This manuscript collects in a joint and orderly manner the existing evidence at the present time about postoperative treatment with radiotherapy in non-small cell lung *** also systematically reviews the current evidence,the international recommendations in the most relevant guidelines,the most controversial aspects in clinical and pathological staging,the specific technical aspects of radiotherapy treatment,and also collects all the potential risk factors that have been postulated as significant in the prognosis of these patients,evaluating the possibility of segmenting a particularly sensitive subpopulation with a high risk of relapse on which an adjuvant treatment with radiotherapy could have an impact on their clinical ***,currently active trials that aspire to provide more evidence on this topic are reviewed.

Complete resection continues to be the gold standard for the treatment of earlystage lung *** landmark Lung Cancer Study Group trial in 1995 established lobectomy as the minimum intervention necessary for the management of early-stage non-small cell lung cancer,as it was associated with lower recurrence and metastasis rates than sublobar resection and lower postoperative morbidity and mortality than *** is a growing tendency to perform sublobar resection in selected cases,as,depending on factors such as tumor size,histologic subtype,lymph node involvement,and resection margins,it can produce similar oncological results to *** treatments such as stereotactic body radiotherapy and radiofrequency ablation can also produce good outcomes in inoperable patients or patients who refuse surgery.

Immunotherapy has represented one of the main medical revolutions of recent decades,and is currently a consolidated treatment for different types of tumors at different stages and scenarios,and is present in a multitude of clinical *** of the diseases in which it is most developed is non-small cell lung *** combination of radiotherapy and immunotherapy in cancer in general and lung cancer in particular currently represents one of the main focuses of basic and clinical research in oncology,due to the synergy of this interaction,which can improve tumor response,resulting in improved survival and disease *** this review we present the biochemical and molecular basis of the interaction between radiotherapy and *** also present the current clinical status of this interaction in each of the stages and cases of non-small cell lung cancer,with the main results obtained in the different studies both in terms of tumor response and survival as well as ***,we mention the main studies underway and the challenges of this interaction in the coming years,including how these treatments should be combined to achieve the greatest efficacy with the fewest possible side effects(dose,type of radiotherapy and drugs,sequence of treatments).

To reduce solar gains,regions of the sky dome are masked,where the solar path will pass in the moments in which the shade will be *** these regions masked by the shadow devices could also provide diffuse lighting,reducing the need for daytime artificial *** a shadow device is perfectly designed it does not mask more sky than necessary avoiding unwanted solar *** the early stages of architectural design many decisions that will affect the project’s energy efficiency have not yet been made and information is lacking to carry out realistic energy or daylighting *** objective was to define an index to qualify,from minimum input data,the effectiveness of a shaded opening by maximizing the visibility to the sky while minimizing undesirable unshaded solar *** basic 3D models and AutoLISP algorithms,it was possible to calculate the aforementioned *** results demonstrate that similar shadow devices present variable Solar Coupling Indices,explaining why the pre-design of an efficient shadow device is an unintuitive *** method allows quantifying the gap between the best solar performance achievable and the performance achieved by a specific solar control device,helping to increase the energy efficiency of the architectural enclosures from the early design stages.

描述了在马德里的西班牙众议院会议厅半圆形建筑中检测到的声学问题,给出了解决该问题的步骤和流程。为了量化混响时间、快速语言传输指数(RASTI)及礼堂内辅助系统产生背景噪声级的声学特征,在半圆形建筑空场中做了一些声学测量。建立了一个仿真模型并通过在空场条件中的测量进行了验证,当开会期间只允许进行声压级测量情况下,可以用该模型来模拟半圆形建筑在开会期间的声学指标。考虑了多种不同方面来优化会议期间的语言清晰度,提出和模拟过的建议主要包括降低会议期间的背景噪声级和提升扩声系统。

Severe Acute Respiratory Syndrome Coronavirus(SARS-CoV)-2 infection induces an exacerbated inflammation driven by innate immunity *** cells(DCs)play a key role in the defense against viral infections,for instance plasmacytoid DCs(pDCs),have the capacity to produce vast amounts of interferon-alpha(IFN-α).In COVID-19 there is a deficit in DC numbers and IFN-αproduction,which has been associated with disease *** this work,we described that in addition to the DC deficiency,several DC activation and homing markers were altered in acute COVID-19 patients,which were associated with multiple inflammatory ***,previously hospitalized and nonhospitalized patients remained with decreased numbers of CD1c+myeloid DCs and pDCs seven months after SARS-CoV-2 ***,the expression of DC markers such as CD86 and CD4 were only restored in previously nonhospitalized patients,while no restoration of integrinβ7 and indoleamine 2,3-dyoxigenase(IDO)levels were *** findings contribute to a better understanding of the immunological sequelae of COVID-19.

目的:探讨和血明目片联合全视网膜激光光凝术(PRP)对糖尿病视网膜病变(DR)患者视力状况、血液流变学及脉络膜厚度的影响。方法:选择2018年3月~2019年12月期间来安徽省第二人民医院接受治疗的60例DR患者,经计算机随机编号按奇偶顺序分为对照组(奇数,n=30)和研究组(偶数,n=30),两组在接受常规降糖治疗的基础上,对照组患者接受PRP治疗,研究组患者接受和血明目片联合PRP治疗,对比两组临床总有效率,观察两组治疗前后的视力状况、血液流变学及脉络膜厚度变化。结果:研究组的临床总有效率高于对照组(P<0.05)。治疗后,两组裸眼视力、视野、视敏度均升高,研究组的升高程度大于对照组(P<0.05)。治疗后,两组血浆黏度(PV)、红细胞聚集指数(AI)、全血高切黏度(NBH)、全血低切黏度(NBL)、血沉(ESR)均下降,研究组的下降程度大于对照组(P<0.05)。治疗后,两组黄斑中心凹下脉络膜厚度(SFCT)、下方脉络膜厚度(ICT)、上方脉络膜厚度(SCT)、鼻侧脉络膜厚度(NCT)、颞侧脉络膜厚度(TCT)均下降,研究组的下降程度大于对照组(P<0.05)。两组均未见明显的不良反应发生。结论:和血明目片联合PRP可有效改善DR患者的视力状况、血液流变学及脉络膜厚度。

Patients with early-stage non-small-cell lung cancer(NSCLC)are candidates for curative surgery;however,despite multiple advances in lung cancer management,recurrence rates remain *** chemotherapy has been demonstrated to significantly prolong overall survival(OS),but this benefit is modest and there is an urgent need for effective new therapies to provide a cure for more *** high efficacy of tyrosine kinase inhibitors(TKIs)against epidermal growth factor receptor-mutated(EGFR)in patients with advanced EGFR-mutated NSCLC has led to the evaluation of these agents in early stages of the *** clinical trials have evaluated the safety and efficacy of EGFR TKIs as an adjuvant treatment,in patients with resected EGFR-mutated NSCLC,and shown that they significantly prolong disease-free survival(DFS),but this benefit does not translate to ***,an interim analysis of the ADAURA trial demonstrated that,surprisingly,osimertinib improved *** led to the study being stopped early,leaving many unanswered questions about its potential effect on OS and its incorporation as a standard adjuvant treatment in this patient *** targeted agents are also being evaluated in locally-advanced disease,with promising results,although prospective studies with larger sample sizes are needed to confirm these *** this article,we review the most relevant studies on the role of EGFR TKIs in the management of early-stage EGFR-mutated NSCLC.
