Usefulness of Impella support in different clinical settings in cardiogenic shock

BACKGROUND The Impella pump has emerged as a promising tool in patients with cardiogenic shock(CS).Despite its attractive properties,there are scarce data on the specific clinical setting and the potential role of Impella devices in CS patients from routine clinical practice.METHODS This is an observational,retrospective,single center,cohort study.All consecutive patients with diagnosis of CS and undergoing support with Impella 2.5?,Impella CP?or Impella 5.0?from April 2015 to December 2020 were included.Baseline characteristics,management and outcomes were assessed according to CS severity,age and cause of CS.Main outcome measured was in-hospital mortality.RESULTS A total of 50 patients were included(median age:59.3±10 years).The most common cause of CS was acute coronary syndrome(ACS)(68%),followed by decompensation of previous cardiomyopathy(22%).A total of 13 patients(26%)had profound CS.Most patients(54%)improved pulmonary congestion at 48 h after Impella support.A total of 19 patients(38%)presented significant bleeding.In-hospital mortality was 42%.Among patients with profound CS(n=13),five patients were previously supported with venoarterial extracorporeal membrane oxygenation.A total of eight patients(61.5%)died during the admission,and no patient achieved ventricular recovery.Older patients(≥67 years,n=10)had more comorbidities and the highest mortality(70%).Among patients with ACS(n=34),35.3%of patients had profound CS;and in most cases(52.9%),Impella support was performed as a bridge to recovery.In contrast,only one patient from the decompensated cardiomyopathy group(n=11)presented with profound CS.In 90.9%of these cases,Impella support was used as a bridge to cardiac transplantation.There were no cases of death.CONCLUSIONS In this cohort of real-life CS patients,Impella devices were used in different settings,with different clinical profiles and management.Despite a significant rate of complications,mortality was acceptable and lower than those observed in other series.

Genome-wide transcriptome profiling identifies genes that are prone to differential expression(DE)across contexts,as well as genes with changes specific to the experimental manipulation.Distinguishing genes that are specifically changed in a context of interest from common differentially expressed genes(DEGs)allows more efficient prediction of which genes are specific to a given biological process under scrutiny.Currently,common DEGs or pathways can only be identified through the laborious manual curation of experiments,an inordinately time-consuming endeavor.Here we pioneer an approach,Specific cOntext Pattern Highlighting In Expression data(SOPHIE),for distinguishing between common and specific transcriptional patterns using a generative neural network to create a background set of experiments from which a null distribution of gene and pathway changes can be generated.We apply SOPHIE to diverse datasets including those from human,human cancer,and bacterial pathogen Pseudomonas aeruginosa.SOPHIE identifies common DEGs in concordance with previously described,manually and systematically determined common DEGs.Further molecular validation indicates that SOPHIE detects highly specific but low-magnitude biologically relevant transcriptional changes.SOPHIE’s measure of specificity can complement log2 fold change values generated from traditional DE analyses.For example,by filtering the set of DEGs,one can identify genes that are specifically relevant to the experimental condition of interest.Consequently,these results can inform future research directions.All scripts used in these analyses are available at https://***/greenelab/generic-expression-patterns.Users can access https://***/greenelab/sophie to run SOPHIE on their own data.

Outcomes after robot-assisted laparoscopic radical prostatectomy

Robot-assisted laparoscopic radical prostatectomy （RALRP） using the da Vinci surgical system is now in widespread use in many countries where economic conditions allow the installation of this expensive technology. Controversy has surrounded the procedure since it was first performed in 2000, with many critics highlighting the lack of evidence to support its use. However, despite the lack of level I evidence, many large studies of patients have confirmed that the procedure is feasible and safe, with low morbidity. Available longer-term oncological data seem to show that outcomes from the robotic approach at least match those of traditional open radical prostatectomy. Functional outcomes also seem satisfactory, although randomized controlled trials are lacking. This paper reviews the current status of RALRP with respect to perioperative data and complications and oncologic and functional outcomes.

China’s 13th Five-Year Plan elevated the national mandate for environmental sustainability.Chinese fisheries are characterized by full retention of high diversity catch harvested using unselective gears,creating ecological risks.Therefore,China launched pilot projects in management by Total Allowable Catch(TAC)in five coastal provinces in 2017 and 2018 to build experience with output controls.Fujian province launched an important pilot in its swimming crab fishery,the first to adopt a multispecies approach.To guide Fujian and other provinces in multispecies management,a workshop in April 2018 shared international experience.The workshop considered 13 case studies spanning a wide range of underlying scientific models and types of harvest controls.Multispecies harvest controls based on simple survey-or index-based models that aggregate trends for many species are typically operationally easier for managers and fishers.However,inadequate management can cause declines of individual species,sometimes leading to adoption of species-specific models and then species-specific harvest controls.This transition often incurs economic costs through scientific and management demands,and constraints on harvest of co-occurring species.The lessons revealed by the case studies suggest multispecies TACs might be effective in the Fujian swimming crab fishery given the modest number of species with similar and productive life history traits,and the market demand for all species.Continued experimentation with different management approaches through pilot projects can enable China to maintain progress toward sustainable fisheries goals under the 14th Five-Year Plan.

The role of optical coherence tomography in neuro-ophthalmology

Optical coherence tomography(OCT)is an ocular imaging technique that can complement the neuro-ophthalmic assessment,and inform our understanding regarding functional consequences of neuroaxonal injury in the afferent visual pathway.Indeed,OCT has emerged as a surrogate end-point in the diagnosis and follow up of several demyelinating syndromes of the central nervous system(CNS),including optic neuritis(ON)associated with:multiple sclerosis(MS),neuromyelitis optica spectrum disorder(NMOSD),and anti-myelin oligodendrocyte glycoprotein(MOG)antibodies.Recent advancements in enhanced depth imaging(EDI)OCT have distinguished this technique as a new gold standard in the diagnosis of optic disc drusen(ODD).Moreover,OCT may enhance our ability to distinguish cases of papilledema from pseudopapilledema caused by ODD.In the setting of idiopathic intracranial hypertension(IIH),OCT has shown benefit in tracking responses to treatment,with respect to reduced retinal nerve fiber layer(RNFL)measures and morphological changes in the angling of Bruch’s membrane.Longitudinal follow up of OCT measured ganglion cell-inner plexiform layer thickness may be of particular value in managing IIH patients who have secondary optic atrophy.Causes of compressive optic neuropathies may be readily diagnosed with OCT,even in the absence of overt visual field defects.Furthermore,OCT values may offer some prognostic value in predicting post-operative outcomes in these patients.Finally,OCT can be indispensable in differentiating optic neuropathies from retinal diseases in patients presenting with vision loss,and an unrevealing fundus examination.In this review,our over-arching goal is to highlight the potential role of OCT,as an ancillary investigation,in the diagnosis and management of various optic nerve disorders.

Tetracycline-inducible protein expression in pancreatic cancer cells: Effects of CapG overexpression

AIM: To establish stable tetracycline-inducible pancreatic cancer cell lines. METHODS: Suit-2, MiaPaca-2, and Panc-1 cells were transfected with a second generation reverse tetracycline-controlled transactivator protein (rtTA2S-M2), under the control of either a cytomegalovirus (CMV) or a chicken β-actin promoter, and the resulting clones were characterised.RESULTS: Use of the chicken (β-actin) promoter proved superior for both the production and maintenance of doxycycline-inducible cell lines. The system proved versatile, enabling transient inducible expression of a variety of genes, including GST-P, CYP2E1, S100A6, and the actin capping protein, CapG. To determine the physiological utility of this system in pancreatic cancer cells, stable inducible CapG expressors were established. Overexpressed CapG was localised to the cytoplasm and the nuclear membrane, but was not observed in the nucleus. High CapG levels were associated with enhanced motility, but not with changes to the cell cycle, or cellular proliferation. In CapG-overexpressing cells, the levels and phosphorylation status of other actin-moduating proteins (Cofilin and Ezrin/Radixin) were not altered. However, preliminary analyses suggest that the levels of other cellular proteins, such as ornithine aminotransferase and enolase, are altered upon CapG induction. CONCLUSION: We have generated pancreatic-cancer derived cell lines in which gene expression is fully controllable.

Uranium Series Disequilibrium and Precision Measurement of Radionuclides Activity in Sediment Sample Using Low Background Gamma Spectrometry

Low background gamma spectrometry was used to measure the radionuclides activity of 238U, 232Th, and 235U series as well as 40K and 137Cs in a sediment sample. The goal of the study was to measure the 238U (63.3 keV peak of 234Th;1001 keV peak of 234mPa) and 235U (143.76 keV, 163.33 keV, and 205.31 keV peaks) activity by low background gamma spectrometry in sediment sample. 235U activity in environmental samples is difficult to accurately measure by gamma spectrometry due to its low abundance in nature and low gamma line intensities at 143.76 keV, 163.33 keV, and 205.31 keV. We have shown that by using low background gamma spectrometry, it is possible to accurately measure the 235U activity in sediment samples. The 235U activity was measured without using the major peak of 185.7 keV (Iγ = 57.2%) which requires interference correction from 186.21 keV of 226Ra. 226Ra activity was determined by measuring 222Rn daughters (214Pb and 214Bi). The precision and accuracy of the gamma activity measurement in the sediment sample were verified by using the HPGe detectors with Certified Reference Material (CRM) Irish Sea Sediment (IAEA-385). The results obtained for the 63.33 keV energy line of 234Th are compared with the 1001 keV energy line of 234mPa. The values of 238U and 235U activities, as well as 40K, 137Cs, and 226Ra, agreed with the certificate values of CRM. The results show that the 238U is in equilibrium with its daughters (234Th, 234mPa, and 210Pb). 232Th is also in equilibrium with its daughters (228Ra, 212Pb, 212Bi and 208Tl). 235U/238U activity ratio of 0.046 ± 0.007 in the sediment is constant in nature but fluctuates du

Validity of a noninvasive estimation of deep body temperature when wearing personal protective equipment during exercise and recovery

Background:Deep body temperature is a critical indicator of heat strain.However,direct measures are often invasive,costly,and difficult to implement in the field.This study assessed the agreement between deep body temperature estimated from heart rate and that measured directly during repeated work bouts while wearing explosive ordnance disposal(EOD)protective clothing and during recovery.Methods:Eight males completed three work and recovery periods across two separate days.Work consisted of treadmill walking on a 1%incline at 2.5,4.0,or 5.5 km/h,in a random order,wearing EOD protective clothing.Ambient temperature and relative humidity were maintained at 24℃and 50%[Wet bulb globe temperature(WBGT)(20.9±1.2)℃]or 32℃and 60%[WBGT(29.0±0.2)℃]on the separate days,respectively.Heart rate and gastrointestinal temperature(TGI)were monitored continuously,and deep body temperature was also estimated from heart rate(ECTemp).Results:The overall systematic bias between TGI and ECTemp was 0.01℃with 95%limits of agreement(Lo A)of±0.64℃and a root mean square error of 0.32℃.The average error statistics among participants showed no significant differences in error between the exercise and recovery periods or the environmental conditions.At TGI levels of(37.0–37.5)℃,(37.5–38.0)℃,(38.0–38.5)℃,and>38.5℃,the systematic bias and±95%Lo A were(0.08±0.58)℃,(–0.02±0.69)℃,(–0.07±0.63)℃,and(–0.32±0.56)℃,respectively.Conclusions:The findings demonstrate acceptable validity of the ECTemp up to 38.5℃.Conducting work within an ECTemp limit of 38.4℃,in conditions similar to the present study,would protect the majority of personnel from an excessive elevation in deep body temperature(>39.0℃).
