AIM: To evaluate the effect of femtosecond laser-assisted lens surgery(FLALS;cataract surgery or refractive lens exchange) on the structure of the optic nerve head and the ***: This prospective longitudinal study included healthy eyes undergoing FLALS. Eyes with glaucoma or any other ocular disease that could alter optical coherence tomography results were excluded. Retinal nerve fiber layer(RNFL), Bruch’s membrane opening-minimum rim width(BMO-MRW) and macular thickness(MT) were measured preoperatively, 1 and 6 mo after surgery using spectral-domain optical coherence tomography(SD-OCT). changes between preoperative and postoperative values were ***: A total of 87 eyes of 46 patients were included in this study. Preoperative RNFL, BMO-MRW and MT in microns(μm) were 100.77±10.39, 330.31±49.99 and 276.30±33.39, respectively. Postoperative RNFL, BMO-MRW and MT were 104.74±11.55, 348.32±54.05 and 279.83±22.65 1 mo after surgery and 102.93±11.17, 343.11±53.4 and 278.90±22.19 6 mo after surgery, respectively;which equals an increase of 3.93%, 5.45% and 1.27%,respectively, 1 mo after surgery, and 2.14%, 3.87% and 0.94% 6 mo after surgery. The differences between the preoperative and the postoperative RNFL and BMO-MRW values were statistically significant(P<0.001). Regarding MT values, there were not statistically significant differences(P=0.26).CONCLUSION: Our study suggests that FLALS does not have a negative impact on the structural status of the optic nerve head in healthy eyes, assessed by SD-OCT. There is a slight increase in the values of RNFL, BMO-MRW and MT 1 mo and 6 mo after surgery.

Patent foram ovale(PFO)is the most common anatomical cause of an interarterial *** is usually asymptomatic but may cause paradoxical embolism and is a risk factor for non-lacunar cryptogenic cerebral ischemia in young *** the first clinical trials did not show a significant superiority of PFO closure in the secondary prevention of cerebral ischemia as compared with standard antithrombotic treatment,six subsequent randomized clinical trials(CLOSURE I,PC Trial,RESPECT,CLOSE,REDUCE,and DEFENSE-PFO)performed in a sample of cryptogenic stroke in patients aged 60 years or younger provided evidence of a significant reduction of recurrent cerebral ischemia after percutaneous PFO ***,the use of percutaneous PFO closure cannot be generalized to the entire population of patients with cerebral ischemia and PFO,but it is indicated in highly selected patients with non-lacunar cryptogenic cerebral infarction with a large right-to-left shunt,an atrial septal aneurysm and no evidence of atrial fibrillation,as well as in association with antithrombotic treatment for an optimal secondary prevention of cerebral ischemia.

Biosorption of mercury by Macrocystis pyrifera and Undaria pinnatifida:Influence of zinc,cadmium and nickel

This study investigated the adsorption of Hg（Ⅱ） on Macrocystis pyrifera and Undaria pinnatifida in monometallic system in the presence of Zn（II）,Cd（Ⅱ） and Ni（II）.The two biosorbents reached the same maximum sorption capacity （q m = 0.8 mmol/g） for *** showed a greater affinity （given by the coefficient b of the Langmuir equation） for mercury compared to *** （4.4 versus 2.7 L/mmol）.Mercury uptake was significantly reduced （by more than 50%） in the presence of competitor heavy metals such as Zn（II）,Cd（II） and Ni（Ⅱ）.Samples analysis using an environmental scanning electron microscopy equipped with an energy dispersive X-ray microanalysis showed that mercury was heterogeneously adsorbed on the surface of both biomaterials,while the other heavy metals were homogeneous *** analysis of biosorbents by Fourier transform infrared spectrometry indicated that Hg（Ⅱ） binding occurred on S=O （sulfonate） and N-H （amine） functional groups.

Postoperative adhesions after abdominal surgery in children:a pilot study in the Philippines

background Postoperative adhesions(POAs)causing mechanical bowel obstruction is a challenging problem for children who underwent prior abdominal *** in adults,the role of conservative management remains controversial and children tend to require ***,despite the longer lifetime risk inherent to the pediatric population,studies are still lacking to come up with guidelines on diagnosis and *** study aimed to review the profiles and outcomes of patients who underwent surgery for POA and to identify significant risk *** This was a retrospective review of 172 patients admitted at the Philippine General Hospital from 2010 to 2018 with the diagnosis of *** who did not undergo surgery and patients who had a different cause of bowel obstruction with the adhesions being only incidental were excluded from the *** Ninety-one patients were included in the *** mean age at surgery was 92.32 months and the male:female ratio was 2.8:*** most common primary diagnoses were appendicitis,intussusception,and abdominal *** occurred within the first postoperative year in 63%.The 30-day morbidity and mortality rates were 9.89%and 7.69%,*** most common complications were fascial and anastomotic dehiscences,and sepsis was the most common cause of *** creation during the index operation,and the presence of bowel gangrene,intestinal resection,and stoma creation during the surgery for POA were statistically significant risk factors for morbidity and *** The findings in this study support the experience in literature that timely operative intervention can prevent bowel strangulation in cases of mechanical bowel obstruction from ***,more data and studies are recommended to arrive at an evidence-based guideline in the management of these children.

In-situ conversion presents a promising technique for exploiting continental oil shale formations,characterized by highly fractured organic-rich rock.A 3D in-situ conversion model,which incorporates a discrete fracture network,is developed using a self-developed thermal-flow-chemical(TFC)*** of the model elucidates the in-situ conversion process in three stages and defines the transformation of fluids into three distinct outcomes according to their end *** findings indicate that kerogen decomposition increases fluid pressure,activating fractures and subsequently enhancing permeability.A comprehensive analysis of activated fracture permeability and heating power reveals four distinct production modes,highlighting that increasing heating power correlates with higher cumulative fluid *** fractures,with heightened permeability,facilitate the mobility of heavy oil toward production wells but hinder its cracking,thereby limiting light hydrocarbon ***,energy efficiency research demonstrates the feasibility of the in-situ conversion in terms of energy utilization,especially when considering the surplus energy from high-fluctuation energy sources such as wind and solar power to provide heating.

Management of critically ill patients with diabetes

Disorders of glucose homeostasis, such as stress-induced hypoglycemia and hyperglycemia, are common complications in patients in the intensive care unit. Patients with preexisting diabetes mellitus(DM) are more susceptible to hyperglycemia, as well as a higher risk from glucose overcorrection, that may results in severe hypoglycemia. In critically ill patients with DM, it is recommended to maintain a blood glucose range between 140-180 mg/d L. In neurological patients and surgical patients, tighter glycemic control(i.e., 110-140 mg/d) is recommended if hypoglycemia can be properly avoided. There is limited evidence that shows that critically ill diabetic patients with a glycosylated hemoglobin levels above 7% may benefit from looser glycemic control, in order to reduce the risk of hypoglycemia and significant glycemic variability.
