Prevalence and risk factors of acute-on-chronic liver failure in a single center from Argentina

AIM To study the prevalence, characteristics, risk factors and mortality at 28 d of acute-on-chronic liver failure(ACLF).METHODS A total of 100 cirrhotic patients admitted to our hospital for more than one day were included during the period between June 2013 and December 2015. We used the European Association for the Study of the Liver-Chronic Liver Failure-Consortium diagnostic criteria for ACLF, considering it as the acute decompensation of cirrhosis associated with the presence of one or more organ failure. For the diagnosis of organic failure the Chronic Liver Failure-Sequential Organ Failure Assessment score was used. Our population was divided into patients with and without ACLF. Clinical characteristics, presence of precipitating events, potential risk factors for developing ACLF and causes of mortality were analyzed. Mortality at 28 d was evaluated.RESULTS Twenty-nine patients(29%) developed ACLF criteria. Alcoholism, detected in 58 patients(58%), was the major etiological agent of cirrhosis. Bacterial infections were recognized as a precipitating event in 41.3% of cases and gastrointestinal bleeding in 27.5%. No precipitating event was identifiable in 27.5% of patients with ACLF. Comparing patients with and without ACLF, statistically significant risk factors were: Child Pugh score 10.2 ± 2.1 vs 8.4 ± 1.6(P < 0.0001), MELD score 20.7 ± 8.5 vs 12.3 ± 4(P < 0.0001), presence of ascites 27(93%) vs 43(60.5%)(P = 0.001), leukocytosis 15300 ± 8033 per cubic millimeter vs 10770 ± 5601 per cubic millimeter(P < 0.0001), and high plasma levels of C reactive protein values 50.9 ± 46.4 mg/L vs 28.6 ± 23.4 mg/L(P < 0.0019). Mortality rate was 62%(18 patients) vs 5.6%(4 patients), respectively(P < 0.0001).CONCLUSION We observed that the ACLF is a frequent entity in this group of patients and has a significantly higher mortality rate.

The Internet of Moving Things(IoMT)takes a step further with respect to traditional static IoT deployments.In this line,the integration of new eco-friendly mobility devices such as scooters or bicycles within the Cooperative-Intelligent Transportation Systems(C-ITS)and smart city ecosystems is crucial to provide novel services.To this end,a range of communication technologies is available,such as cellular,vehicular WiFi or Low-Power Wide-Area Network(LPWAN);however,none of them can fully cover energy consumption and Quality of Service(QoS)requirements.Thus,we propose a Decision Support System(DSS),based on supervised Machine Learning(ML)classification,for selecting the most adequate transmission interface to send a certain message in a multi-Radio Access Technology(RAT)set up.Different ML algorithms have been explored taking into account computing and energy constraints of IoMT enddevices and traffic type.Besides,a real implementation of a decision tree-based DSS for micro-controller units is presented and evaluated.The attained results demonstrate the validity of the proposal,saving energy in communication tasks as well as satisfying QoS requirements of certain urgent messages.The footprint of the real implementation on an Arduino Uno is 444 bytes and it can be executed in around 50µs.

This study presents an analysis of sediments in an Andean river impacted by both natural conditions and anthropogenic activities. Fifty samples were collected from selected sites throughout the Moquegua River drainage basin, and Tambo River headwaters at Pasto Grande, in Peru, and analysed with X-ray diffraction, energy dispersive X-ray spectroscopy, scanning electron microscopy, Fourier-transform infrared spectroscopy and thermogravimetric analysis. Si, Ca, Al, Fe, and O, common constituents of soils, were dominant, along with K, N, and P. The latter originating as primary macronutrients or chemical residuals from fertilizers used in agriculture. Higher concentrations of macronutrients and organic components were found in agricultural and urban areas, respectively. Arsenic minerals were also detected, which occur naturally, but possibly at levels augmented by anthropogenic activity. The application of cluster analysis revealed clustering between arsenic, arsenolite, and potassium but no significant geospatial correlation between sample sites in the drainage basin.

In 2008,the European Association for the study of the liver(EASL) defined occult hepatitis B virus infection (OBI) as the"presence of hepatitis B virus(HBV) DNA in the liver(with detectable or undetectable HBV DNA in the serum) of individuals testing hepatitis B surface antigen(HBsag) negative by currently available assays".Several aspects of occult HBV infection are still poorly understood,including the definition itself and a standardized approach for laboratory-based detection,which is the purpose of this review.The clinical significance of OBI has not yet been established;however,in terms of public health,the clinical importance arises from the risk of HBV transmission.Consequently,it is important to detect high-risk groups for occult HBV infection to prevent transmission.The main issue is,perhaps,to identify the target population for screening OBI.Viremia is very low or undetectable in occult HBV infection,even when the most sensitive methods are used,and the detection of the viral DNA reservoir in hepatocytes would provide the best evaluation of occult HBV prevalence in a defined set of patients.However,this diagnostic approach is obviously unsuitable:blood detection of occult hepatitis B requires assays of the highest sensitivity and specificity with a lower limit of detectionsag.

Breeding to Optimize Chinese Agriculture(OPTICHINA) was a three-year EU–China project launched in June of 2011. As designed, the project acted as a new strategic model to reinforce systematic cooperation on agricultural research between Europe and China. The OPTICHINA International Conference "Breeding to Optimize Agriculture in a Changing World" was held in Beijing, May 26–29, 2014. The conference included six thematic areas:(1) defining and protecting the yield potential of traits and genes;(2) high-throughput precision phenotyping in the field;(3) molecular technologies in modern breeding;(4) plant ideotype;(5) data analysis,data management, and bioinformatics; and(6) national challenges and opportunities for China. The 10 articles collected in this special issue represent key contributions and topics of this conference. This editorial provides a brief introduction to the OPTICHINA project, followed by the main scientific points of articles published in this special issue. Finally, outcomes from a brainstorming discussion at the end of the conference are summarized, representing the authors' opinions on trends in breeding for a changing world.

OBJECTIVES Elderly patients show a higher incidence of ischemic and bleeding events after percutaneous transluminal coron-ary intervention(PCI).We sought to investigate outcomes in elderly patients treated with antithrombotic strategy guided by bleeding and ischemic risks after revascularization with last generation everolimus-eluting stent(EES).METHODS Prospective multicenter registry including patients over 75 years revascularized with EES and antithrombotic ther-apy guided by clinical presentation,PCI complexity and PRECISE DAPT score.Co-primary safety endpoints were:(1)composite of cardiac death,myocardial infarction and stent thrombosis and;(2)bleeding(BARC 2-5).Primary efficacy endpoint was target lesion revascularization.A matched group of patients revascularized with current drug-eluting stents and no such tailored antith-rombotic therapy was used as control.RESULTS Finally,1064 patients were included in SIERRA-75 cohort,80.8±4.2 years,36.6%women,71%acute coronary syn-dromes(ACS)and 53.6%complex PCI.Co-primary safety endpoint of major adverse cardiovascular events was met in 6.2%,co-primary safety endpoint of bleeding in 7.8%and primary efficacy endpoint of TKLR in 1.5%.The multivariable adjusted model showed no significant association of the prescribed short/long dual antiplatelet therapy(DAPT)durations with any endpoint suggesting a well tailored therapy.No stent thrombosis reported in the subgroup with 1-3 months DAPT duration.As compared to control group,bleeding BARC 2-5 was significantly lower in SIERRA-75 group(7.4%vs.10.2%,P=0.04)as well as the net safety-efficacy endpoint(14.3%vs.18.5%,P=0.02).CONCLUSIONS In elderly population,the application of this risks-adjusted antithrombotic protocol after revascularization with last generation EES seems to be associated with an improved prognosis in terms of ischemic and bleeding outcomes.

Food security is a global concern in view of the ongoing global （social and climate） change. The challenges to food securityfrom global change, sharply increasing food and feed demands, whereas increasing the frequency and severity of stresses,coupled with degradation and scarcity of natural resources, are indeed urgent and real （Prasanna et al. 2013）. Crop productionmust double by 2050 to meet future production demands （Tilman et al. 2011）. The ability to achieve this increaserepresents a significant challenge to plant breeders. Crop yields must increase at a rate of 2.47o per year, yet the averagerate of increase is only 1.3% with yields stagnating in up to 40% of land under cereal production （Ray et al. 2012, 2013）. Therefore,crops with higher yield potential and more resistant to abiotic stress conditions are needed to address such challenges.

The biochemical and molecular processes involved in the habituation of maize cells to growth in the presence of the cellulose biosynthesis inhibitor dichlobenil （DCB） were investigated. DCB affects the synthesis of cellulose both in active and stationary growth phases and alters the expression of several Cesa genes. Of these, ZmCesa5 and ZmCesa7 seem to play a major role in habituating cells to growth in the presence of DCB. As a consequence of the reduction in cellulose, the expression of several genes involved in the synthesis of hydroxycinnamates is increased, resulting in cell walls with higher levels of ferulic and p-coumaric acids. A proteomic analysis revealed that habituation to DCB is linked to modifications in several metabolic pathways. Finally, habituated cells present a reduction in glutathione S-transferase detoxifying activity and antioxidant activities. Plant cell adaptation to the disturbance of such a crucial process as cellulose biosynthesis requires changes in several metabolic networks, in order to modify cell wall architecture and metabolism, and survive in the presence of the inhibitor. Some of these modifications are described in this paper.

In Spain,the proportion of elderly individuals has increased to 19.1%.By 2068,the proportion of elderly individuals is estimated to increase to 29.4% given the largest birth cohorts in Spain’s history(Instituto Nacional de Estadistica,INE).This challenge needs a supportive plan,as economic and caring problems may occur because of age-related health problems,including cognitive decline and dementia[1].
