Blood transfusion for the treatment of acute anaemia in inflammatory bowel disease and other digestive diseases

Allogeneic blood transfusion (ABT) is frequently used as the first therapeutic option for the treatment of acute anaemia in patients with inflammatory bowel disease (IBD), especially when it developed due to gastrointestinal or perioperative blood loss, but is not risk-free. Adverse effects of ABT include, but are not limited to, acute hemolytic reaction (wrong blood or wrong patient), febrile non-hemolytic transfusional reaction, bacterial contamination, transfusion-related acute lung injury, transfusion associated circulatory overload, transfusion-related immuno-modulation, and transmission of almost all infectious diseases (bacteria, virus, protozoa and prion), which might result in increased risk of morbidity and mortality. Unfortunately, the main physiological goal of ABT, i.e. to increase oxygen consumption by the hypoxic tissues, has not been well documented. In contrast, the ABT is usually misused only to increase the haemoglobin level within a f ixed protocol [mostly two by two packed red blood cell (PRC) units] independently of the patient' s tolerance to normovolemic anaemia or his clinical response to the transfusion of PRC units according to a "one-by-one" administration schedule. Evidencebased clinical guidelines may promote best transfusion practices by implementing restrictive transfusion protocols, thus reducing variability and minimizing the avoidable risks of transfusion, and the use of autologous blood and pharmacologic alternatives. In this regard, preoperative autologous blood donation (PABD) consistently diminished the frequency of ABT, although its contribution to ABT avoidance is reduced when performed under a transfusion protocol. In addition, interpretation of utility of PABD in surgical IBD patients is hampered by scarcity of published data. However, the role of autologous red blood cells as drug carriers is promising. The prevalence of anemia across studies on patients with inflammatory bowel disease (IBD) is high (30%). Both iron defi ciency (ID) and anemia of chronic disease contribute most to the development of anemia in IBD. The prevalence of ID is even higher (45%). Anemia and ID negatively impact the patient's quality of life. Therefore, together with an adequate control of disease activity, iron replacement therapy should start as soon as anemia or ID is detected to attain a normal hemoglobin (Hb) and iron status. Many patients will respond to oral iron, but compliance may be poor, whereas intravenous (IV) compounds are safe, provide a faster Hb increase and iron store repletion, and presents a lower rate of treatment discontinuation. Absolute indications for IV iron treatment should include severe anemia, intolerance or inappropriate response to oral iron, severe intestinal disease activity, or use of an erythropoietic stimulating agent. Four different products are principally used in clinical practice, which differ in their pharmacokinetic properties and safety profi les: iron gluconate and iron sucrose (lower single doses), and iron dextran and ferric carboxymaltose (higher single doses). After the initial resolution of anemia and the repletion of iron stores, the patient's hematological and iron parameters should be carefully and periodically monitored, and maintenance iron treatment should be provided as required. New IV preparations that allow for giving 1000-1500 mg in a single session, thus facilitating patient management,provide an excellent tool to prevent or treat anemia and ID in this patient population, which in turn avoids allogeneic blood transfusion and improves their quality of life.

The Abundance of the Remaining Mammalian Fauna and the Impacts of Hunting in a Biodiversity Hotspot’s Hotspot in the Atlantic Forest of North-Eastern Brazil

Although hunting in the north-eastern Atlantic forest of Brazil began more than 500 years ago, no study to date has evaluated its impacts on the region’s mammalian fauna. For one year we carried out diurnal and nocturnal surveys using the Line Transect method in seven forest fragments varying from 7.32 ha to 469.76 ha, within a 4000 ha forest island archipelago, in Pernambuco State, Atlantic forest of northeastern Brazil. We calculated species density, population size, biomass and synergetic biomass, and recorded direct and indirect human impacts along the study transects. We recorded 44 mammalian species, of which 45.5% (n = 20) went extinct through hunting. The smallest forest fragment had the lowest richness, diversity, population size, and total biomass. It also had no synergetic biomass. The largest fragment had the highest richness, total density, and population size. There was a statistically significant relationship between fragment area and number of gunshots heard and suspended hunting platforms found;between population size and gunshots heard, suspended hunting platforms, free-roaming and feral dogs, and between total density and free-roaming and feral dogs. After more than 500 years of colonization hunting is still devastating, with larger fragments being linked to more hunters. Higher mammal abundances attracted more free-roaming and feral dogs, which have adapted to hunt wildlife on their own. Unless we protect every single forest fragment and create sustainable landscapes, we will not be able to save this hotspot’s hotspot.

Large Language Models(LLMs)are increasingly demonstrating their ability to understand natural language and solve complex tasks,especially through text *** of the relevant capabilities is contextual learning,which involves the ability to receive instructions in natural language or task demonstrations to generate expected outputs for test instances without the need for additional training or gradient *** recent years,the popularity of social networking has provided a medium through which some users can engage in offensive and harmful online *** this study,we investigate the ability of different LLMs,ranging from zero-shot and few-shot learning to *** experiments show that LLMs can identify sexist and hateful online texts using zero-shot and few-shot approaches through information ***,it is found that the encoder-decoder model called Zephyr achieves the best results with the fine-tuning approach,scoring 86.811%on the Explainable Detection of Online Sexism(EDOS)test-set and 57.453%on the Multilingual Detection of Hate Speech Against Immigrants and Women in Twitter(HatEval)***,it is confirmed that the evaluated models perform well in hate text detection,as they beat the best result in the HatEval task *** error analysis shows that contextual learning had difficulty distinguishing between types of hate speech and figurative ***,the fine-tuned approach tends to produce many false positives.

Objective: We describe patients with MCS, the evolution of the Quick Environmental Exposure and Sensitivity Inventory (QEESI) score with a special focus on people whose fillings were removed. Methods: We have conducted a retrospective longitudinal cohort study in patients diagnosed with MCS and attended in the outpatient Internal Medicine department of the University Hospital of San Juan de Alicante, from January 1, 2008 to January 1, 2021. Sociodemographic, clinical, QEESI and treatment-related variables were collected. We performed descriptive and inferential analyses. Mixed linear models were used to analyze the QEESI. Calculations were carried out with an α error of 5%. Results: Thirty-three patients were included (72.7% women, mean age 56.2). MCS was mainly triggered by mercury (N = 20) and food intolerance (N = 22). The mean interval from symptoms onset was 120 months (SD 81.6). 114 QEESIs were analyzed: 82 (N = 17 without amalgams) and 32 (N = 16 with amalgams). In patients without amalgams, severity scores increased across all subscales except the masking index (vs. with amalgams). Mean scores for the group without amalgams (vs. with amalgams) were: chemical intolerance, 62.8 points (vs. 63.4 and 46.7);other intolerances, 52.7 points (vs. 62.8 and 50.3);symptom severity, 63.2 (vs. 76.7 and 63.3);masking index, 3.9 (vs. 3.2 and 2.8);and life impacts, 63.1 (vs. 58.4 and 49.8). Conclusion: The profile of patient with MCS is a middle-aged woman who is a frequent user of healthcare services, presents a long diagnostic delay and has borne a great personal, work and socioeconomic impact. The QEESI is useful for the clinical follow-up of patients, including the optimal treatment response in the case of amalgams. Clinical Significance: People affected by Multiple Chemical Sensitivity deserve the attention, understanding and help of health professionals and family members, to face an invisible illness for those who do not suffer from it. Two aromatic co-polyamides were synthesized combining two diacid monomers containing bulky pendant groups, 5-(9,10-dihydro-9,10-ethanoanthracene-11,12-dicarboximido)isophthalic acid(DEAIA) and 5-tert-butylisophthalic acid(TERT), with 4,4′-(hexafluoroisopropylidene)dianiline(HFA) or 2,3,5,6-tetramethyl-1,4-phenylenediamine(Durene) by direct polycondensation. The structures of the obtained aromatic co-polyamides were confirmed by FTIR, Raman and 1 H-NMR. The co-copolyamide films, DHTH and DDTD, exhibited rms-roughness values between 0.94 and 1.60 nm, respectively. Moreover, they presented good thermal stability up to300 °C. Young's moduli of the co-polyamide films were between 4.1 and 4.3 GPa. X-ray diffraction results showed that the co-polyamide films were amorphous due to the incorporation of both bulky pendant groups, tert-butyl and dibenzobarrelene. The combination of bulky pendant groups provided intrinsically transparent co-polyamide films with a transmittance higher than 88% in the range of 400-780 *** to these outstanding film and optical properties, they are suggested to be flexible substrates in applications for solar cell and other portable electronic devices.

Background:Athletes tend to have better visuo-motor performance than do sedentary ***,several basic visual-function and perceptual parameters remain unexplored to *** this study,we investigated whether differences exist in visual function,performance,and processing between basketball players and individuals without a sport-involvement ***:A total of 33 healthy men with no visual impairment or pathology were divided into 2 groups,depending on the involvement in sport(semi-professional basketball players and sedentary individuals).We tested their baseline heart-rate variability in the resting position apart from subjective questionnaires to determine their physical fitness level,and we checked their visual function,performance,and processing through an extended battery of optometric ***:The 2 groups differed in resting heart-rate variability parameters(p<0.001),confirming their dissimilarities in regular time practising sports per *** basketball players showed a closer breakpoint and recovery nearpoint of convergence,a higher fusional-vergence rate,better discriminability halos,and better eye-hand coordination(all p values<0.05).Conclusion:These results show evidence that athletes,basketball players in this case,exhibit better performance in several visual abilities in comparison to a group of individuals without sporting backgrounds,suggesting an improvement due to the systematic involvement of those skills during basketball practice.
