AIM:To compare the performance of different types of abdominal drains used in bariatric ***:A vertical banded Roux-en-Y gastric bypass was performed in 33 morbidly obese *** of the peritoneal cavity was performed in each case using three different types of drain selected in a randomized manner:a latex tubular drain,a Watterman tubulolaminar drain,and a silicone channeled *** permeability,contamination of the drained fluid,ease of handling,and patient discomfort were evaluated postoperatively over a period of 7 ***:The patients with the silicone channeled drain had larger volumes of drainage compared to patients with tubular and tubulolaminar drains between the third and seventh postoperative *** addition,a lower incidence of discomfort and of contamination with bacteria of a more pathogenic profile was observed in the patients with the silicone channeled ***:The silicone channeled drain was more comfortable and had less chance of occlusion,which is important in the detection of delayed dehiscence.

Objective: to assess the evolution of serum creatinine in patients undergoing radical nephrectomy (malignant) or total (benign), to identify risk factors connected with an unfavorable renal function outcome. Material and Methods: observational, transversal and retrospective study, through analysis of records and lab exams of 146 patients undergoing nephrectomy from January 2015 to December 2018. Results: Statistically significant difference was found between etiology and patients’ age (p-value Conclusion: This study shows that a large number of young patients undergo nephrectomy, many times avoidable;within a benign disease context with an increased risk of developing renal failure. Greater attention from the public service is required to tackle such chronic condition and its complications.

AIM:To evaluate the association between Helicobacter pylori(***) infection and MLH1 and MGMT methylation and its relationship with microsatellite instability(MSI).METHODS:The methylation status of the MLH1 and MGMT promoter region was analysed by methylation specific methylation-polymerase chain reaction(MSPPCR) in gastric biopsy samples from uninfected or ***-infected children(n = 50),from adults with chronic gastritis(n = 97) and from adults with gastric cancer(n = 92).MLH1 and MGMT mRNA expression were measured by real-time PCR and normalised to a constitutive gene(β actin).MSI analysis was performed by screening MSI markers at 4 loci(Bat-25,Bat-26,D17S250 and D2S123) with PCR;PCR products were analysed by single strand conformation polymorphism followed by silver *** analyses were performed with either the χ 2 test with Yates continuity correction or Fisher's exact test,and statistical significance for expression analysis was assessed using an unpaired Student's ***:Methylation was not detected in the promoter regions of MLH1 and MGMT in gastric biopsy samples from children,regardless of *** infection *** MGMT promoter was methylated in 51% of chronic gastritis adult patients and was associated with *** infection(P < 0.05);this region was methylated in 66% of gastric cancer patients,and the difference in the percentage of methylated samples between these patients and those from ***-infected chronic gastritis patients was statistically significant(P < 0.05).MLH1 methylation frequencies among ***-infected and non-infected chronic gastritis adult patients were 13% and 7%,*** observed methylation of the MLH1 promoter(39%) and increased MSI levels(68%) in samples from gastric cancer patients in comparison to samples from ***-infected adult chronic gastritis patients(P < 0.001 and P < 0.01,respectively).The frequency of promoter methylation for both genes was higher in gastric cancer sam

The presence of cholestasis in both mild and severe forms of acute biliary pancreatitis(ABP)does not justify,of itself,early endoscopic retrograde cholangiography(ERC)or endoscopic sphincterotomy(ES).Clinical support treatment of acute pancreatitis for one to two weeks is usually accompanied by regression of pancreatic edema,of cholestasis and by stone migration to the duodenum in 60%-88%of *** the other hand,in cases with both cholestasis and fever,a condition usually characterized as ABP associated with cholangitis,early ES is normally ***,in daily clinical practice,it is practically impossible to guarantee the coexistence of cholangitis and mild or severe acute ***,fever and cholestasis,as well as mental confusion and hypotension,may be attributed to inflammatory and necrotic events related to ABP. Under these circumstances,evaluation of the bile duct by endo-ultrasonography(EUS)or magnetic resonance cholangiography(MRC)before performing ERC and ES seems ***,it is necessary to assess the effects of the association between early and opportune access to the treatment of local and systemic inflammatory/infectious effects of ABP with cholestasis and fever, and to characterize the possible scenarios and the subsequent approaches to the common bile duct,directed by less invasive examinations such as MRC or EUS.

Morphological effects of autoclaved diet on the myenteric neurons of rats

AIM:To evaluate the effect of autoclaved diet on the jejunum neurons of the myenteric plexus of rats during their ***:The experimental groups were made up of rats going through weaning whose mothers received either an autoclaved or a non-autoclaved diet during gestation and lactation,and rats that were fed the same diet as their mothers during the post-weaning *** order to measure the neurons'body profile and to quantify the number of neurons per area,preparations were stained by the nicotinamide adenine dinucleotide-diaphorase ***:No significant changes were observed in rats'body weight or in the number of neurons regardless of the diet used(P>0.05) .There was a decrease in the jejunum-ileum length in rats treated with an autoclaved diet(P<0.05) .An increase in the neuronal cross-sectional area was seen in rats that had received the autoclaved diet,an effect that was significant for animals undergoing *** addition,all observed factors showed significant differences when related to the age of the ***:The autoclaved diet did not alter the quantity of neurons,but increased their cell body area,suggesting changes similar to those observed in protein deficiency.

Molecular Analyses of Early-Onset Gastric Cancer in Brazilian Patients: <i>TP</i>53 Mutations, Cadherin-Catenin and Mucins Proteins Expression

Early gastric carcinomas may develop with a molecular profile differing from sporadic carcinomas occurring at a later age. In this study, we analyzed a retrospective series of 88 patients with gastric adenocarcinoma diagnosed before the age of 45 years for the presence of TP53 mutations, clinicopathological features and immunohistochemistry to evaluate the expression of markers considered to be important in gastric carcinogenesis (E-cadherin, β-catenin, MUC1, MUC2, MUC5AC, MUC6 and p53). The majority of proportion of tumors were diffuse-type (70%) and advanced stage (56%). Familial history of cancer was positive in 21% of the cases. There was a significant association between altered expression of E-cadherin and β-catenin, and between p53 expression and perineural invasion. TP53 mutations were detected in 14.5% of evaluated cases, including a germline mutation (p.R337H) in a 12-year old patient. Overall survival analysis showed significant differences in relation with tumor stage and histopathology. The evaluated biomarkers did not present prognostic value in non-exploratory multivariate analyses. The low frequency of TP53 mutations in this series suggests these alterations are not a major molecular event in gastric cancer occurring at early age, although the identification of a case with germline p.R337H mutation is consistent with the hypothesis that a small proportion of early, apparently sporadic gastric cancer, may be associated with widespread Brazilian founder mutations. Further studies are needed to evaluate the prognostic significance of markers for specific groups of patients according to tumor histology and familial history.

Unmanned Aerial Vehicles Flight Safety Improvement Using In-Flight Awareness

This paper presents a novel onboard system called In-Flight Awareness Augmentation System (IFA2S) to improve flight safety. IFA2S is designed to semi-automatically (with human supervision) avoid hazards and accidents due to either internal or external causal factors. The requirements were defined in an innovative way using Systems-Theoretic Process Analysis (STPA) method and applied next to model the system. IFA2S increases aircraft awareness regarding both itself and its environment and, at the same time, recognizes platform and operational constraints to act in accordance to predefined decision algorithms. Results are presented through simulations and flight tests using state machines designed to allow the adoption of appropriate actions for the identified hazards. The different decision algorithms are evaluated over as many as possible hazard situations by simulations conducted with software Labview and XPlane flight simulator. Flight tests are performed in a small fixed wing aircraft and make use of a limited version IFA2S, partially attending identified requirements. Results support the conclusion that IFA2S is capable of improving flight safety.
