Use of a lyophilized bovine pericardium graft to repair tunical defect in patients with Peyronie＇s disease： experience in a clinical setting

Many different materials are available for grafting during surgery for Peyronie＇s disease （PD）. To study the outcomes associated with the use of a lyophilized bovine pericardium graft （Peri-Guard） to repair tunical defects in patients with PD, descriptive study of 43 patients with disabling penile curvature was performed. Curvature was measured before and after the intervention using a standardized procedure. Surgical technique was plaque excision and grafting of the tunical defect carried out by a single surgeon. A lyophilized bovine pericardium patch was used for grafting. We measured the change in penile curvature and length, the ability to perform successful sexual intercourse without further pharmacological or surgical treatment, and the satisfaction with treatment. Follow-up data were available for a total of 41 patients. Median age was 50 years （IQR 48-52）; mean follow-up time was 14 ± 6 months. Complete penile straightening was achieved in 33 patients （80.5%） and five patients （12.2%） had curvature under 20. Three months after surgery and stretching therapy, mean （s.d.） flaccid penile length increased from 11.2 ± 2.8 cm to 12.1 ± 2.9 cm （P = 0.062）. Seventy-five percentage of patients reported being able to achieve successful intercourse without further treatment. Eighty-five percentage of patients reported to be satisfied with the treatment. There was no evidence of tissue rejection or infection, and only four patients suffered an adverse event. Plaque excision and grafting of the tunical defect with lyophilized bovine pericardium seems to be a safe and effective treatment for patients with PD.

Neutrophil-to-lymphocyte ratio(NLR)and platelet-to-lymphocyte ratio(PLR)have been associated with multiple entities and several types of cancers.They can be assumed as markers of inflammatory imbalance.The objective of this study is to evaluate the NLR and PLR in Peyronie’s disease(PD)and to establish a comparison of its values in the acute and chronic stages.We recruited patients with PD from March 2018 to March 2019.The patients enrolled underwent medical and sexual history as well as a physical examination.The values of blood count of each patient were collected both in the acute and chronic stages.Wilcoxon test was used to compare the acute and chronic stage ratios.Kruskal-Wallis test was carried out to evaluate the impact of treatments on the ratios.To identify cutoff values,we used sensibility and specificity tables and receiver operating characteristic(ROC)curves.A total of 120 patients were enrolled.Their mean age was 55.85(range:18-77)years and the mean penile curvature was 48.43°(range:10°-100°).In the acute stage,the mean NLR was 2.35 and the mean PLR was 111.22.These ratios,in the chronic stage,were 1.57 and 100.00,respectively.Statistically significant differences between acute and stable stages for both indices were found(NLR:P<0.0001;PLR:P=0.0202).The optimal cutoff for classification in acute or stable stage was 2 for NLR and 102 for PLR.According to our results,with an ordinary blood count,we could have important indications regarding the disease stage of the patient,and consequently on the most appropriate type of therapy to choose.

There is no doubt that land cover and climate changes have consequences on landslide activity,but it is still an open issue to assess and quantify their impacts.Wanzhou County in southwest China was selected as the test area to study rainfall-induced shallow landslide susceptibility under the future changes of land use and land cover(LULC)and climate.We used a high-resolution meteorological precipitation dataset and frequency distribution model to analyse the present extreme and antecedent rainfall conditions related to landslide activity.The future climate change factors were obtained from a 4-member multimodel ensemble that was derived from statistically downscaled regional climate simulations.The future LULC maps were simulated by the land change modeller(LCM)integrated into IDRISI Selva software.A total of six scenarios were defined by considering the rainfall(antecedent conditions and extreme events)and LULC changes towards two time periods(mid and late XXI century).A physically-based model was used to assess landslide susceptibility under these different scenarios.The results showed that the magnitude of both antecedent effective recharge and event rainfall in the region will evidently increase in the future.Under the scenario with a return period of 100 years,the antecedent rainfall in summer will increase by up to 63%whereas the event rainfall will increase by up to 54%for the late 21st century.The most considerable changes of LULC will be the increase of forest cover and the decrease of farming land.The magnitude of this change can reach+22.1%(forest)and–9.2%(farmland)from 2010 until 2100,respectively.We found that the negative impact of climate change on landslide susceptibility is greater than the stabilizing effect of LULC change,leading to an over decrease in stability over the study area.This is one of the first studies across Asia to assess and quantify changes of regional landslide susceptibility under scenarios driven by LULC and climate change.Our results aim to guide land use planning and clima

Comparison of the patient and partner satisfaction with 700CX and Titan penile prostheses

Most widespread three-component penile prosthesis models are 700CXTM and Titan~. Our purpose is to assess patient and partner satisfaction after the first implant. This is a multicenter, retrospective, nonrandomized study in which all patients who met the inclusion criteria between 2009 and 2013 were included. In total, 248 patients agreed to participate. To evaluate patient satisfaction, a validated but modified 11-question questionnaire was completed （EDITS）; and a nonvalidated two-item questionnaire was given to the partner. Statistical analysis used an ordinal logistic regression model. Two hundred and forty-eight patients （194 with 700CXTM vs 54 with Titan） and 207 couples completed the questionnaire （165 with 700CXTM vs 42 with Titan）. Overall satisfaction was high. Both showed great reliability for sexual intercourse and high compliance with prior expectations. Most patients were able to manage the penile prosthesis correctly within 6 months. Postoperative penile shortening led to some dissatisfaction in 42% and 46% of cases （700CX/Titan）. Significant differences were found in three questions of patients＇ questionnaire. There were more patients satisfied with the 700CXTM （P= 0.0001）. No patient with Titan implant took longer than 6 months to optimal management. Only 4% of patients with 700CXTM implant were dissatisfied with the deflation, in contrast to 24% with the Titan （P = 0.0031）. Of the two partners＇ questions, one showed a statistically significant difference （P = 0.0026）. It seems that group 700CX＂ would recommend to re-implant the prosthesis with a greater tendency. The overall satisfaction was very high for both prostheses. The final aspect of the erected and flaccid penis was satisfactory, but both groups showed significant discontent with its final size. Partners＇ overall satisfaction was high.

Behavioral and physiological traits of ectotherms are especially sensitive to fluctuations of environmental temperature.In particular,niche-specialist lizards are dependent on their physiological plasticity to adjust to changing environmental conditions.Lizards of the genus Phymaturus are viviparous,mainly herbivorous,and inhabit only rock promontories in the steppe environments of Patagonia and the Andes.Herein,we examine the vulnerability of the southernmost Phymaturus species to global warming:the endemic Phymaturus calcogaster,which lives in a mesic environment in eastern Patagonia.We studied body temperatures in the field(T_(b)),preferred body temperatures in a thermogradient(T_(pref)),the operative(T_(e))and environmental temperatures,and the dependence of running performance on body temperature.P.calcogaster had a mean T_(b)(27.04℃)and a mean Te(31.15℃)both lower than their preferred temperature(T_(pref)=36.61℃)and the optimal temperature for running performance(T_(o)=37.13℃).Lizard activity seems to be restraint during the early afternoon due high environmental temperatures.However,both,the high safety margin and warming tolerance suggest that the expected increase in environmental temperatures due to global warming(IPCC report in 2018)would not threaten,but indeed enhance locomotor performance in this population.

Transcriptional regulation,determined by the chromatin structure and regulatory elements interacting at pro-moter regions,is a key step in plant responses to environmental cues.Nitrate(NO3-)is a nutrient signal that regulates the expression of hundreds of genes in Arabidopsis thaliana.Here,we integrate mRNA sequencing,genome-wide RNA polymeraseⅡ(RNPⅡ),chromatin immunoprecipitation sequencing,and DNase Sequencing datasets to establish the relationship between RNPⅡoccupancy and chromatin accessibility in response to NO3-treatments in Arabidopsis roots.Genomic footprinting allowed us to identify in vivo regula-tory elements controlling gene expression in response to NO3-treatments.NO3--modulated transcription factor(TF)footprints are important for a rapid increase in RNPⅡoccupancy and transcript accumulation over time.We mapped key TF regulatory interactions and functionally validated the role of NAP,an NAC-domain containing TF,as a new regulatory factor in NO3-transport.Taken together,our study provides a comprehensive view of transcriptional networks in response to a nutrient signal in Arabidopsis roots.

Percutaneous drainage as a first therapeutic step prior to surgery in liver hydatid cyst abscess:Is it worth it?

AIM To delay surgery until the patient is in a better condition, and thus to decrease postoperative morbidity. METHODS Using this algorithm we treated three patients aged 55, 75 and 80 years. In all three patients the clinical presentation was fever without a clear source of infection; all had nonspecific symptoms such as general malaise, dyspnea, and abdominal discomfort in the previous 15 d. They came to the emergency room at our hospital due to deterioration of their general condition. Analytical tests showed leukocytosis, neutrophilia and increased polymerase chain reaction. In all cases an abdominal computed tomography(CT) was performed and liver hydatid abscess(LHA) was detected. The mean size of the LHA was 12 cm.RESULTS All patients underwent CT-guided percutaneous drainage. The purulent material obtained was cultured, and Klebsiella pneumoniae, Streptococcus viridans and Streptococcus salivarius were identified. Antibiotic treatment was given adapted to antibiotic sensitivity testing. Surgery was performed two weeks after admission, once the patient's condition had improved. All three patients underwent an almost total cystectomy, cholecystectomy and omentoplasty in the residual cavity. Complications were: Clavien Ⅰ(atelectasis and pleural effusion) and Clavien Ⅱ(transfusion). The average length of stay(pre and postoperative) was 23 d. At the follow-up, no relapses were recorded.CONCLUSION LHA management is not standardized. Emergency surgery offers suboptimal results. Percutaneous drainage plus antibiotics allows improving patient's general condition. This enables treating patients in greater safety and also reduces complications.

According the Coulomb earth pressure theory,it is obtained that,for normally consolidated soils,the lateral pressure coefficient of a soil at rest is equal to 1,and it is independent of the soil type,either granular or cohesive;or that the material is in a loose or compact state;hard or a soft cohesive soil.Also,a methodology to calculate the earth pressure for intermediate states between at rest condition and the active pressure is presented.In addition,a methodology to calculate the earth pressure for intermediate states between at rest condition and the passive pressure is presented.Two practical examples are presented:one for a frictionless wall;and another for a coarse wall.Practical recommendations are given for the use of the lateral earth pressure coefficient for different applications.

Model based current mode control design and experimental validation for a 3φ rectifier under unbalanced grid voltage conditions

This paper addresses the control design and the experimental validation of a current mode control for a three phase voltage source rectifier. The proposed control law is able to fulfill the voltage regulation and the current tracking control objectives despite of unbalanced and distorted grid voltages. The proposed control law consists of two loops, which are referred as inner tracking loop and the outer voltage regulation loop. The inner loop is designed to provide damping to the system, which also includes and adaptive mechanism. The construction of the current reference is based on the positive component detection of thegrid voltage. Therefore, the current produced by the power rectifier is proportional to the fundamental component of the grid voltage, despite of the presence of unbalanced grid voltages. The voltage regulation loop is designed as a proportional-integral controller, which is aimed to regulate the DC output voltage to a desired level. Finally, experimental results are obtained in an experimental prototype of2 kW to evaluate the performance of the proposed controller.
