APRI as a predictor of early viral response in chronic hepatitis C patients

AIm:To evaluate the aspartate aminotransferase(AST) to platelet ratio index(APRI) as a predictive factor of early viral response in chronic hepatitis C naive ***:We performed an ambispective case-control *** enrolled chronic hepatitis C naive patients who were evaluated to start therapy with PEGylated interferon α-2b(1.5 μg/kg per week) and ribavirin(>75 kg:1200mg and mg).Patients were allocated into two groups,group 1:Hepatitis C patients with early viral response(EVR),group 2:Patients without *** ratio(OR) and 95% confi dence interval(CI) were calculated to assess the relationship between each risk factor and the EVR in both ***:During the study,80 patients were analyzed,45 retrospectively and 35 *** mean ± SD age of our subjects was 42.9 ± 12 years;weight 70 kg(±11.19),AST 64.6 IU/mL(±48.74),alanine aminotransferase(ALT) 76.3 IU/mL(±63.08) and platelets 209000 mill/mm3(±84 429).Fifty-five(68.8%) were genotype 1 and 25(31.3%) were genotype 2 or 3;the mean hepatitis C virus RnA viral load was 2 269 061 IU/mL(±7220266).In the univariate analysis,APRI was not associated with EVR [OR 0.61(95% CI 0.229-1.655,P=0.33)],and the absence of EVR was only associated with genotype 1 [OR 0.28(95% CI 0.08-0.94,P=0.034)].After adjustment in a logistic regression model,genotype 1 remains signifi ***:APRI was not a predictor of EVR in chronic hepatitis C;Genotype 1 was the only predictive factor associated with the absence of EVR in our patients.

Postoperative complications in gastrointestinal surgery: A “hidden” basic quality indicator

Postoperative complications represent a basic quality indicator for measuring outcomes at surgical units. At present, however, they are not systematically measured in all surgical procedures. A more accurate assessment of their impact could help to evaluate the real morbidity associated with different surgical interventions, establish measures for improvement, increase efficiency and identify benchmarking services. The Clavien-Dindo Classification is the most widely used system worldwide for assessing postoperative ***, the postoperative period is summarized by the most serious complication without taking into account others of lesser magnitude. Recently,two new scoring systems have emerged, the Comprehensive Complication Index and the Complication Severity Score, which include all postoperative complications and quantify them from 0(no complications) to 100(patient’s death), These allow the comparison of results. It is important to train surgical staff to report and classify complications and to record 90-d morbidity rates in all patients. Comparisons with other services must take into account patient comorbidities and the complexity of the particular surgical procedure. To avoid subjectivity and bias, external audits are necessary. In addition, ensuring transparency in the reporting of the results is an urgent obligation.

Rhombohedral phase Hf_(x)Zr_(1.x)O_(2)(HZO,x from 0 to 1)films are promising for achieving robust ferroelectric polarization without the need for an initial wake-up pre-cycling,as is normally the case for the more commonly studied orthorhombic ***,a large spontaneous polarization observed in rhombohedral films is not fully understood,and there are also large discrepancies between experimental and theoretical *** this work,in rhombohedral ZrO_(2)thin films,we show that oxygen vacancies are not only a key factor for stabilizing the phase,but they are also a source of ferroelectric polarization in the *** is shown experimentally through the investigation of the structural properties,chemical composition and the ferroelectric properties of the films before and after an annealing at moderate temperature(400℃)in an oxygen environment to reduce the V_(o)concentration *** experimental work is supported by density functional theory(DFT)calculations which show that the rhombohedral phase is the most stable one in highly oxygen defective ZrO_(2)*** DFT calculations also show that V_(o)contribute to the ferroelectric *** findings reveal the importance of V_(o)for stabilizing rhombohedral ZrO_(2)thin films with superior ferroelectric properties.

Phosphorus fractions and sorption dynamics in the sediments of two Ca-SO4 water reservoirs in the central Argentine Andes

Phosphorus（P） fractionation and sorption behavior were studied in the sediments of two calcium-sulfate(Ca-SO4: Ca2+170 mg L-1; SO42-400 mg L-1) water reservoirs in the southern central Argentine Andes-El Carrizal Reservoir（ECR）,which stratifies in summer and El nihuil Reservoir（EnR）,which is vertically mixed throughout the year. Sediment size classes reflected the lithology of both basins and the reservoirs relative location: ECR（downstream from a valley adjacent to a mountain environment） had higher clay proportions while the sand fraction was more important in EnR（just next to a mountain environment）.In both reservoirs, the chemical composition revealed low alteration and calcium enrichment. Total P content was relatively high(1.1-1.6 mg P kg-1dry weight) as expected from the contribution of marine formations. P fractionation was dominated by Ca-P（ECR, 69% and EnR, 63%） followed by organic-P（23%）in summer-anoxic ECR and by iron/aluminum（Fe/Al）-P（27%） in oxic EnR. Batch experiments showed that sorption behavior in ECR had a typical Langmuir isotherm plot while in EnR that model fitted only at low dissolved P values(mg PL-1) and co-precipitation evidence was observed at higher P concentrations. It is proposed that lower Fe/Al-P content in anoxic（ECR） vs. oxic（EnR） sediments results from the P released from iron-bound phosphorus due to sulfate reduction-ferrous sulfide formation in an anoxic environment. This condition should enable ECR sediments to adsorb P into fresh oxidized iron,and EnR sediments to co-precipitate P with calcium due to Fe/Al-P saturation, as observed in batch experiments. This paper constitutes the first description of phosphorus content and dynamics in reservoir sediments in arid central western Argentina, where water sulfate concentrations are naturally high due to basin lithology.

Rock mass structural recognition from drill monitoring technology in underground mining using discontinuity index and machine learning techniques

A procedure to recognize individual discontinuities in rock mass from measurement while drilling(mWD)technology is developed,using the binary pattern of structural rock characteristics obtained from in-hole images for *** from two underground operations with different drilling technology and different rock mass characteristics are considered,which generalizes the application of the methodology to different sites and ensures the full operational integration of mWD data *** approaches are followed for site-specific structural model building:a discontinuity index(DI)built from variations in mWD parameters,and a machine learning(mL)classifier as function of the drilling parameters and their *** prediction ability of the models is quantitatively assessed as the rate of recognition of discontinuities observed in borehole *** between the parameters involved in the models for each site,and differences in their weights,highlight the site-dependence of the resulting *** mL approach offers better performance than the classical DI,with recognition rates in the range 89%to 96%.However,the simpler DI still yields fairly accurate results,with recognition rates 70%to 90%.These results validate the adaptive mWD-based methodology as an engineering solution to predict rock structural condition in underground mining operations.

Gut microbiota-related complications in cirrhosis

Gut microbiota plays an important role in cirrhosis. The liver is constantly challenged with commensal bacteria and their products arriving through the portal vein in the so-called gut-liver axis. Bacterial translocation from the intestinal lumen through the intestinal wall and to mesenteric lymph nodes is facilitated by intestinal bacterial overgrowth, impairment in the permeability of the intestinal mucosal barrier, and deficiencies in local host immune defences. Deranged clearance of endogenous bacteria from portal and systemic circulation turns the gut into the major source of bacterialrelated complications. Liver function may therefore be affected by alterations in the composition of the intestinal microbiota and a role for commensal flora has been evidenced in the pathogenesis of several complications arising in end-stage liver disease such as hepatic encephalopathy, splanchnic arterial vasodilatation and spontaneous bacterial peritonitis. The use of antibioticsis the main therapeutic pipeline in the management of these bacteria-related ***,other strategies aimed at preserving intestinal homeostasis through the use of pre-,pro-or symbiotic formulations are being studied in the last *** this review,the role of intestinal microbiota in the development of the most frequent complications arising in cirrhosis and the different clinical and experimental studies conducted to prevent or improve these complications by modifying the gut microbiota composition are summarized.

Climate-driven variations in productivity reveal adaptive strategies in Iberian cork oak agroforestry systems
