AIM: To investigate the association of hepatitis B virus (HBV) genotype and HBeAg seroconversion after nucleotide analogue ***: Chronic hepatitis B patients receiving lamivudine followed up for at least 6 months post-treatment were studied. Consecutive treatment-naive patients who were prospectively followed up in the clinic for at least 18 months were studied as controls. HBeAg seroconversion was defined as loss of HBeAg, appearance of anti-HBe and normalization of alanine aminotransferase for at least 6 ***: Thirty-five patients on lamivudine and 96 control patients followed up for 39 (18-49) months were *** was given for 12 (10-18) months, and patients were followed up for 15 (6-34) months after drug *** B and C HBV were found in 43 and 88 patients and HBeAg seroconversion occurred in 12 (28 %) and 16(18 %) patients, respectively (P=0.30). There was no difference in HBeAg seroconversion between patients infected by genotype B and C HBV in the control (35 % vs 21%, P=0.25) and lamivudine-treated (14 % vs 10 %,P=1.00) ***: HBeAg seroconversion after treatment by lamivudine was not influenced by the HBV genotype.

Background: Hepatectomy is a widely accepted curative treatment for hepatocellular carcinoma (HCC). However, the disease frequently recurs after a curative hepatectomy. The objective of this study is to provide a better understanding of the pattern of disease recurrence and the risk factors involved so as to improve the postoperative surveillance. Methods: A retrospective study for all patients receiving hepatectomy for HCC between 2003 and 2014 was performed. Emphasis was made on the timing and pattern of recurrent disease, and type of treatment given. Results: There were 506 patients in the study. Median follow-up was 43.7 months. The 1-, 3-, 5-, 10-year overall and disease free survival were 89.5%, 74.1%, 63.9%, 49.0% and 69.5%, 54.3%, 43.4%, 30.9%respectively. Recurrent disease occurred in 267 patients, 47.2% occurred within 9 months of hepatectomy and 80.1% recurred only in liver. Median survival was shorter for recurrence occurring within 9 months compared with those occurring between 10 months and 2 years postoperatively (36.2 vs. 65.7 months, P3.5 cm and history of rupture were risk factors for recurrence within 9 months. Conclusions: These findings suggest that recurrent diseases are common after curative hepatectomy for HCC and most recurrences occur in the remnant liver. Since almost half of recurrences occurred within first 9 months after hepatectomy, a more stringent postoperative surveillance with target imaging of liver in this period is needed. Early diagnosis of recurrent disease and curative retreatment hopefully can bring about a longer survival.

BACKGROUND In women worldwide, breast cancer is the most common cancer. Breast cancer accounted for 26.6% of all new cancers in females diagnosed in 2015 in hong *** To examine women's awareness, perception, knowledge, and screening practice of breast cancer in hong *** We carried out a population-based survey using random telephone interviews to women aged 18 or above using the United Kingdom Cancer Research Breast Cancer Awareness Measure(United Kingdom CAM). The data was analysed using proportions, chi-square test(χ~2-test) and adjusted odds ratios(ORs).RESULTS A total of 1000 participants completed the CAM questionnaire from 1,731 responses(response rate = 57.8%) from September to October 2017. One in five and one in four respondents recalled ≥ 3 early warning signs and ≥ 2 risk factors of breast cancer respectively. The majority(62.6%) reported they were not confident that they would notice a change in their breasts. Among the respondents, 16.8% would have regular mammography at least every two *** general, 4 in 10 women had tried practices on preventing breast *** with better result in recalling breast cancer signs and symptoms were more likely to seek immediate medical help when noticed a change in their breasts(χ2-test P = 0.038), and more likely had tried prevention practice(χ~2-test P< 0.001). Globally,in-home drug-storage compliance is often unsatisfactory,especially among older adults,and can lead to negative health *** study used a cross-sectional and descriptive design to examine inhome drug-storage compliance among older *** sampling was used to recruit 117 older adults in hong *** in-home drugstorage checklist was used to assess the in-home drug-storage environments(light,temperature,and humidity)and drug-storage methods(drug safety,packaging,information,and expiration)of each older *** findings showed that hong Kong older adults’overall compliance rate in drug storage was 87.25%,and their compliance rate for drug-storage methods(84.59%)was lower than that for drug-storage environments(97.02%).Older adults who were of advanced age,who were less educated or who lived alone demonstrated lower in-home drug-storage *** indicates the need to revise existing health-education strategies to encourage in-home drug-storage *** professionals should assess older adults’drug-storage compliance to identify less compliant subgroups and deliver specific drug-storage support as *** members should also be involved in this process.

胆道纤维化(BF)是病理性胆道损伤修复的结果,主要表现为胆管壁的增厚硬化和管腔的进行性狭窄,最终可能导致胆道梗阻、胆汁性肝硬化、肝衰竭、肝胆恶性肿瘤等严重不良结局。目前的研究将BF描述为某些胆管病的病理特征,对其病因、病理生理、分子机制和治疗缺乏系统总结。BF是胆道系统普遍存在但容易被忽略的疾病状态,可能通过病理性胆道损伤后的异常修复机制促进肝胆疾病的发生和进展,笔者结合国内外最新研究进展,综述BF的概念、临床表现、病因、发病机制和治疗策略,为临床医师提供参考。
