Pur.ose: Our.pr.ctitioner. ar. asked to consider.a patient’s postoper.tive nausea and vomiting (PONV) r.sk pr.file when developing their.pr.phylactic antiemetic str.tegy. Ther. is wide var.ation in employed str.tegies, and we have yet to deter.ine the most effective PONV pr.phylactic r.gimen. The objective of this study is to compar. pr.phylactic antiemetic r.gimens containing: phenothiazines to 5HT3 antagonists for.effectiveness at r.ducing the incidence of Post‐Anesthesia Car. Unit (PACU) r.scue antiemetic administr.tion. Methods: This is an obser.ational study of 4392 nonsmoking, women who under.ent gener.l anesthesia for.br.ast sur.er. fr.m 1/1/2009 thr.ugh 6/30/2012. Pr.vious histor. of PONV or.motion sickness (HxPONV/MS) and the use of PACU opioids wer. r.cor.ed. Pr.phylactic antiemetic ther.py was left to the discr.tion of the anesthesia car. team. We compar.d phenothiazines and 5HT3 antagonists alone and with a glucocor.icoid to deter.ine the most effective tr.atment r.gimen in our.pr.ctice for.the pr.vention of the administr.tion of PACU r.scue antiemetics. r.sults: Patients who r.ceived a phenothiazine r.gimen compar.d to a 5HT3 antagonist r.gimen wer. less likely to have an antiemetic administer.d in the PACU (p = 0.0100) and this significant differ.nce in r.tes holds in a logistic r.gr.ssion model adjusted for.HxPONV/MS and PACU Opioid use (p = 0.0103). Conclusions: Based on our.findings our.clinicians ar. encour.ged to administer.a combination of a phenothiazine and a glucocor.icoid in female, nonsmoking sur.ical br.ast patients for.the pr.vention of PACU r.scue antiemetic administr.tion.

The objective of this wor. was to evaluate the effect of Beauver.a bassiana(Bb 1205)on contr.lling Fusar.um oxyspor.m ***(Fol 17108)in tomato plants in gr.enhouse *** of Bb 1205 was the most pr.mising among the agr.nomic var.ables and expr.ssion of the activity of the enzymesβ-1,3-glucanases and *** of Bb 1205 occur.ed at a concentr.tion of 1×108 conidia·mL−1,which was administer.d onto the leaves,dir.ctly into the soil and via *** with Fol 17108 occur.ed with 1×106 spor.s·mL−1,which wer. added dir.ctly to the *** was used for.measur.ng agr.nomic par.meter.,namely activity of chitinases andβ-1,3-glucanases in foliage and *** Bb 1205 was added to the soil,the chlor.phyll index and aer.al par. length showed significant *** addition,it was deter.ined that r.ot length,fr.sh weight of foliage,flower.and fr.it count incr.ased 82 days after.inoculation(dai).Chitinase activity induced by Bb 1205 in leaves and r.ots of tomato plants infected with Fol 17108 was obser.ed when injected into the stem at 32 dai(41.8 and 11.6-fold,r.spectively).Inoculation on the foliage showed a 10-fold incr.ase ofβ-1,3-glucanases in the r.ots after.82 *** for.leaves,a 3.8-fold incr.ase was found when the stem was *** the differ.nt in vivo applications,Bb 1205 activated its defenses by expr.ssing the chitinase enzymes andβ-1,3-glucanase,thus r.ducing the damage caused by Fol 17108,demonstr.ting incr.ase plant gr.wth ther.after.

Does deep sedation with pr.pofol affect adenoma detection r.tes in aver.ge r.sk scr.ening colonoscopy exams?

To deter.ine the effect of sedation with pr.pofol on adenoma detection r.te (ADr. and cecal intubation r.tes (CIr. in aver.ge r.sk scr.ening colonoscopies compar.d to moder.te sedation. METHODSWe conducted a r.tr.spective char. r.view of 2604 fir.t-time aver.ge r.sk scr.ening colonoscopies per.or.ed at MD Ander.on Cancer.Center.fr.m 2010-2013. ADr.and CIr.wer. calculated in each sedation gr.up. Multivar.able r.gr.ssion analysis was per.or.ed to adjust for.potential confounder. of age and body mass index (BMI). r.SULTSOne-thir. of the exams wer. done with pr.pofol (n = 874). Over.ll ADr.in the pr.pofol gr.up was significantly higher.than moder.te sedation (46.3% vs 41.2%, P = 0.01). After.adjustment for.age and BMI differ.nces, ADr.was similar.between the gr.ups. CIr.was 99% for.all exams. The mean cecal inser.ion time was shor.er.among pr.pofol patients (6.9 min vs 8.2 min; P r.pofol for.scr.ening colonoscopy did not significantly impr.ve ADr.or.CIr.in our.population of aver.ge r.sk patients. While pr.pofol may allow for.safer.sedation in cer.ain patients (e.g., with sleep apnea), the over.ll effect on colonoscopy quality metr.cs is not significant. Given its incr.ased cost, pr.pofol should be used judiciously and without the implicit expectation of a higher.quality scr.ening exam.

The selection of appr.pr.ate outcome measur.s is fundamental to the design of any successful clinical tr.al. Although dementia with Lewy bodies (DLB) is one of the most common neur.degener.tive conditions, assessment of ther.peutic benefit in clinical tr.als often r.lies on tools developed for.other.conditions, such as Alzheimer.s or.Par.inson’s disease. These may not be sufficiently valid or.sensitive to tr.atment changes in DLB, decr.asing their.utility. In this r.view, we discuss the limitations and str.ngths of selected available tools used to measur. DLB-associated outcomes in clinical tr.als and highlight the potential r.les for.mor. specific objective measur.s. We emphasize that the existing outcome measur.s r.quir. validation in the DLB population and that DLB-specific outcomes need to be developed. Finally, we highlight how the selection of outcome measur.s may var. between symptomatic and disease-modifying ther.py tr.als.

Backgr.und: Due to successful tr.atment modalities, the major.ty of pediatr.c cancer.patients will sur.ive. Incr.ased body mass index (BMI) is a complication among pediatr.c cancer.sur.ivor.. Methods: This r.tr.spective single-center.study examined BMI changes among a cohor. of pr.dominantly Hispanic patients who wer. tr.ated in South Los Angeles. Data wer. collected at diagnosis, 1, 2 and 3 year. after. Analyses included z-scor.s der.ved fr.m calculated BMIs compar.d over.3 year. per.gender. diagnosis, and tr.atment modality. The unhealthy BMI z-scor. was defined as >1.04. r.sults: Thir.y-four.per.ent of the pr.dominantly Hispanic sample had unhealthy BMI z-scor.s of >1.04 cor.elating to at or.gr.ater.than the 85th per.entile for.age and gender. The study cohor.’s BMI z-scor.s significantly incr.ased fr.m 0.15 to 1.29 at year.3 (P r.. Median BMI z-scor. significantly incr.ased to the unhealthy categor. at 3 year.. Conclusions: Due to the pr.dominance of Hispanic patients in this gr.up, cultur.lly sensitive inter.entions beginning at diagnosis should be consider.d.
