BACKGROUND The Impella pump has emerged as a promising tool in patients with cardiogenic shock(CS).Despite its attractive properties,there are scarce data on the specific clinical setting and the potential role of Impella devices in CS patients from routine clinical *** This is an observational,retrospective,single center,cohort *** consecutive patients with diagnosis of CS and undergoing support with Impella 2.5?,Impella CP?or Impella 5.0?from April 2015 to December 2020 were *** characteristics,management and outcomes were assessed according to CS severity,age and cause of *** outcome measured was in-hospital *** A total of 50 patients were included(median age:59.3±10 years).The most common cause of CS was acute coronary syndrome(ACS)(68%),followed by decompensation of previous cardiomyopathy(22%).A total of 13 patients(26%)had profound *** patients(54%)improved pulmonary congestion at 48 h after Impella support.A total of 19 patients(38%)presented significant ***-hospital mortality was 42%.Among patients with profound CS(n=13),five patients were previously supported with venoarterial extracorporeal membrane oxygenation.A total of eight patients(61.5%)died during the admission,and no patient achieved ventricular *** patients(≥67 years,n=10)had more comorbidities and the highest mortality(70%).Among patients with ACS(n=34),35.3%of patients had profound CS;and in most cases(52.9%),Impella support was performed as a bridge to *** contrast,only one patient from the decompensated cardiomyopathy group(n=11)presented with profound *** 90.9%of these cases,Impella support was used as a bridge to cardiac *** were no cases of *** In this cohort of real-life CS patients,Impella devices were used in different settings,with different clinical profiles and *** a significant rate of complications,mortality was acceptable and lower than those observed

The vertebrate fossil record from the Balearic Islands(western Mediterranean)has improved considerably over the past decade,especially in Mallorca and *** Menorca,the Pliocene terrestrial fauna was updated by the discovery and description of the large-sized leporid Nuralagus,several reptiles and an *** Mal-lorca,paleontological exploration yielded 2 deposits with a Late Miocene/Early Pliocene chronology,Calóden Rafelino(CdR)and Na Burguesa-1(NB-1).So far,4 new mammalian taxa and 2 new reptiles have been iden-tified for the CdR deposit,whereas the faunal assemblage from the recently discovered deposit(Apr 2012)of NB-1 is currently composed of,at least,6 terrestrial mammals,8 reptiles and an *** faunal composi-tion and some primitive characteristics of the obtained taxa suggest that the chronology of this deposit is slight-ly earlier than the *** terrestrial vertebrates recorded in these 2 Mallorcan deposits are changing the view of the paleofaunal assemblage previously known for the Plio-Pleistocene of the *** charac-teristics displayed by some of the taxa suggest that these faunas would be at the beginning of an isolated *** this paper we present a preliminary report on the fossils recovered from the NB-1 deposit,as well as some unpublished data from CdR,and we analyze the whole fauna from both Mallorcan deposits,focusing on taxonomical and paleobiogeographical aspects.

Identification of the precise molecular pathways involved in oncogene-induced transformation may help us gain a better understanding of tumor initiation and promotion. Here, we demonstrate that SOX2^+ foregut epithelial cells are prone to oncogenic transformation upon mutagenic insults, such as Kras^G12D and p53 deletion. GFP-based lineage-tracing experiments indicate that SOX2^+ cells are the cells-of-origin of esophagus and stomach hyperplasia. Our observations indicate distinct roles for oncogenic KRAS mutation and P53 deletion. p53 homozygous deletion is required for the acquisition of an invasive potential, and Kras^G12D expression, but not p53 deletion, suffices for tumor formation. Global gene expression analysis reveals secreting factors upregulated in the hyperplasia induced by oncogenic KRAS and highlights a crucial role for the CXCR2 pathway in driving hyperplasia. Collectively, the array of genetic models presented here demonstrate that stratified epithelial cells are susceptible to oncogenic insults, which may lead to a better understanding of tumor initiation and aid in the design of new cancer therapeutics.

Patients suffering from liver cirrhosis(LC) frequently require non-hepatic abdominal surgery,even before liver *** is an important risk factor itself for surgery,due to the higher than average associated morbidity and *** high surgical risk occurs because of the pathophysiology of liver disease itself and to the presence of contributing factors,such as coagulopathy,poor nutritional status,adaptive immune dysfunction,cirrhotic cardiomyopathy,and renal and pulmonary dysfunction,which all lead to poor *** evaluation of these factors and the degree of liver disease can help to reduce the development of complications both during and after abdominal *** the emergency setting,with the presence of decompensated LC,alcoholic hepatitis,severe/advanced LC,and significant extrahepatic organ dysfunction conservative management is preferred.A multidisciplinary,individualized,and specialized approach can improve outcomes;preoperative optimization after risk stratification and careful management are mandatory before *** techniques can also improve *** review the impact of LC on surgical outcome in non-hepatic abdominal surgeries required in this cirrhotic population before,during,and after surgery.

Liver involvement was one of the first extraglandular manifestations to be reported in patients with primary Sj(o)gren syndrome (SS).In the 1990s,a study of liver involvement in patients with primary SS integrated the evaluation of clinical signs of liver disease,liver function and a complete panel of *** developments in the field of hepatic and viral diseases have significantly changed the diagnostic approach to liver involvement in *** most recent studies have shown that,after eliminating hepatotoxic drugs and fatty liver disease,the two main causes of liver disease in primary SS are chronic viral infections and autoimmune liver *** differential diagnosis of liver disease in primary SS (viral vs autoimmune) is clinically important,since the two processes require different therapeutic approaches and have different *** respect to viral infections,chronic HCV infection is the main cause of liver involvement in SS patients from the Mediterranean area,while chronic HBV infection may be the main cause of liver involvement in SS patients from Asian *** eliminating viral hepatitis,primary biliary cirrhosis (PBC) should be considered the main cause of liver disease in primary ***-related SS patients may have a broad spectrum of abnormalities of the liver,including having no clinical or analytical data suggestive of liver *** hepatitis (AIH) is the second most frequently found autoimmune liver disease to be associated with SS (all reported cases are type I),and nearly 10% of these patients have an AIH-PBC ***,IgG4-related disease must be investigated in patients with SS presenting with sclerosing cholangitis,especially when autoimmune pancreatitis or retroperitoneal fibrosis are also present.
