Diagnostic and therapeutic challenges of gastrointestinal angiodysplasias: A critical review and view points

Gastrointestinal angiodysplasias (GIADs), also called angioectasias, are the most frequent vascular lesions. Its precise prevalence is unknown since most of them are asymptomatic. However, the incidence may be increasing since GIADs affect individuals aged more than 60 years and population life expectancy is globally increasing worldwide. They are responsible of about 5% to 10% of all gastrointestinal bleeding (GIB) cases. Most GIADs are placed in small bowel, where are the cause of 50 to 60% of obscure GIB diagnosed with video capsule endoscopy. They may be the cause of fatal severe bleeding episodes;nevertheless, recurrent overt or occult bleeding episodes requiring repeated expensive treatments and disturbing patient’s quality-of-life are more frequently observed. Diagnosis and treatment of GIADs (particularly those placed in small bowel) are a great challenge due to insidious disease behavior, inaccessibility to affected sites and limitations of available diagnostic procedures. Hemorrhagic causality out of the actively bleeding lesions detected by diagnostic procedures may be difficult to establish. No treatment guidelines are currently available, so there is a high variability in the management of these patients. In this review, the epidemiology and pathophysiology of GIADs and the status in the diagnosis and treatment, with special emphasis on small bowel angiodysplasias based on multiple publications, are critically discussed. In addition, a classification of GIADs based on their endoscopic characteristics is proposed. Finally, some aspects that need to be clarified in future research studies are highlighted.

Several ectoparasites parasitise nestlings decreasing their body condition,growth and *** minimise any loss of fitness due to ectoparasites,birds have developed a wide variety of defence mechanisms,potentially including hatching *** to the Tasty Chick Hypothesis(TCH),the cost of parasitism would be reduced if ectoparasites tend to eat on less immunocompetent nestlings,typically the last-hatched chick in asynchronously hatched broods,as they are in poor body *** predictions of the TCH are that immune capacity is lower in smaller nestlings than in larger ones and that parasites should provoke a more negative effect on smaller ***,we test these predictions in a population of Blue Tits(Cyanistes caeruleus)whose broods are parasitised by Hen Fleas(Ceratophyllus gallinae)and Blowflies(Protocalliphora azurea).We recorded the presence of both ectoparasites and analysed the immunocompetence(number of leucocytes per 10,000 erythrocytes and cutaneous immune response to phytohaemagglutinin)and body condition of smaller and larger nestlings within individual *** leucocyte count was higher in smaller nestlings than in larger ones,whereas the cutaneous immune response did not differ between smaller and larger *** nestlings,but not larger nestlings,had lower body mass when fleas were ***,by contrast,had no detectable negative effect on ***,our findings provide partial support to the *** immune capacity in smaller nestlings than in larger ones was not supported,but Hen Fleas seemed to negatively impact on smaller nestlings more than on larger ones.

Recovery Slope of Heart Rate Variability as an Indicator of Internal Training Load

The way in which the Root Mean Square of the Successive Differences between adjacent RR intervals (RMSSD) recovers immediately after exercise could be a good indicator of internal training load (ITL). The aim of this study is to design a recovery index based on RMSSD. Forteen healthy men took part in this study. The experiment lasted 2 weeks, with 4 separate (48 - 72 h) sessions. First session was an incremental treadmill test to determine ventilatory thresholds (VT1 and VT2) and maximal aerobic speed (MAS). Each subject ran at VT1 speed (second day), VT2 speed (third day) and a time-to-exhaustion test at MAS (fourth day). The duration of VT1 and VT2 loads was selected in such a way that the product intensity-duration (training load) was the same. HRV was measured from 10’ prior to test (Rest) to 30’ after completed (Recovery). Recovery slopes were calculated from RMSSD values at 10 and 30 minutes. Borg scale was recorded at the end of every test and the Training Impulse (TRIMP) values were calculated using Banister equations. The RMSSD values dropped substantially regardless of the intensity and the duration of exercise (average 4 ms). The RMSSD recovery was linear during the 30 min and different depending on the intensity of exercise. To propose a recovery index, we calculated the slope of RMSDD over the 30 minutes (slope-30) and also the first 10 minutes (slope-10). Given that the slopes presented an exponential behavior in relation with effort intensity, three curves were obtained (average values, plus SD and minus SD) defining a nomogram. For practical application, we propose: 1) to measure RMSSD the last 5 minutes of exercise and any period of 5 minutes during the first 30 minutes recovery;2) to calculate the slope of RMSSD between exercise and recovery;3) to compare with the nomogram.

Effects of feeding yeast(Saccharomyces cerevisiae), organic selenium and chromium mixed on growth performance and carcass traits of hair lambs

Yeasts and organic minerals are used in diets to improve health, productive performance and some carcass characteristics of ruminants and non-ruminants. Thirty-two lambs（Pelibuey×Katahdin; BW=（30.55±1.67） kg; n=8） were used in a 56-d feeding experiment to study the effects of different levels of live yeast（Saccharomyces cerevisiae; yeast）, selenium（Se） and chromium（Cr） mixed（Se-Cr）, and a mixture of yeast-Se-Cr on growth performance and carcass traits. Animals were stratified by body weight（BW） and randomly assigned to one of four treatments： 1） control group（0.0 g kg–1 yeast）; 2） yeast（1.50 g kg–1 dry matter intake（DMI） d–1）; 3） Se-Cr premix（1.5 mg kg–1 DMI d–1 for each mineral）; and 4） yeast-Se-Cr mixture. There were no treatment effects on final BW; whereas lambs fed Se-Cr or yeast-Se-Cr had higher（P〈0.05） DMI than animals supplemented with only yeast. Average daily gain（ADG）, gain：feed ratio, chop area, dorsal fat and carcass yield were similar（P〉0.05） among treatment groups. In conclusion, supplementation with yeast, Se-Cr mixed or yeast-Se-Cr did not improve ADG, final BW, back fat content and carcass yield of growing of Pelibuey×Katahdin lambs. Supplementation with Se-Cr and yeast-Se-Cr increased DMI, and approximately 250 g ADG animal–1 d–1 was produced with no negative effects on growth and health of the animals.

Modeling and multi-objective optimization of a gasoline engine using neural networks and evolutionary algorithms

In this paper, a multi-objective particle swarm optimization （MOPSO） algorithm and a nondominated sorting genetic algorithm II （NSGA-II） are used to optimize the operating parameters of a 1.6 L, spark ignition （SI） gasoline engine. The aim of this optimization is to reduce engine emissions in terms of carbon monoxide （CO）, hydrocarbons （HC）, and nitrogen oxides （NOx）, which are the causes of diverse environmental problems such as air pollution and global warming. Stationary engine tests were performed for data generation, covering 60 operating conditions. Artificial neural networks （ANNs） were used to predict exhaust emissions, whose inputs were from six engine operating parameters, and the outputs were three resulting exhaust emissions. The outputs of ANNs were used to evaluate objective functions within the optimization algorithms： NSGA-II and MOPSO. Then a decision-making process was conducted, using a fuzzy method to select a Pareto solution with which the best emission reductions can be achieved. The NSGA-II algorithm achieved reductions of at least 9.84%, 82.44%, and 13.78% for CO, HC, and NOx, respectively. With a MOPSO algorithm the reached reductions were at least 13.68%, 83.80%, and 7.67% for CO, HC, and NOx, respectively.

Minimal Vasovagal Dysautonomia in Patients with Rare or Unique Syncope

Introduction: It is common to find people sent to perform a Head Up Tilt Test (HUT) who suffered a single syncope, or syncopes that occur during certain periods and never appear again. We wonder how these people are different from those who have never had syncope. Methods: We found 300 patients who suffered only one (unique) or a maximum of 5 vasovagal syncopes during their life. And their HUT was positive for vasovagal dysautonomia. We compared them, with 120 healthy volunteers who have never had syncope. We try to explain how some constitutional predisposing factors act in these patients, and are associated with environmental triggers to precipitate the syncope. Results: We found differences between cases and controls in predisposing factors such as: heredity, joint hypermobility, baroreflex failure, venous compliance and some neurological diseases. Then an environmental factor acts as a trigger for syncope: prolonged standing, stress, pain and emotions, dehydration, use of certain drugs, abundant food. Conclusions: There are people with minimally expressed vasovagal dysautonomia who have an organic predisposition to present vasovagal syncopes (heredity, joint hypermobility, baroreflex failure, venous compliance, some neurological diseases, etc.). But this predisposition is not enough by itself to produce syncopes. One or more environmental factors must be added, acting as a trigger that would be the reason why these episodes are so infrequent.

Meckel's diverticulum diagnosis by video capsule endoscopy: A case report and review of literature

BACKGROUND Meckel’s diverticulum （MD） occurs predominantly in chil-dren and adolescents. It is rarely diagnosed in adults. Preoperative diagnosis is diffcult due to low sensitivity of the radiological imaging studies. The role of video capsule endoscopy （VCE） in the diagnosis of MD is unknown, and the endoscopic patterns are not defned. We will describe four of our cases of MD evaluated with VCE and make a review of the literature focusing on the endoscopic *** SUMMARYWe present four cases of MD confirmed by surgery. They were all adult males with ages going from 18 to 50 years, referred to our service from 2004 to 2018, due to obscure gastrointestinal bleeding （OGIB）. TheyGarcía-Compeán D et al . Merckel’s diverticulum diagnosis by VCEhad a history of 1 mo to 10 years of overt and occult bleeding episodes. Laboratory blood test showed an iron-deficiency anemia from 4 to 9 g/dL of hemoglo-bin that required multiple hospitalizations and blood transfusions in all cases. Repeated upper digestive endoscopies and colonoscopies were negative. Small bowel was examined with VCE, which revealed double lumen images in all cases, one with polyps and three with circumferential ulcers in the diverticulum. However, based on VCE findings, preoperative diagnosis of MD was suggested only in two patients. Capsule was retain-ed in one patient, which was recovered with surgery. The anatomopathological report revealed ulcerated ectopic gastric mucosa in all *** is useful for the diagnosis of MD. However, endo-scopic characteristics must be recognized in order to establish preoperative diagnosis.

Kinetic study of austenite formation during continuous heating of unalloyed ductile iron

The austenite formation kinetics in unalloyed cast ductile iron was studied on the basis of dilatometry measurements, and Avrami＇s equation was used to estimate the material＇s kinetic parameters. A continuous heating transformation diagram was constructed us-ing heating rates in the range of 0.06 to 0.83℃·s^-1. As the heating rate was augmented, the critical temperatures, c1A and Aα, as well as the intercritical range, which was evaluated as the difference between the critical temperatures, α c1Δ T =A_a- A_c1 , increased. At a low heating rate, the kinetics of austenite formation was slow as a consequence of the iron＇s silicon content. The effect of heating rate on k and n, the kinetic parameters of Avrami＇s equation, was also determined. Parameter n, which is associated with nucleation sites and growth geometry, de-creased with an increase in heating rate. In addition, parameter k increased with the increase of heating rate, suggesting that the nucleation and growth rates are carbon-and silicon-diffusion controlled during austenite formation under continuous heating.

The effect of Saccharomyces cerevisiae on digestion and mortality in the volcano rabbit(Romerolagus diazi)

An experiment was conducted to evaluate whether supplementation with a probiotic could enhance digestion and reduce mortality in the volcano rabbit in captivity. Two enclosures at Chapultepec Zoo, Mexico（114 individuals） were used in a crossover design（two periods of 60 days） with the following treatments： control group and supplementation with Saccharomyces cerevisiae（2×108 CFU/exhibit/day）. Supplementation with the probiotic negatively affected（P〈0.01） the digestibility of dry matter, organic matter, neutral detergent fiber（NDF） and energy. Mortality increased（P〈0.04） following supplementation with the probiotic（4.26% vs. 8.89%）, primarily in the juvenile rabbits. The results indicate that yeast supplementation in the volcano rabbit negatively affects digestion and mortality in captivity.
