Diagnostic-therapeutic management of bile duct cancer

Biliary tract cancer, or cholangiocarcinoma, comprises a heterogeneous group of malignant tumors that can emerge at any part of the biliary tree. This group is the second most common type of primary liver cancer. Diagnosis is usually based on symptoms, which may be heterogeneous, and nonspecific biomarkers in serum and biopsy specimens, as well as on imaging techniques. Endoscopy-based diagnosis is essential, since it enables biopsy specimens to be taken. In addition, it can help with locoregional staging of distal tumors. Endoscopic retrograde cholangiopancreatography is a key technique for the evaluation and treatment of malignant biliary tumors. Correct staging of cholangiocarcinoma is essential in order to be able to determine the degree of resectability and assess the results of treatment. The tumor is staged based on the TNM classification of the American Joint Committee on Cancer. The approach will depend on the classification of the tumor. Thus, some patients with early-stage disease could benefit from surgery;complete surgical resection is the cornerstone of cure. However, only a minority of patients are diagnosed in the early stages and are suitable candidates for resection. In the subset of patients diagnosed with locally advanced or metastatic disease, chemotherapy has been used to improve outcome and to delay tumor progression. The approach to biliary tract tumors should be multidisciplinary, involving experienced endoscopists, oncologists, radiologists, and surgeons.

Prevalence of Spinal Metastasis in Neurosurgical Procedures: A Descriptive Study

Objective: This study sought to identify the prevalence of operations for spinal epidural neoplastic metastases relative to other spine and neurosurgical operative procedures. Methods: This study was descriptive and involved a retrospective review of data collected from patients who underwent neurosurgeries between February 1997 and January 2015 at a single quaternary hospital. The examined population was distributed across five descriptive categories to perform numerical distributions among neurosurgical operative procedures. Results: A total of 12,802 neurosurgical procedures were identified. These procedures were classified as follows: Skull and brain, 11,192 (87.42%);spinal column and spinal nervous tissue, 1462 (11.42%);and peripheral nerves, 148 (1.16%). Surgical procedures for the 1462 (100%) cases of spinal column and spinal nervous tissue diseases were distributed by nosology as follows: Degenerative intervertebral disk and spondylosis, 768 (52.54%);neoplastic, 279 (19.08%);traumatic, 221 (15.11%);congenital, 163 (11.14%);infectious and inflammatory, 27 (1.85%);and vascular, 4 (0.28%). With respect to the distribution of the 279 (100%) surgical procedures for spinal column and spinal nervous tissue neoplastic diseases, 124 (44.44%) procedures were for intradural neoplasms, and 155 (55.56%) procedures were for epidural spinal column neoplasms. The 155 (100%) operations for epidural neoplastic diseases were distributed into two groups: Primary epidural neoplasms, 42 (27.10%);and secondary epidural neoplasms, 113 (72.90%). Spinal column epidural neoplastic metastases (secondary neoplasms) represented 0.88% of the 12,802 neurosurgical procedures. Conclusions: Surgical procedures for spinal metastasis are uncommon with respect to all neurosurgical operative procedures. Trend analysis reveals an unchanging trend of prevalence for these procedures.

Composition and Molecular Identification of Bacterial Community in Seawater Desalination Plants

Biofouling is an important problem for reverse osmosis (RO) membrane manufacturers. Bacteria are mainly involved in generating fouling and obturating RO membranes. Insights into biofilm bacteria composition could help prevent biofouling, reduce the cost of using RO-fouling membranes and guarantee safe water. Culture-dependent and independent techniques were then performed in order to identify bacteria associated with RO membranes. Bacteria cultures described the presence of six pure colonies, four of which were identified through API testing. Based on 16s rRNA gene analysis, a predominant bacterium was identified and annotated as Sphingomonas sp. The 16s rRNA gene clone library, on the other hand, showed that the bacterium, Pseudomonas marincola, accounted for nearly 30% of the clone library, while the rest of bacteria were chimeras (62%) and non-representative species (3%). In conclusion, culture-dependent and independent approaches showed that two dominant bacteria were commonly observed in RO desalination membranes.

OI 630.0 nm airglow observations,from a new observatory at Bom Jesus de Lapa,were used to study the interaction between EPBs(Equatorial Plasma Bubbles)and the MSTID(Medium-Scale Traveling Ionospheric Disturbance)over the Northeast region in *** the night of September 16 to 17,2020,an EPB was observed propagating eastward,in an apparent fossil stage,until it interacted with a dark band electrified MSTID(e MSTID).After the interaction,four EPBs merged,followed by an abrupt southward development and *** of the data suggests that an eastward polarization electric field,induced by the dark band e MSTID,forced the EPB into an upward drift,growing latitudinally along the magnetic field lines and then bifurcating.

Sintering and Wear Behavior of a FeCrCB Hardfacing Alloy Applied by Tape Casting: A Study of Cooling Rate Effect

This study presents a simple process to deposit a hardfacing coating on a steel substrate, based on the sintering of metallic powder applied by tape casting (by a slurry of metallic powder suspension onto a steel substrate) thus avoiding the use of traditional welding processes and their variants. The effect of the cooling of hardfacing after the sintering process, by air at room temperature or by quenching in water, was studied. This new method ensures a good metallurgical bonding between the substrate steel and the hardfacing layer and shows mechanical property improvement on coated pieces, similar to those exhibited by hardfacing coatings produced by several kinds of welding processes. The characterization of the hardfacing coatings was made by X-ray diffraction, optical microscopy, scanning electron microscopy, microhardness and wear resistance according to the ASTM G65 standard. The characterization results show that the presented faces are: M7C3, M3C, MC, M2B and M23B6;there are three different phases in the micrograph glass phase, eutectic phase and hard phase with a volumetric fraction of 0.14, 0.20 and 0.66, respectively, for the air cooled and 0.15, 0.16 and 0.69 when quenched in water. The average microhardness value for the parts cooled in air was 832.5 HV and for that cooled in water was 958.9 HV, and the wear resistance was a mass loss of 0.219 and 0.128 g for parts cooled in air and water, respectively. These results show that the hardfacing coating could have twice the hardness and wear resistance than that observed for the boron steel used as a substrate.

Effectiveness of different biochars in remediating a salt-affected Luvisol in Northeast Brazil

Reagent-free determination of amikacin content in amikacin sulfate injections by FTIR derivative spectroscopy in a continuous flow system

The quantitative estimation of amikacin （AMK） in AMK sulfate injection samples is reported using FTIR-derivative spectrometric method in a continuous flow system. Fourier transform of mid-IR spectra were recorded without any sample pretreatment. A good linear calibration （r40.999, %RSDo 2.0） in the range of 7.7-77.0 mg/mL was found. The results showed a good correlation with the manufacturer＇s and overall they all fell within acceptable limits of most pharmacopoeial monographs on AMK sulfate.
