Five Year Follow up after Surgical Treatment of Type 2 Diabetes with Laparoscopic Sleeve Gastrectomy Associated with a Duodenal Ileal Interposition

Introduction: Type 2 diabetes mellitus (T2D) is a chronic disease, primarily caused by a combination of defective insulin secretion and the inability of insulin-sensitive tissues to respond to insulin. Laparoscopic sleeve gastrectomy associated with duodenal ileal interposition (SGDII) has been shown to be a feasible treatment option for patients with T2D, as it provides improvement and control of glycemia, dyslipidemia and arterial hypertension. The aim of this study was to evaluate the mid and long-term results of SGDII for the treatment of diabetic patients, considering diabetic remission, weight loss and postoperative complications. Materials and Methods: Retrospective study with 96 patients with T2D submitted to SGDII, between 2010 and 2016. The glycated hemoglobin (HbA1c) value Results: Sixty-one patients (62.8%) were male, and 36 patients (37.2%) were female. The average age was 50.9 years. Median bMI was 33.43 kg/m2. The incidence of major postoperative complications in the first 30 days was 2.1%, with no mortality. Dyslipidemia control was sustained, after five years, in 62% of the patients. The five years follow-up demonstrated that 80% of patients had T2D remission at one year, 74.5% at three years and 61.8% at five years. Univariate analysis demonstrated that preoperative values of HbA1c and bMI, preoperative use of insulin, gender, and 30-day complication were not predictors of remission at all study intervals. The average duration of the disease was nine years and the mean glycated hemoglobin before surgery was 8.95%. Conclusion: SGDII resulted in good glycemic control at 5-years follow-up and represents a valid alternative for the treatment of T2D.

INTRODUCTION Seed dispersal is a key process in the life cycle of most plants,with ecological and evolutionary consequences that go beyond vegetation dynamics and cascade to entire ecosystem functioning and stability(Traveset et al.2014).Seed dispersal largely constrains plant recruitment and the distribution of future plant cohorts,thus playing a critical role in the restoration of disturbed sites(Cain et al.2000;Higgins et al.2003).It can also threaten plant diversity by facilitating plant invasions(Vavra et al.2007;Heleno 2020).

Evaluation and modeling of runoff and sediment yield for different land covers under simulated rain in a semiarid region of brazil

This paper quantifies the runoff and sediment yield for four different land covers in a semiarid region of brazil. The WESp model, a distributed, event-oriented runoff-erosion model, was applied and its physical parameters, NSand KR, were adjusted based on observed runoff and sediment yield data using simulated rainfall with an average intensity of 53 mm h-1. The sediment yield obtained was 53.02 kg ha-1(caatinga vegetation), 231.96 kg ha-1(bare soil), 309.75 kg ha-1(beans), and 847.38 kg ha-1(corn). The results showed that caatinga cover yields the lowest erosion and runoff when compared to the other *** results also show that the sediment yield and runoff values simulated with NS, KI, and KR parameters were well calibrated, within acceptable deviations. The caatinga vegetation was more effective in protecting the soil, when compared to the other types of coverage. The beans and corn covers had the highest values of runoff and sediment yield, even higher than those observed for bare soil.

Zika virus (ZIKV), a mosquito-borne flavivirus, has been associated with benign infections for decades. However, it has become a public health concern due to its association with severe fetal and neurological complications. Although many efforts have been made to control ZIKV infection, approved vaccines or antiviral drugs are still lacking. Consequently, the development of new effective anti-ZIKV agents is urgently needed. In this context, we investigated the antiviral potential of pyrazolone derivatives against ZIKV replication using in silico and in vitro methods. The four pyrazolone derivatives evaluated (1a, 1b, 1c, and 1d) inhibited over 50% of ZIKV replication with low cytotoxicity. Among them, compound 1b exhibited the most potent activity (EC50 = 4.3 μM) and the highest selectivity (SI = 342). Mechanism of action studies indicated that these compounds act at early stages of virus replication, and compound 1b can also directly inactivate ZIKV particles. Molecular docking studies suggested that these compounds can bind to and block the activity of ZIKV NS5 methyltransferase. Finally, pharmacokinetic and toxicological predictions have reinforced the safety and drug-like profiles of these derivatives. In conclusion, the pyrazolone scaffold proved to be valuable for anti-ZIKV drug development, and the derivatives studied deserve further investigation.

Induced pluripotent stem cells(ipSC)technology has propelled the field of stem cells biology,providing new cells to explore the molecular mechanisms of pluripotency,cancer biology and aging.A major advantage of human ipSC,compared to the pluripotent embryonic stem cells,is that they can be generated from virtually any embryonic or adult somatic cell type without destruction of human *** addition,ipSC can be generated from somatic cells harvested from normal individuals or patients,and used as a cellular tool to unravel mechanisms of human development and to model diseases in a manner not possible *** these fundamental aspects of human biology and physiology that are revealed using ipSC or ipSC-derived cells,these cells hold an immense potential for cell-based therapies,and for the discovery of new or personalized pharmacological treatments for many ***,we review some of the current challenges and concerns about ipSC *** introduce the potential held by ipSC for research and development of novel health-related *** briefly present the efforts made by the scientific and clinical communities to create the necessary guidelines and regulations to achieve the highest quality standards in the procedures for ipSC generation,characterization and long-term ***,we present some of the audacious and pioneer clinical trials in progress with ipSC-derived cells.

Exploring the role of mononuclear phagocytes in the epididymis

The onslaught of foreign antigens carried by spermatozoa into the epididymis, an organ that has not demonstrated immune privilege, a decade or more after the establishment of central immune tolerance presents a unique biological challenge. Historically, the physical confinement of spermatozoa to the epididymal tubule enforced by a tightly interwoven wall of epithelial cells was considered sufficient enough to prevent cross talk between gametes and the immune system and, ultimately, autoimmune destruction. The discovery of an intricate arrangement of mononuclear phagocytes （Mps） comprising dendritic cells and macrophages in the murine epididymis suggests that we may have underestimated the existence of a sophisticated mucosal immune system in the posttesticular environment. This review consolidates our current knowledge of the physiology of Mps in the steady state epididymis and speculates on possible interactions between auto-antigenic spermatozoa, pathogens and the immune system by drawing on what is known about the immune system in the intestinal mucosa. Ultimately, further investigation will provide valuable information regarding the origins of pathologies arising as a result of autoimmune or inflammatory responses in the epididymis, including epididymitis and infertility.

Litter production and decomposition are critical to forest productivity,nutrient cycling,and carbon seques-tration in tropical woody ***,nutrient release and leaf litter stoichiometry in tropical legume tree plantations over the long term after outplanting are poorly understood or even *** improving our under-standing of the pattern of changes in the decomposition of N-fixing leaf litters and their possible impact on carbon stor-age,we measured litter production,mass loss and nutrient release for 240 d during litter decomposition for two tropical legume tree species(plathymenia reticulata and Hymenaea courbaril),in Rio de Janeiro,*** production for *** was 5.689 kg ha^(−1) a^(−1) and 3.231 kg ha^(−1) a^(−1) for *** patterns of mass loss rates were similar;however,nutrient release was greater for ***,while *** showed immobilization of nutrients,especially for N,which increased by almost 20%in the early phase of decomposition followed by gradual *** from the N-fixing species did differ in nutrient chemistries over time,which was not surprising given that initial nutrient concentrations varied broadly,except for C and *** of the nutrient concentrations increased as the remaining litter mass decreased in both species,except for C and *** C:N and N:p ratios differed between the species,but N:p did not correlate to mass *** species had N-rich leaves,but *** decomposition was very likely p-limited,while *** seemed to be co-limited by N and *** results showed different patterns in nutrient release and the stoichi-ometry involved in the decomposition dynamics of the two tropical N-fixing species,even though they have similar litter decay *** species,but especially ***,may help re-establish nutrient cycling in disturbed ecosystems.

Assessing tree crown fire damage integrating linear spectral mixture analysis and supervised machine learning on Sentinel-2 imagery
