BACKGROUND Metastatic pancreatic ductal adenocarcinoma(PDAC)is a lethal malignancy with dispiriting survival *** is a promising approach to many cancer types,but achieves poor outcomes in advanced PDAC due to its immunosuppressive tumor *** describe a case of metastatic PDAC effectively treated with *** SUMmarY We report the case of a 67-year-old woman with unresectable locally advanced PDAC,treated with gemcitabine plus nab-paclitaxel followed by radiotherapy plus *** nine months,pancreatic tumor progression was observed at the level of the hepatic hilum with the appearance of a new pulmonary nodule suggestive of a second primary,confirmed by left lung *** immunotherapy was then initiated with pembrolizumab,an immune checkpoint inhibitor targeting programmed cell death protein-1 that covers the two tumor *** patient showed a complete metabolic response that was maintained throughout the *** patient continues to be disease-free at 5.6 years since the start of *** These results suggest that the administration of pembrolizumab after chemoradiotherapy has a beneficial effect in patients with metastatic *** our knowledge,this is the first reported case of a patient with metastatic PDAC and metastatic lung cancer showing such a long-lasting complete response after pembrolizumab treatment without curative *** studies are required to determine biomarkers that identify PDAC patients most likely to benefit from this immunotherapy.

Evaluation of Hemoglobin and Serum Erythropoietin Levels in Patients with Polycythemia Vera and Secondary Polycythemia

Objective: This study compares hemoglobin and erythropoietin levels in patients with polycythemia vera and secondary polycythemia. Study Design: A retrospective cross-sectional study evaluating the serum erythropoietin and hemoglobin levels in patients with polycythemia vera and secondary polycythemia. This study was performed simultaneously in Texas state of the U.S. and Fars Province in Iran. Methods: Hemoglobin, hematocrit and erythropoietin test results were collected from patients aged 19 to 75 years who were diagnosed with polycythemia vera and secondary polycythemia. Patients records with history of thrombocythemia, congestive heart failure, dyspnea, anemia and pregnant woman were excluded from study. Patients in each decade of life were examined in separate groups, so that changes in hemoglobin related to aging did not affect the research results. Results: 75% of the patients were men, and 25% were women. A total of 1580 patients were analyzed in this study. 57.3% of patients in UTMB and 38.8 patients in Iran have hemoglobin level above 17 mg/dl. 74% of patients in UTMB and 88% of patients in Iran have erythropoietin below 10 IU/mL. Polycythemia in UTMB was more common in people over 50 and in Iran in patients under 50 years old. The serum hemoglobin and erythropoietin levels in patients with polycythemia vera were not significantly different in compare to secondary polycythemia patients. Data showed that there were 84 polycythemia patients per 100,000 people. The results of this study in UTMB and Iran showed that 4.5% and 7%, respectively, of patients with polycythemia had a positive JAK2 test. Conclusion: Low erythropoietin levels may not be helpful in differentiating polycythemia vera from secondary polycythemia. .

Long-term monitoring reveals the effect of precipitation and silviculture on Nothofagus regeneration in Northern Patagonia mixed forests

Background Tree regeneration is a key component of resilience because it promotes post-disturbance recovery of forests. Northwestern Patagonia from Argentina is occupied by Nothofagus alpina（Na）, N. obliqua（No）, and N. dombeyi（Nd） forests that grow along an intense precipitation gradient, managed throughout shelterwood silvicultural system by technicians of the Lanin Natural Reserve. The objective of this study was to evaluate the influence of seeding cuttings over the dynamics of Nothofagus mixed forests across landscape（precipitation gradient） based mainly on the composition and abundance of tree regeneration, permanent sampling plots and generalized linear mixed models. In particular, we analysed:（i） the structure of sexual and asexual regeneration during 20 years after harvest（the dynamics of managed forests）, and（ii） the structure of sexual regeneration in primary and managed forests after > 20 years of harvest（the effect of silviculture）.Results Nd was the most abundant species in the regeneration of managed forests during all periods in both sites despite its lower contribution to the adult cohort. During the 10–20 years period after harvest, the humid site exhibited higher regeneration density than the mesic site(120,000 and 6000 ind ha-1, respectively), and it decreased afterwards. The number of established regeneration（> 2 m height） was lower for Na in the mesic site and for No in the humid site(0 and 57 ind ha-1, respectively). However, in comparison to No, Na showed a higher number of sprouted stumps and sprouts per stump, and a higher sprout height in the mesic site. No exhibited higher sprout mortality in the humid site. Chitosan Nanoparticles as Biostimulant in Lettuce (Lactuca sativa L.) Plants

Biodegradable nanoparticles such as chitosan nanoparticles (CSNPs) are used in sustainable agriculture since theyavoid damage to the environment;CSNPs have positive effects such as the accumulation of bioactive compoundsand increased productivity in plants. This study aimed to investigate the impact of applying CSNPs on lettuce,specifically focusing on enzymatic activity, bioactive compounds, and yield. The trial was conducted using a completelyrandomized design, incorporating CSNPs: 0, 0.05, 0.1, 0.2, 0.4, and 0.8 mg mL−1. The doses of 0.4 mg mL−1improve yields up to 24.6% increases and 0.1 mg mL−1 of CSNPs increases total phenols by 31.2% and antioxidantcapacity by 34.6%. In addition, when low concentrations of CSNPs (0.05 and 0.1 mg L−1) were applied, anincrease in catalase was determined. The CSNPs represent a good alternative to be used as a biostimulant in sustainableagriculture because they improve the yield and quality of lettuce by increasing the bioactive compounds.

BACKGROUND Esophagopericardial fistula(EPF)is a rare,life-threatening condition with limited scientific literature and no established management *** case report highlights a successful multidisciplinary approach and the innovative use of endoscopic vacuum assisted closure(endoVAC)therapy in treating this complex *** SUMmarY A 16-year-old male with a history of esophageal atresia and colon interposition presented with progressive chest pain,fever,and *** revealed an EPF with associated pleural and pericardial *** management with an esophageal stent failed,prompting the use of an endoVAC *** patient underwent multiple endoVAC device changes and received broad-spectrum antibiotics and nutritional *** fistula successfully closed,and the patient recovered,demonstrating no new symptoms at a 6-month *** EndoVAC therapy can effectively manage EPF,providing a minimally invasive treatment option.

This research work relates the surface of a square and the area circumscribed by a circle, resulting in a value called Nikola Tesla constant. This constant starts with the calculation of the areas of the square and the inscribed circle, giving a ratio of 9/40 and from which a residual area of the area proportions of the geometric figures described is obtained. Plotting smooth curves, particularly those in round shapes, can be represented efficiently with the use of Nikola Tesla constant, reducing complex mathematical calculus.

When data privacy is imposed as a necessity,Federated learning(FL)emerges as a relevant artificial intelligence field for developing machine learning(ML)models in a distributed and decentralized *** allows ML models to be trained on local devices without any need for centralized data transfer,thereby reducing both the exposure of sensitive data and the possibility of data interception by malicious third *** paradigm has gained momentum in the last few years,spurred by the plethora of real-world applications that have leveraged its ability to improve the efficiency of distributed learning and to accommodate numerous participants with their data *** virtue of FL,models can be learned from all such distributed data sources while preserving data *** aim of this paper is to provide a practical tutorial on FL,including a short methodology and a systematic analysis of existing software ***,our tutorial provides exemplary cases of study from three complementary perspectives:i)Foundations of FL,describing the main components of FL,from key elements to FL categories;ii)Implementation guidelines and exemplary cases of study,by systematically examining the functionalities provided by existing software frameworks for FL deployment,devising a methodology to design a FL scenario,and providing exemplary cases of study with source code for different ML approaches;and iii)Trends,shortly reviewing a non-exhaustive list of research directions that are under active investigation in the current FL *** ultimate purpose of this work is to establish itself as a referential work for researchers,developers,and data scientists willing to explore the capabilities of FL in practical applications.
