRNAs were analyzed for their diagnostic potential as single markers or in combination with other miRNAs or established protein biomarkers AFP and PIVKA-Ⅱ via real-time quantitative PCR in training(n = 200) and validation cohorts(n = 300).RESULTS We identified 26 miRNAs that differentiated chronic liver disease controls from(early) HCC via two independent discovery approaches. Effect of snow on mountain river regimes： an example from the Pyrenees

The purpose of this study was to characterize mountain river regimes in the Spanish Pyrenees and to assess the importance of snow accumulation and snowmelt on the timing of river flows. Daily streamflow data from 9 gauging stations in the Pyrenees were used to characterize river regimes. These data were analyzed by hydrological indices, with a focus on periods when snow accumulation and snowmelt occurred. These results were combined with data on Snow Water Equivalent （SWE）（from measure- ments of depth and density of snow in the main river basins and also simulated by a process-based hydrological model）, snowmelting （simulated by a process-based hydrological model）, precipitation （from observations）, and temperature （from observations）. Longitude. and elevation gradients in the Pyrenees explain the transition of river regimes from those that mostly had low nival signals （in the west and at low elevations） to those that mostly had high nival signals （low winter runoff and late spring peakflow, in the east and at high elevations）. Although trend analyses indicated no statistically signifi- cant changes, there was a trend of decreased nival signal over time in most of the analyzed rivers. Our results also demonstrated that snow processes cannot explain all of the interannual variability of river regimes, because the temporal distribution of liquid precipitation and tempera- ture play key roles in hydrography.

Regulatory T cells in patients with inflammatory bowel diseases treated with adacolumn granulocytapheresis

AIM: To investigate if the clinical efficacy of granulocytes and monocytes by adsorption (GMA) is associated with an increased frequency of peripheral regulatory T cells (Tregs), as these cells have proven to be successful in suppressing inflammatory bowel disease (IBD) in animal models. METHODS: We report four cases of corticosteroid- dependent ulcerative colitis (UC) and two Crohn’s disease (CD) cases with severe cutaneous lesions who received GMA therapy. The frequency of CD4+ CD25high (Tregs) in peripheral blood was analyzed by flow cytometry and the expression of FoxP3 and TGF beta in purified CD4+ T cells was determined by real time PCR prior to and one month after the last apheresis session, and at the time of endoscopic and clinical assessing. RESULTS: Increased expression of Fox P3 mRNA was found in all five patients who responded to cytapheresis with remission of clinical symptoms, mucosal inflammation and cutaneous lesions, and an increased frequency of circulating Tregs was found in four patients. These changes were not observed in the patient with UC who did no respond to GMA. Variations in TGF-β (mRNA) did not parallel that of FoxP3 mRNA. CONCLUSION: The clinical efficacy of GMA on IBD and related extra intestinal manifestations was associated with an expansion of circulating CD4+ CD25+ Tregs and higher expression of FoxP3 in CD4+ T cells. Accordingly, an elevated CD4+ CD25+ FoxP3 may be a valuable index of remission in patients with IBD and other chronic relapsing-remitting inflammatory conditions during treatment with GMA.

Mechanical and natural ventilation systems in a greenhouse designed using computational fluid dynamics

The aim of this study was to analyse air exchange and temperature distribution in a greenhouse with combined mechanical and natural ventilation and to design more efficient mechanical ventilation *** this purpose,a computational fluid dynamics(CFD)model of the greenhouse was *** configurations were considered:Configuration 1(mechanical ventilation and closed roof ventilators),Configurations 2 and 3(mechanical ventilation and roof ventilators open 30%and 100%,respectively).After validation,the CFD model was used to improve the design of the greenhouse mechanical ventilation system in each of the three configurations *** greenhouse lengths,28 m,50 m,75 m and 100 m,were used in the *** to fan ventilation only,roof ventilation improved the climate of fan-ventilated greenhouses in terms of the air exchange rate(22%)and climate uniformity because the internal air was mixed better than with mechanical ventilation *** the greenhouse length increased,more advantages were achieved with natural ventilation compared to mechanical *** most configurations,there was a strong linear correlation between temperature gradient and greenhouse *** greenhouse whose regression line had the steepest slope was the one with closed roof *** the fan capacity produced a general reduction in temperature,but the effect was less intense for the greenhouses with open roof *** to box inlet ventilators,an enlarged continuous inlet in the wall opposite the fans increased overall system performance because it eliminated backflow recirculation zones,which are prone to produce high temperatures.

Splenic rupture following colonoscopy

Colonoscopy is a safe and routinely performed diagnostic and therapeutic procedure for different colorectal diseases. Although the most common complications are bleeding and perforation, extracolonic or visceral injuries have also been described. Splenic rupture is a rare complication following colonoscopy, with few cases reported. We report a 60-year-old female who presented to surgical consultation 8 h after a diagnostic colonoscopy. Clinical, laboratory and imaging findings were suggestive for a massive hemoperitoneum. At surgery, an almost complete splenic disruption was evident, and an urgent splenectomy was performed. After an uneventful postoperative period, she was discharged home. Splenic injury following colonoscopy is considered infrequent. Direct trauma and excessive traction of the splenocolic ligament can explain the occurrence of this complication. Many times the diagnosis is delayed because the symptoms are due to colonic insuffl ation, so the most frequent treatment is an urgent splenectomy. A high index of suspicion needs an early diagnosis and adequate therapy.
