Hepatitis B and inflammatory bowel disease: Role of antiviral prophylaxis

Hepatitis B virus (HBV) is a very common infection worldwide. Its reactivation in patients receiving immunosuppression has been widely described as being associated with significant morbidity and mortality unless anti-viral prophylaxis is administered. Treatment in inflammatory bowel disease (IBD) patients has changed in recent years and immunosuppression and biological therapies are now used more frequently than before. Although current studies have reported an incidence of hepatitis B in inflammatory bowel disease patients similar to that in the general population, associated liver damage remains an important concern in this setting. liver dysfunction may manifest in several ways, from a subtle change in serum aminotransferase levels to fulminant liver failure and death. patients undergoing double immunosuppression are at a higher risk, and reactivation usually occurs after more than one year of treatment. As preventive measures, all IBD patients should be screened for HBV markers at diagnosis and those who are positive for the hepatitis B surface antigen should receive antiviral prophylaxis before undergoing immunosuppression in order to avoid HBV reactivation. Tenofovir/entecavir are preferred to lamivudine as nucleos(t)ide analogues due to their better resistance profile. In patients with occult or resolved HBV, viral reactivation does not appear to be a relevant issue and regular DNA determination is recommended during immunosuppression therapy. Consensus guidelines on this topic have been published in recent years. The prevention and management of HBV infection in IBD patients is addressed in this review in order to address practical

Currently immunosuppressive and biological agentsare used in a more extensive and earlier way in patients with inflammatory bowel disease, rheumatic or dermatologic diseases. Although these drugs have shown a significant clinical benefit, the safety of these treatments is a challenge. Hepatitis B virus(HBV) reactivations have been reported widely, even including liver failure and death, and it represents a deep concern in these patients. Current guidelines recommend to preemptive therapy in patients with immunosuppressants in general, but preventive measures focused in patients with corticosteroids and inflammatory diseases are scarce. Screening for HBV infection should be done at diagnosis. The patients who test positive for hepatitis B surface antigen, but do not meet criteria for antiviral treatment must receive prophylaxis before undergoing immunosuppression, including corticosteroids at higher doses than prednisone 20 mg/d during more than two weeks. Tenofovir and entecavir are preferred than lamivudine because of their better resistance profile in long-term immunosuppressant treatments. There is not a strong evidence, to make a general recommendation on the necessity of prophylaxis therapy in patients with inflammatory diseases that are taking low doses of corticosteroids in short term basis or low systemic bioavailability corticosteroids such as budesonide or beclomethasone dipropionate. In these cases regularly HBV DNA monitoring is recommended, starting early antiviral therapy if DNA levels begin to rise. In patients with occult or resolved hepatitis the risk of reactivation is much lower, and excepting for Rituximab treatment, the prophylaxis is not necessary.

Macrobioerosion and Microbioerosion in Marine Molluscan Shells from Holocene and Modern Beaches (39°-40°S, South of Buenos Aires province, Argentina)

The marine sediments of the area of Verde peninsula - Jabali Island （39°28＇S/62°19%V-40°28＇ S/62°11＇W） Holocene in age （3-2 ky）, and modern beaches contain a significant amount of bioeroded mollusc shells. Fifteen sites were analyzed, in which 20.11% of the mollusc shells （2168 valves） presented bioerosion traces, in 54 species （30 bivalves and 24 gastropods）. Fourteen ichnogenera were reported： Entobia, Maeandropolydora, Iramena, Caulostrepsis, pennatichnus, pinaceocladichnus, Trypanites, and Gastrochaenolites （Domichnia）, Gnathichnus and Radulichnus （pascichnia）, Finichnus and Centrichnus （Fixichnia）, Oichnus （praedicnia）（macrobioerosion）, y Semidendrina （microbioerosion）, the latter is first reported in mollusc shells in Argentina. Eleven ichnospecies were identified Finichnus peristroma, Maeandropolydora sulcans, Gnathichnus pentax, pinaceocladichnus onubensis, Caulostrepsis taeniola, Centrichnus eccentricus, Radulichnus inopinatus, Oichnus simplex, Oichnus paraboloides, Oichnus gradatus, and Gastrochaenolites torpedo （lithic remains）. The dominant ichnogenera in the Holocene deposits are Iramena, Entobia and Oichnus. The same ichnogenera are constant with different abundance in the modern beaches, and increasing representation of pinaceocladichnus and pennatichnus. The dominant ichnofacies in the Holocene deposits is Trypanites, revealing a benthonic marine community composed of cheilostome bryzoans, clionaid sponges, predator gastropods, regular echinoids, polychaete annelids, bivalves, thallophytas and fungi. Generally, the area was described as a sublittoral, low-energy, stable environment with high rate of oxygenation, and sandy bottoms, with rocky bottoms at Villalonga locality.

The late pleistocene littoral ridges of southern South America,especially those of the north of Argentinean patagonia,contain remains of mollusk shells with bioerosion *** sites from marine isotope stages(2 from MIS 7,4 from MIS 5 e)and five sites from modern beaches from northern Río Negro province,with 40 taxa(17 bivalves and 23 gastropods)were analyzed,in the area between west of Baliza San Matías and las Grutas(41°S).Three ichnogenera were identified in deposits of MIS 7:Entobia,Gastrochaenolites(Domichnia)and Oichnus(praedichnia)with one ichnospecies(***).Six ichnogenera were identified in deposits of MIS 5 e:Entobia,Meandropolydora,pinaceocladichnus,Iramena,Caulostrepsis and Oichnus with one ichnospecies(***).Seven ichnogenera were identified from modern beaches:Entobia,Meandropolydora,Iramena,Caulostrepsis,pinaceocladichnus,Gastrochaenolites(Domichnia)and Oichnus(praedicnia),with two ichnospecies(*** and ***).On this basis,it is inferred that pleistocene benthic communities were constituted from cheilostome bryzoans and polychaete annelids,together with drilling bivalves,predatory gastropods and overall warm-water species of *** these,Chama iudicai was found on plicatula gibbosa from MIS 7 and MIS 5 *** record of warm temperature species suggests a higher sea surface temperature(SST)than the present one and/or the influence of the warm southward Brazilian Current affecting the waters of Golfo San Matí*** the modern beaches surveyed,there is an increase in polychaete annelids,cheilostome bryzoans,temperate-cold species and drilling bivalves,as well as sandy substrate species,which together with an increase of Gastrochaenolites and Caulostrepsis,suggest a colder climate due to the greater influence of the Malvinas Current during the Holocene.

AIM:To investigate whether we could create natural autologous tissue patches in the subcutaneous space for organ repair. METHODS: We implanted the following three types of inert foreign bodies in the subcutaneous tissue of rats to produce autologous tissue patches of different geometries:(1) a large-sized polyvinyl tube(l = 25 mm,internal diameter = 7 mm) sealed at both ends by heat application for obtaining a large flat piece of tissue patch for organ repair;(2) a fine polyvinyl tubing(l = 25 mm,internal diameter = 3 mm) for creating cylindrically shaped grafts for vascular or nerve repair; and(3) a slurry of polydextran particle gel for inducing a bladder-like tissue. Implantation of inert materials was carried out by making a small incision on one or either side of the thoracic-lumbar region of rats. Subcutaneous pockets were created by blunt dissection around the incision into which the inert bodies were inserted(1 or 2 per rat). The incisions were closed with silk sutures,and the animals were allowed to recover. In case of the polydextran gel slurry 5 m l of the slurry was injected in the subcutaneous space using an 18 gauge needle. After implanting the foreign bodies a newly regenerated encapsulating tissue developed around the foreign bodies. The tissues were harvested after 4-42 d of implantation and studied by gross examination,histology,and histochemistry for organization,vascularity,and presence of mesenchymal stem cells(MSCs)(CD271+CD34+ cells). RESUlTS: Implanting a large cylindrically shaped polyvinyl tube resulted in a large flat sheet of tissue that could be tailored to a specific size and shape for use as a tissue patch for repairing large organs. Implanting a smaller sized polyvinyl tube yielded a cylindrical tissue that could be useful for repairing nerves and blood vessels. Juvenile hormone （JH）, a sesquiterpenoid synthetized by the insect corpora allata （CA）, plays critical roles in metamorphosis and reproduction. penultimate or last step of JH synthesis is catalyzed by juvenile hormone acid O-methyltransferase （JHAMT）. Here we report the cloning and expression analysis of the JHAMT orthologue in the cockroach, Blattella germanica （l.）（BgJHAMT）. BgJHAMT is mainly expressed in CA, with only expression traces in ovary. Three different isoforms, differing in the 3＇-UTR sequence, were identified. Isoform A shows between 35 and 65 times higher expression than B and C in CA from penultimate nymphal instar and adult females. RNAi-triggered knock down of BgJHAMT produces a dramatic reduction of JH synthesis, concomitant with a decrease of fat body viteUogenin expression and basal follicle length. BgJHAMT mRNA levels in CA of females along the gonadotrophic cycle parallel, with a slight advancement, JH synthesis profile. BgJHAMT mRNA levels were reduced in starved females and in females in which we reduced nutritional signaling by knocking down insulin receptor and target of rapamycin （TOR）. Results show that conditions that modify JH synthesis in adult B. germanica females show parallel changes of BgJHAMT mRNA levels and that the JH-specifie branch of the JH synthesis pathway is regulated in the same way as the mevalonate branch. Furthermore, we demonstrate that nutrition and its signaling through the insulin receptor and TOR pathways are essential for activating BgJHAMT expression, which suggests that this enzyme can be a checkpoint for the regulation of JH production in relation to nutritional status.
