This article examines the complex relationship between disease perception,negative emotions,and their impact on postoperative recovery in patients with perianal *** conditions not only cause physical discomfort,but also carry a significant emotional burden,often exacerbated by social ***-logical factors,including stress,anxiety,and depression,activate neuroendocrine pathways,such as the hypothalamic–pituitary–adrenal axis,disrupting the gut microbiota and leading to *** disruption can delay wound healing,prolong hospital stay,and intensify *** on the findings of Hou et al,our article highlights the critical role of illness perception and negative emotions in shaping recovery *** advocates for a holistic approach that integrates psychological support and gut microbiota modulation,to enhance healing and improve overall patient outcomes.

Accuracy of transnasal endoscopy with a disposable esophagoscope compared to conventional endoscopy

AIm: To assess feasibility of unsedated esophagoscopy using a small-caliber disposable transnasal esophagosco-py and to compare its accuracy with standard ***: We prospectively included subjects who were referred for upper endoscopy. All subjects un-derwent transnasal endoscopy with E.G. Scan^(Tm). The disposable probe has a 3.6 mm gauge and at its distal end there is a 6 mm optical capsule, with a viewing angle of 125°. Patients underwent conventional endos-copy after the completion of E.G. Scan^(Tm). We describe the findings detected by the E.G. Scan^(Tm) and calculate the diagnostic accuracy, sensitivity, specificity, positive predictive value, negative predictive value and Kappa index for esophageal ***: A total of 96 patients(54 women), mean age of 50.12 years(14 to 79), were evaluated. In all cases we were able to perform esophagoscopy with ***^(Tm). The average realization time was 5 min. A total of 58 alterations were detected in the esophagus, 49 gastric abnormalities and 13 duodenal abnormalities. We found that for esophageal varices, E.G. Scan^(Tm) has sensitivity, specificity and diagnostic accuracy of 95%, 97% and 97%, respectively. Kappa coefficients were 0.32 for hiatal hernia, 0.409 for erosive gastroesopha-geal reflux disease, 0.617 for Barrett's esophagus, and 0.909 for esophageal ***: Esophagoscopy with E.G. Scan? is a well-tolerated, fast and safe procedure. It has an ap-propriate diagnostic accuracy for esophageal varices when compared with conventional endoscopy.

AIm: To compare the ability of model for end-stage liver disease(mELD)-Na and maddrey discrimination function index(DFI) to predict mortality at 30 and 90 d in patients with alcoholic hepatitis(AH).mETHODS: We prospectively assessed 52 patients with AH. Demographic, clinical and laboratory parameters were obtained. mELD-Na and maddrey DFI were calculated on admission. Short-term mortality was assessed at 30 and 90 d. Receiver operating characteristic curve analysis was performed. RESULTS: Thirty-day and 90-d mortality was 44% and 58%, respectively. In the univariate analysis, sodium levels was associated with mortality at 30 and 90 d(P = 0.001 and P = 0.03). Child stage, encephalopathy, ascites, or types of treatment were not associated with mortality. mELD-Na was the only predictive factor for mortality at 90 d. For 30-d mortality area under the curve(AUC) was 0.763(95%CI: 0.63-0.89) for maddrey DFI and 0.784 for mELD-Na(95%CI: 0.65-0.91, P = 0.82). For 90-d mortality AUC was 0.685(95%CI: 0.54-0.83) for maddrey DFI and 0.8710 for mELD-Na(95%CI: 0.76-0.97, P = 0.041). CONCLUSION: AH is associated with high shortterm mortality. Our results show that mELD-Na is a more valuable model than DFI to predict short-term mortality.

AIm: Recently, drinking load tests with water or nutritional beverages have been proposed as diagnostic tools for functional dyspepsia (FD), therefore we sought to reproduce if these tests can discriminate between FD patients and controls in a mexican population. mETHODS: Twenty FD-Rome Ⅱ patients were matched by age and gender with 20 healthy controls. All underwent both drinking tests at a 15 mL/min rate, randomly, 7 d apart. Every 5 min within each test, four symptoms were evaluated (satiety, bloating, nausea and pain) by Likert scales. maximum tolerated volume (mTV) was defined as the ingested volume when a score of 5 was reached for any symptom or when the test had to be stopped because the patients could not tolerate more volume. Sensitivity and specificity were analyzed. RESULTS: FD patients had higher symptom scores for both tests compared to controls (water: t= 4.1, P= 0.001 ＜0.01; Nutren(R): t= 5.2, P= 0.001＜0.01). The mTV forwater and Nutren(R) were significantly lower in FD (water: 1014±288 vs 1749±275 mL; t = 7.9, P = 0.001＜0.01;Nutren(R): 652±168 vs 1278±286 mL; t= 6.7, P = 0.001＜0.01). With the volume tolerated by the controls, the percentile 10 was determined as the lower limit fortolerance. Sensitivity and specificity were 0.90, 0.95 for water and 0.95, 0.95 for Nutren(R) ***: A drinking test with water or a nutritional beverage can discriminate between FD patients and healthy subjects in mexico, with high sensitivity and specificity. These tests could be used as objective, noninvasive, and safe diagnostic approaches for FD patients.

BACKGROUND The definition of metabolic-dysfunction-associated fatty liver disease(mAFLD)allows identification of metabolically complicated *** risk scores are related to cardiovascular risk(CVR)scores and could be useful for the identification of patients at risk of systemic *** To evaluate the relationship between mAFLD and CVR using the Framingham risk score in a group of mexican *** Cross-sectional,observational and descriptive study carried out in a cohort of 585 volunteers in the state of Veracruz with mAFLD *** risk of liver fibrosis was calculated with aspartate aminotransferase-to-platelet ratio index,nonalcoholic fatty liver disease score and fibrosis-4,as well as with transient hepatic elastography with Fibroscan~?.The CVR was determined by the Framingham *** One hundred and twenty-five participants(21.4%)with mAFLD criteria were evaluated,average age 54.4 years,63.2%were women,body mass index 32.3 kg/m~*** Framingham CVR was high in 43 patients(33.9%).Transient elastography was performed in 55.2%of volunteers;39.1%with high CVR and predominance in advanced fibrosis(F3–F4).The logistic regression analysis showed that liver fibrosis,diabetes and hypertension independently increased *** One of every three patients with mAFLD had a high CVR,and in those with high fibrosis risk,the CVR risk was even greater.
