The Assessment of Medical Students’ Chronotypes

Objective: This study aimed to investigate the circadian typology of medical students at public universities from one of the northeastern states in Brazil, active in the morning and afternoon shifts, checking the compatibility between their study time, productivity, income and chronotype. Methods: To obtain the results, the assessment of the chronotype in 342 medical students of both sexes was considered. Each participant was submitted to the questionnaire developed by Horne and Ostberg, adapted by Cardinali, Colomberk and Rey. It’s composed of 9 questions, providing the general ranking of each individual. Based upon scored, the chronotypes were classified. Results: The sample was composed of 49.12% (168) male and 50.87% (174) female subjects, a similar gender distribution of enrolled students during data collection time. By applying the questionnaire, the prevalent rating among the medical students as moderately morning (29.82%) and intermediate (43.85%) has been relevant. The academic performance assessment in two morning-intermediate groups (8.16) had no significant distance from others considered afternoon active (8.20). Conclusion: The health sciences student, especially Medicine, faces constant adaptation of scheduling, in the attempt of fulfilling the requirements throughout the course, causing chronogram desynchronization effects and forced readaptation to each different school semester.

介绍了对采用铜质形状记忆合金(SMA)的部分约束连接试验研究的详细内容。提出在规则空心钢梁与宽翼缘钢柱之间的端板连接构造,其中4根处于奥氏体阶段的CuAlBe(3mm)钢筋,在端板和柱翼缘之间起着预拉作用。通过在梁末端施加特征频率为0.25Hz和1Hz的循环位移,对连接的物理模型进行试验。采用电位计和测压元件来测量钢筋中的应变、应力和梁端的位移及荷载。采用预先循环拉力测试过的相同热处理的钢筋,显示其有超弹性性能,应变可达到2.3%。在静态拉伸试验中,断裂应变大约为8%,为穿晶断裂机制。单根试验钢筋的等效阻尼比随应变增大而有所增加,对应于应变2.3%时等效阻尼比为6%。梁柱连接同时也表现出超弹性性能、中等程度的耗能水平,在多个周期作用偏移达到3%的情况下仍无强度退化。最后,对一个三层标准结构进行数值模拟,以对比刚性钢结构和采用SMA连接的部分约束结构的性能。

Digital intubation was discovered as one of the first methods to face a difficult airway without direct laryngoscopy. From the very beginning, this technique has been surrounded by much controversy, mainly because it required to be performed by an expert. Nowadays, it remains a useful technique when treating patients with difficult airways, so it is of utmost importance all personnel involved with airway management must know and perfect this technique when scenarios where conventional laryngoscopy or rescue devices for difficult airway are not available or contraindicated. The present work’s main objective is to suggest digital intubation as a safe and effective technique for the management of patients with difficult airways when there are no other devices available. The authors present a successful case of digital intubation on a patient with a difficult airway, demonstrating this technique is useful when performed by expert practitioners and when there is no other equipment available.

Introduction: Degeneration of the cervical spine (CDSD) prevalence is nearly 90% by the 7th decade. This is the first research that compares the outcomes between the Anterior Approach (AA) and Posterior Approach (PA) to cervical myelopathy (CM) in the elderly. Materials and Methods: A retrospective observational study of electronic health records at the Hospital Central Sur de Alta Especialidad (HCSAE), PEMEX from January 2010 to May 2020 with patients older than 60 years submitted to cervical surgery. For the analysis we elaborated two groups according to the surgical approach: AA vs PA;we analyzed the trans-operative behavior, the immediate outcome, and after 3 months, 6 months, and 1 year. Results: As a total of 145 patients, the prevalence of CM in elderly was the 63.8% with a median age of 69 (64 - 75) years. We found statistical differences in strength outcome only in the P3m (p = 0.011), for sensitivity we found major prevalence of affection in the PA group. Background: The trigger finger, also known as stenosing tenosynovitis of the flexor tendon, is a common cause of hand disability;This can result in significant limitations in certain daily living activities. It is possible to treat this condition with conservative or surgical methods. Methods: This is a retrospective, observational, cross-sectional and descriptive study. The source population consisted of all patients with a diagnosis of trigger finger who had been treated in the department of plastic and reconstructive surgery of our institution between 2020 and 2023. The primary objective of this study is to determine the prevalence and characterize the clinical and surgical characteristics of the population affected by trigger finger in our hospital. Results: The total population was 237 patients, of which 165 were women (70%) and 72 men (30%). The most affected hand was the right hand, with a total of 158 patients (67%). In the right hand, the middle finger was the most affected (45%). Of the 237 patients, 80 of them had obesity (34%), 86 had type 2 diabetes mellitus (36%), 95 had systemic arterial hypertension (40%). Open technique was performed in 190 (80%) patients, while percutaneous technique was performed in 47 (20%) of them. Conclusion: The female sex was twice as frequent, and the middle finger and thumb were the most affected. Most of the patients received surgical treatment with open approach and percutaneous approach in 20% of them.

Immobility Responses Affected by Potassium in Old Rats

Four immobility responses (IR): elicited by clamping, bandaging, grasping and inversion, and their modification by potassium and spironolactone was studied in old Wistar rats (body weight, 500 g). When undrugged, only clamping and grasping, but not bandaging and inversion induced an IR in rats. Potassium and spironolactone significantly enhanced the duration of IR induced by clamping but not by grasping. They also induced an immobility response by bandaging, but not by inversion. The data suggest that IR induced by clamping and bandaging are somehow related to changes in the potassium serum levels. Consequently, such a relationship may be a suitable model to study some forms of paralysis in human beings which are related to changes in the potassium serum levels.
