Introduction: Vulnerable is someone who can be hurt or receive injury, physically or morally. The work environment appears as one of the dimensions in which social vulnerability develops and social and occupational vulnerability is defined. The literature does not include an assessment of quantified occupational vulnerability as it already exists in the case of social vulnerability. The aim of this paper is to identify and quantify the variables included in the Protocol for the Assessment of Vulnerability to COVID-19 in such a way that the result can be quantified and allows for a predictive effect on the degree of vulnerability. Methodology: The starting point is the design of a protocol proposal that includes 29 variables. Data is collected from a sample of 420 workers, quantifying the results according to each variable, the overall scores for each group of aspects assessed and, finally, the total score that estimates the degree of vulnerability according to established ranges. Results: Variables that have the highest coefficient and, therefore, the greatest weight in the probability of reaching a group of high or medium vulnerability are cardiovascular disease, cancer, and coagulation alteration. The weight of the labour aspects due to inadequate working conditions stands out. Conclusions: The results obtained with this protocol allow us to make a quantified assessment of labour vulnerability to COVID-19 by integrating individual variables of the worker, his working conditions and the preventive actions of his company against COVID-19, and can be useful as an Occupational Vulnerability Index (OVI).

The Structure of <i>Pinus hartwegii</i>at the Cofre de Perote, Veracruz, Mexico

A study was conducted on the forest structure of Pinus hartwegii Lindl., located at the Cofre de Perote National Park. The density, diameter, height and wooded coverage were evaluated along an altitudinal gradient from 3500 to 4000 meters in 20 plots of 100 m2. The structure of the population was found to be in the form of an “inverted J”. No statistically significant differences were found for any of the above-mentioned variables evaluated in the altitudinal range. A high density of suppressed individuals was found to occur at 50% of the sites sampled. Given the suppression characteristics of the trees, shelterwood is suggested for a better development of the woodland mass, as well as the introduction of mycorrhizal plants.

Toll-like receptor 4 polymorphisms and bacterial infections in patients with cirrhosis and ascites

AIM To assess the relationship between the presence of toll-like receptor 4(TLR4) polymorphisms and bacterial infections in cirrhotic patients with ascites. METHODS We prospectively included consecutive patients with cirrhosis and ascites hospitalized during a 6-year period. Patients with human immunodeficiency virus(HIV) infection or any other immunodeficiency, patients with advanced hepatocellular carcinoma(beyond Milan's criteria) or any other condition determining poor short-term prognosis, and patients with a permanent urinary catheter were excluded. The presence of D299 G and/or T399 I TLR4 polymorphisms was determined by sequencing and related to the incidence and probability of bacterial infections, other complications of cirrhosis, hepatocellular carcinoma, and mortality during follow-up. A multivariate analysis to identify predictive variables of mortality in the whole series was performed. RESULTS We included 258 patients: 28(10.8%) were carriers of D299G and/or T399I TLR4 polymorphisms(polymorphism group) and 230 patients were not(wildtype group). The probability of developing any bacterial infection at one-year follow-up was 78% in the polymorphism group and 69% in the wild-type group(P = 0.54). The one-year probability of presenting infections caused by gram-negative bacilli(51% vs 44%, P = 0.68), infections caused by gram-positive cocci(49% vs 40%, P = 0.53), and spontaneous bacterial peritonitis(29% vs 34%, respectively, P = 0.99) did not differ between the two groups. The oneyear probability of transplant-free survival was 55% in the polymorphism group and 66% in the wild-type group(P = 0.15). Multivariate analysis confirmed that age, Child-Pugh score, active alcohol intake, previous hepatic encephalopathy, hepatocellular carcinoma and serum creatinine were associated with a higher risk of death during follow-up. BACKGROUND Currently,there is insufficient data about the accuracy in the diagnosing of pancreatic cystic lesions(PCLs),especially with novel endoscopic techniques such as with direct intracystic micro-forceps biopsy(mFB)and needle-based confocal laser-endomicroscopy(nCLE).AIM To compare the accuracy of endoscopic ultrasound(EUS)and associated techniques for the detection of potentially malignant PCLs:EUS-guided fine needle aspiration(EUS-FNA),contrast-enhanced EUS(CE-EUS),EUS-guided fiberoptic probe cystoscopy(cystoscopy),mFB,and *** This was a single-center,retrospective *** identified patients who had undergone EUS,with or without additional diagnostic techniques,and had been diagnosed with *** determined agreement among malignancy after 24-mo follow-up findings with detection of potentially malignant PCLs via the EUSguided techniques and/or EUS-guided biopsy when available(EUS malignancy detection).RESULTS A total of 129 patients were included, with EUS performed alone in 47/129. In 82/129 patients,EUS procedures were performed with additional EUS-FNA (21/82), CE-EUS (20/82), cystoscopy(27/82), mFB (36/82), nCLE (44/82). Agreement between EUS malignancy detection and the 24-mo follow-up findings was higher when associated with additional diagnostic techniques thanEUS alone [62/82 (75.6%) vs 8/47 (17%);OR 4.35, 95%CI: 2.70-7.37;P < 0.001]. The highestmalignancy detection accuracy was reached when nCLE and direct intracystic mFB were bothperformed, with a sensitivity, specificity, positive predictive value, negative predictive value andobserved agreement of 100%, 89.4%, 77.8%, 100% and 92.3%, respectively (P < 0.001 comparedwith EUS-alone).CONCLUSIONThe combined use of EUS-guided mFB and nCLE improves detection of potentially malignantPCLs compared with EUS-alone, EUS-FNA, CE-EUS or cystoscopy.

A simulation model based on nonlinear ordinary differential equations to interpret the transmission dynamics of Zika Virus (ZIKV), is formulated and analyzed, integrating the asymptomatic human population and coupled to the Aedes aegypti dynamics, the epidemic threshold Basic Reproduction Number R0 is determined, as the spectral radius of Next-Generation Matrix and the system is simulated with MAPLE computing program taking the parameter values from literature.

Modulation of gastric accommodation by duodenal nutrients

AIM： To determine the relative potency and contribution of intestinal nutrients to net gastric accommodative relaxation and conscious perception. METHODS： In 12 healthy subjects, we randomly tested duodenal loads of lipids and carbohydrates （12 mL administered in 4 rain） at various caloric concentrations （0.0125-0.8 kcal/mL） separated by 12-24 rain wash-out periods of saline infusion. Maximal gastric relaxation was induced at the end of each experiment by i.v glucagon （5 μg/kg）, as reference. The reflex gastric response was measured by a barostat, and symptom perception by a 0-6 score questionnaire. RESULTS： Upids induced a dose-response gastric relaxation with a steep and early rise. Maximal effect （179±42 mL relaxation） reached at a relatively low concentration （0.2 kcal/mL）, maximal lipid-induced relaxation was 61±6% of the glucagon effect. By contrast, duodenal infusion of carbohydrates induced weaker relaxation that became significant only at the high end of the physiological concentration range （65±14 ml with 0.8 kcal/mL）. Intestinal nutrient loads, either of lipid or carbohydrates, did not induce significant changes in perception （0.6±0.4 and 0.1±0.4 score increase for the highest concentrations, respectively）. CONCLUSION： Chyme entering the small bowel induces nutrient-spedfic gastric relaxatory reflexes by a physiologically saturable mechanism. Normally, neither the intestinal nutrient load nor the gastric accommodative response is perceived.

In this systems paper,we present MillenniumDB:a novel graph database engine that is modular,persistent,and open *** is based on a graph data model,which we call domain graphs,that provides a simple abstraction upon which a variety of popular graph models can be supported,thus providing a flexible data management engine for diverse types of knowledge *** engine itself is founded on a combination of tried and tested techniques from relational data management,state-of-the-art algorithms for worst-case-optimal joins,as well as graph-specific algorithms for evaluating path *** this paper,we present the main design principles underlying MillenniumDB,describing the abstract graph model and query semantics supported,the concrete data model and query syntax implemented,as well as the storage,indexing,query planning and query evaluation techniques *** evaluate MillenniumDB over real-world data and queries from the Wikidata knowledge graph,where we find that it outperforms other popular persistent graph database engines(including both enterprise and open source alternatives)that support similarqueryfeatures.

Determinants of percutaneous coronary intervention success in repeat chronic total occlusion procedures following an initial failed attempt

AIM To investigate the rates and determinants of success of repeat percutaneous coronary intervention(PCI) following an initial failed attempt at recanalising the chronic total occlusions(CTO) *** In 445 consecutive first attempt CTO-PCI procedures in our institution,procedural failure occurred in 149(33.5%). Sixty-four re-PCI procedures were performed in 58 patients(39%) all had a single CTO. Procedural and outcome data in the re-PCI population was entered into the institutional database. A retrospective analysis of clinical,angiographic and procedural data was performed. RESULTS Procedural success was achieved in 41(64%) procedures. Univariate analysis of clinical and angiographic characteristics showed that re-PCI success was associated with intravascular ultrasound(IVUS) guidance(19.5% vs 0%,P = 0.042),while failure was associated with severecalcification(30.4% vs 9.7%,P = 0.047) and a JCTO score > 3(56.5% vs 17.1% P = 0.003). Following multiple regression analysis the degree of lesion complexity(J-CTO score > 3),IVUS use,involvement of an experienced CTO operator and LAD CTO location were significant predictors of successful re-PCI. Overall the complication rate was low,with the only MACCE two periprocedural MI's neither of which required intervention. CONCLUSION Re-PCI substantially increases the overall success rate of CTO revascularization. Predictors of re-PCI success included the use of IVUS,the involvement of an experienced CTO operator in the repeat attempt and the location of the CTO.

The extant Cabrera’s vole,Microtus cabrerae,differs in morphology and evolutionary history from the other species of *** arvicoline has unique derived features in the cranium,mandible and ***bly its most conspicuous features are its large size,the high skull in lateral view,the long and distally broad nasals,and the triangle shape of the anteroconid complex,with a marked labio-lingual asymmetry of the occlusal surface of the first lower *** this study,we propose a phylogenetic lineage that includes Cabrera’s vole in what until now has been the Microtus subgenus *** information and several life history traits support the elevation of Iberomys to the rank of *** Iberomys comprises species that have appeared in succession during the Quaternary:in the Early Pleistocene,the extinct *** in the Middle Pleistocene,the extinct *** and in the Late Pleistocene,the extant ***,the extant species shows several biological singularities,such as multiple polymorphic copies of the SRY male-specific gene in both males and females,and the lowest basal metabolic rate in relation to weight among arvicoline ***,its habitat requirement is unique among the Iberian ***,the biological and paleontological data that we present in this work support the elevation of its taxonomic rank to that of ge*** study also suggests a modification of nomenclature:Microtus(Iberomys)brecciensis is replaced with *** and the common name of the extant M.(I.)cabrerae changed from‘topillo’to‘iberon’to improve conservation and protection actions.

Alterations of Blood Immune Cells in Elderly Patients with COVID-19 and with or without Pre-Existing Type 2 Diabetes

Elderly individuals, especially those with pre-existing conditions like type 2 diabetes mellitus (T2DM), have a high risk for developing severe cases of COVID-19. The aim of this work was to characterize the alterations of blood immune cells (BIC) in patients with symptomatic COVID-19 and confirmed SARS-CoV-2 infection, ≥60 years and who needed hospitalization in the Centro de Salud Hospital of Tucuman, Argentina, during the second peak of the pandemic in Argentina. Ten patients were enrolled from December 2020 to May 2021. Blood samples were taken at the time of admission (day 0) and five days after (day 5) for routine laboratory tests and the characterization of BIC by flow cytometry. Most of the patients were men (70%) aged between 60 and 78 years. The 70% of patients had T2DM while 50% had arterial hypertension. At day 0, all the patients had increased neutrophils and inflammatory markers (C reactive protein and D-dimers) and reduced numbers of lymphocytes, HLA-DRhi monocytes, CD16+CD56+ NK cells, CD3+HLA−DR+CD25+ cells, CD4+ and CD8+ T cells in blood. Patients received a standard treatment for COVID-19 care (O2, corticosteroids and antibiotics). The hospital treatment normalized the levels of BIC (day 5) in 30% of patients who were those with no comorbidities. In patients with T2DM, BIC recovery was variable. In T2DM patients who required administration of plasma (30%), prolonged O2 therapy (40%) or referral to the intensive care unit (10%) significant reductions of CD16+CD56+, CD3+HLA−DR+CD25+, CD4+ and CD8+ cells were observed between days 0 and 5. In line with previous studies, our results show that absolute counts of major lymphocyte subsets in blood are significantly and substantially decreased during the course of severe COVID-19 disease
