Healthcare use among people with diabetes mellitus in Europe:a population-based cross-sectional study

Objective This study aimed to determine the association of health determinants,lifestyle and socioeconomic variables on healthcare use in people with diabetes in *** A cross-sectional study was conducted using data from the European Health Interview Survey wave 2(ie,secondary analysis).Setting The sample included data from 25 European *** The sample included 16270 patients with diabetes aged 15 years or older(49.1%men and 50.9%women).Results The survey data showed that 58.2%of respondents had seen their primary care physician in the past month and 22.6%had been admitted to the hospital in the past *** of primary care was associated with being retired(prevalence ratio(PR)1.13,95%CI 1.07 to 1.19)and having very poor self-perceived health(PR 1.80,95%CI 1.51 to 2.15),long-standing health problems(PR 1.14,95%CI 1.04 to 1.24),high blood pressure(PR 1.06,95%CI 1.03 to 1.10)and chronic back pain(PR 1.07,95%CI 1.04 to 1.11).Hospital admission was associated with very poor self-perceived health(PR 3.03,95%CI 2.14 to 4.31),accidents at home(PR 1.54,95%CI 1.40 to 1.69),chronic obstructive pulmonary disease(COPD)(PR 1.34,95%CI 1.22 to 1.47),high blood pressure(PR 1.08,95%CI 1.01 to 1.17),chronic back pain(PR 0.91,95%CI 0.84 to 0.98),moderate difficulty walking(PR 1.33,95%CI 1.21 to 1.45)and severe difficulty walking(PR 1.67,95%CI 1.51 to 1.85).Conclusions In the European diabetic population,the high cumulative incidences of primary care visits and hospital admissions are associated with labour status,alcohol consumption,self-perceived health,long-standing health problems,high blood pressure,chronic back pain,accidents at home,COPD and difficulty walking.

Phenolic metabolism and molecular mass distribution of polysaccharides in cellulose-deficient maize cells

As a consequence of the habituation to low levels of dichlobenil （DCB）, cultured maize cells presented an altered hemicellulose cell fate with a lower proportion of strongly wall-bound hemicelluloses and an increase in soluble extracellular polymers released into the culture medium. The aim of this study was to investigate the relative molecular mass distributions of polysaccharides as well as phenolic metabolism in cells habituated to low levels of DCB （1.5 μM）. Generally, cell wall bound hemicelluloses and sloughed polymers from habituated cells were more homogeneously sized and had a lower weight-average relative molecular mass. In addition, polysaccharides underwent massive cross-linking after being secreted into the cell wall, but this cross-linking was less pronounced in habituated cells than in non-habituated ones. However, when relativized, ferulic acid and p-coumaric acid contents were higher in this habituated cell line. Feasibly, cells habituated to low levels of DCB synthesized molecules with a lower weight-average relative molecular mass, although cross-linked, as a part of their strategy to compensate for the lack of cellulose.

In the current socio-economic scenario characterized by a growing shortage of resources and progressive budget constraints, the need to better coordinate processes in health institutions appears as a relevant aspect to ensure the future sustainability of system. In this sense, Relational Coordination(RC) provides a valuable opportunity for the reconfiguration of clinical guidelines concerning isolated single-level considerations. In this research the RC model has been applied to explain best results in the process of diagnosing and offering clinical treatments for lung cancer. Lung cancer presents the higher rates of tumor's mortality worldwide. Through unstructured and informal interviews with clinicians at both levels(Primary/Specialist Care), a diagnosis of the situation in relation to joint management of lung cancer is provided. Solutions of continuity in terms of coordination are explained due to the observation of lack of effective knowledge transfer between the two levels. It is this disconnection which justifies the introduction of a modified model of RC for the study and implementation of transfer relations between the knowledge holders, in order to structure consolidated and cooperative evidence-based models that lead to a substantial shortening in the response times with a marked outcomes improvement. To our knowledge, the application of this model to a Public Health problem bringing together both levels of care, hasn't been made till now.

T-cell allorecognition of donor glutathione S-transferase T1 in plasma cell-rich rejection

AIM To investigate the role of glutathione S-transferase T1 donor-specific T lymphocytes in plasma cell-rich rejection of liver *** The study group included 22 liver transplant patients. Among them, 18 patients were mismatched for the glutathione S-transferase T1(GSTT1) alleles(don+/rec-), and 4 were matched(don+/rec+). Seven of the mismatched patients produced anti-GSTT1 antibodies and developed plasma cell-rich rejection(former de novo immune hepatitis). For the detection of specific Tlymphocytes, peripheral blood mononuclear cells were collected and stored in liquid nitrogen. The memory T cell response was studied by adding to the cell cultures to a mix of 39 custom-made, 15-mer overlapping peptides, which covered the entire GSTT1 amino acid sequence. The specific cellular response to peptides was analyzed by flow cytometry using the markers CD8, CD4, IL-4 and IFNγ.RESULTS Activation of CD8^+ T cells with different peptides was observed exclusively in the group of patients with plasma-cell rich rejection(3 out of 7), with production of IL-4 and/or IFNγ at a rate of 1%-4.92% depending on the peptides. The CD4^+ response was most common and not exclusive for patients with the disease, where 5 out of 7 showed percentages of activated cells from 1.24% to 31.34%. Additionally, two patients without the disease but with the mismatch had cells that became stimulated with some peptides(1.45%-5.18%). Highly unexpected was the finding of a double positive CD4^+CD8^(low) T cell population that showed the highest degree of activation with some of the peptides in 7 patients with the mismatch, in 4 patients with plasma cell-rich rejection and in 3 patients without the disease. Unfortunately, CD4^+CD8^(low) cells represent 1% of the total number of lymphocytes, and stimulation could not be analyzed in 9 patients due to the low number of gated cells. Cells from the 4 patients included as controls did not show activation with any of the peptides. CONCLUSION Patients

To evaluate the relationship between measures of body composition in obese adolescents by the methods of bioelectrical impedance analysis, deuterium oxide dilution and anthropometric measures, proposing an equation. The variables were weight, height, BMI, triceps and subscapular skinfold thickness, waist and arm muscle circumference, lean body mass, fat mass and total body water by bioelectrical impedance and deuterium oxide dilution methods. The study included 40 obese adolescents, 45% male, age distribution was 2.42 ± 1.19 years and females 55%, and the predominant age was 12.61 ± 1.78. Linear regression equations were developed, capable of predicting body composition from information supplied by the method of deuterium oxide dilution (gold standard), bioelectrical impedance and anthropometry. The variables gender, age, height, arm circumference, triceps and suprailiac skin fold thickness, resistance and reactance were used to estimate lean body mass, fat mass and total body water by the method of deuterium and significantly correlated with variables, resistance, reactance, sex and total body water (TBW) by bioimpedance method. Among the equations developed, five were suitable for this sample, therefore, it is suggested that more studies should be done to test the applicability of the equations in other samples so that we can validate the equations encountered in obese adolescents.

Tolerance in liver transplantation: Biomarkers and clinical relevance

Transplantation is the optimal treatment for end-stage organ failure,and modern immunosuppression has allowed important progress in short-term outcomes. However,immunosuppression poorly influences chronic rejection and elicits chronic toxicity in current clinical practice. Thus,a major goal in transplantation is to understand and induce tolerance. It is well established that human regulatory T cells expressing the transcription factor Fox P3 play important roles in the maintenance of immunological self-tolerance and immune homeostasis. The major regulatory T cell subsets and mechanisms of expansion that are critical for induction and long-term maintenance of graft tolerance and survival are being actively investigated. Likewise,other immune cells,such as dendritic cells,monocyte/macrophages or natural killer cells,have been described as part of the process known as "operational tolerance". However,translation of these results towards clinical practice needs solid tools to identify accurately and reliably patients who are going to be tolerant. In this way,a plethora of genetic and cellular biomarkers is raising and being validated worldwide in large multicenter clinical trials. Few of the studies performed so far have provided a detailed analysis of the impact of immunosuppression withdrawal on pre-existing complications derived from the long-term administration of immunosuppressive drugs and the side effects associated with them. The future of liver transplantation is aimed to develop new therapies which increase the actual low tolerant vs non-tolerant recipients ratio.

Plasma betatrophin levels in patients with liver cirrhosis

AIM: To investigate the plasma levels of betatrophin in patients with ***: Forty patients diagnosed at the clinic with liver cirrhosis according to biological, ultrasonographic,or histological criteria were *** severity of cirrhosis was classified according to Pugh's modification of Child's classification and MELD score. Insulin resistance(IR) was assessed by the Homeostasis Model Assessment. A total of 20 patients showed a MELD score higher than 14. The control group consisted in 15 sex-and aged-matched *** blood samples were obtained for subsequent analysis. Serum insulin was determined by Liaison automated immune chemiluminiscence assay(DiaSorin S.p.A.) using a sandwich assay. The sensitivity of the assay was 0.2 μU/mL. The intra and interassay variation coefficients were jects(P = 0.0001). Betatrophin levels were also associated with disease severity, being higher in Child-Pugh C patients compared to Child-Pugh B(P< 0.0005) and in patients who displayed a MELD score higher than 14 points compared to patients with lower punctuation(P = 0.01). In addition, we found a positive correlation between plasma betatrophin levels and the severity of cirrhosis according to Child-Pugh classification(r = 0.53; P < 0.01) or MELD score(r = 0.45; P <0.01). In the overall cohort, a moderate correlation between serum betatrophin and plasmatic bilirrubin(r= 0.39; P < 0.01) has been observed, as well as an inverse correlation between betatrophin and albumin(r =-0.41; P < 0.01) or prothrombin time(r =-0.44;P <0.01).
