Mechanical circulatory and/or respiratory assistance with extracorporeal membrane oxygenation (ECMO) has become a standard of care for patients with circulatory (venoarterial) and/or respiratory (venovenous) failure refractory to standard therapies. Adult patients with congenital heart disease are an increasingly recognized and growing population and include various groups, such as undiagnosed cases in childhood and palliated and/or corrected cases, which require subsequent care because of residual lesions, cardiac arrest/insufficiency, and arrhythmias, among other conditions. In addition, these patients are prone to developing pathologies that are typical of adulthood with a generally increased risk of morbidity and mortality because of their low reserves and organic damage associated with the underlying heart disease, which makes them candidates for ECMO. These patients represent an additional challenge in this therapy because malformations and the presence of a shunt can generally affect the usual cannulation methods and hemodynamic and oximetry monitoring. Thus, the configuration decision must be made on a case-by-case basis. Here, we present a cannulation method, venopulmonary artery ECMO, which provides hemodynamic and respiratory support, and may be ideal for patients with shunts and/or right ventricular dysfunction. To our knowledge, this is the first report of this configuration in patients with congenital heart diseases.

Chemotherapy and radiotherapy:Could they contribute to the development of new tumors and metastases?

Objective: Both chemotherapy and radiotherapy have demonstrated high effectiveness as the best mechanisms in the fight against cancer;however, various studies seem to confirm that they could also favor the development of other unwanted effects of great importance for the patient. The main objective of this study is to find out the possible existence of this type of links. Method: This is a systematic literature review that seeks to find out which and how long cases of late interactions related to chemotherapy and radiotherapy treatments have been known. The bibliographic review was carried out based on references published in the last five years. Results: Various studies confirm the possible relationship between chemotherapy and radiotherapy treatments with the development of new undesirable side effects, especially as a consequence of the hepatotoxicity generated in the case of chemotherapy and radiation in radiotherapy. However, in this last type of treatment, the problems raised are really few. Conclusions: The existence of a risk of suffering new unwanted side effects after different types of treatment seems to have been demonstrated, especially in the case of chemotherapy. In the case of radiotherapy, adverse effects are practically non-existent, although they are no less important.

Impact of device resistances in the performance of graphene-based terahertz photodetectors

In recent years, graphene field-effect-transistors(GFETs) have demonstrated an outstanding potential for terahertz(THz) photodetection due to their fast response and high-sensitivity. Such features are essential to enable emerging THz applications, including 6G wireless communications, quantum information, bioimaging and security. However, the overall performance of these photodetectors may be utterly compromised by the impact of internal resistances presented in the device, so-called access or parasitic resistances. In this work, we provide a detailed study of the influence of internal device resistances in the photoresponse of high-mobility dual-gate GFET detectors. Such dual-gate architectures allow us to fine tune(decrease) the internal resistance of the device by an order of magnitude and consequently demonstrate an improved responsivity and noise-equivalent-power values of the photodetector, respectively. Our results can be well understood by a series resistance model, as shown by the excellent agreement found between the experimental data and theoretical calculations. These findings are therefore relevant to understand and improve the overall performance of existing high-mobility graphene photodetectors.

Temporomandibular disorders (TMD) negatively affect quality of life, causing pain and restricted jaw movements. This study evaluates the effectiveness of Transcutaneous Electrical Nerve Stimulation (TENS) on TMD patients before orthodontic treatment at the Juarez Autonomous University of Tabasco. A quantitative, quasi-experimental study was conducted with 30 patients who met the inclusion criteria: aged 18 to 65 years, with TMJ pain symptoms, diagnosed with temporomandibular disorders using the Helkimo index, and who had provided informed consent for the study. The range of mandibular movement and the Visual Analog Scale (VAS) were measured to record values before therapy. The electrodes of the TENS therapy were positioned on both sides of the jaw in the preauricular area and the masseter muscle, the therapy was administered for 40 minutes, utilizing a frequency of 2 - 4 Hz and a pulse duration of 300 microseconds. Following this, the range of mandibular movement and the VAS were reassessed to evaluate the outcomes., significantly improved maximum mouth opening (37.7 ± 6.4 mm to 44.6 ± 5.88 mm), right laterality (3.86 ± 1.57 mm to 5.43 ± 1.21 mm), left laterality (3.13 ± 1.63 mm to 4.53 ± 1.61 mm), protrusion (4.01 ± 1.7 mm to 5.36 ± 1.32 mm), and pain perception (4.56 ± 2.045 to 2.03 ± 1.5) (P lar movement and reduces pain, making it a well-tolerated, non-invasive complementary treatment for TMD.

Dear Editor,Short-term fasting(up to 48 hours)activates strong physiological and molecular responses[1].We and others showed a p53-independent transcriptional activation of the cell cycle inhibitor p21 upon fasting,most strongly in the liver,muscles,and many other tissues[2,3].Studies with mice[4,5]and human patients[6]have shown the beneficial effects of short-term fasting during anti-cancer chemotherapy treatments.

Given the cultural and environmental potential presented by historical water infrastructures in urban contexts,this article states the evidence of a not much documented urban phenomenon in Portugal,the refurbishment of water heritage protected areas motivated by European-based regeneration *** employing a case-study design,four good practices-placed inÉvora,lisbon,Braga and Guimarães-are chosen to enlighten this growing and aware *** assets located there-theÁgua de Prata Aqueduct,theÁguas livres Aqueduct,the Sete Fontes water-supply system and the Zona de Couros industrial area,respectively-are geographical,historical and culturally characterised,while the projects that concern them are documented by visiting the areas and analysing the available ***,open-data platforms and heritage protection decrees are processed to graphically code the topography,hydrology,urban land use,infrastructures,landmarks and protected areas,employing QGIS free software for Geographic Information *** a result,the four practices are documented,discussed and compared.A table assesses and summarises their pre-existing and currently added values and a series of diagrams illustrates the landscape changes *** graphics validate the practices and update the status of the assets,enhancing the rediscovery of the existing landscapes and showing the main challenges and future opportunities faced by these areas.

The use of unmanned aerial vehicles(UAV)for forest monitoring has grown significantly in recent years,providing information with high spatial resolution and temporal *** with multispectral sensors allow the use of indexes such as the normalized difference vegetation index(NDVI),which determines the vigor,physiological stress and photo synthetic activity of *** study aimed to analyze the spectral responses and variations of NDVI in tree crowns,as well as their correlation with climatic factors over the course of one *** study area encompassed a 1.6-ha site in Durango,Mexico,where Pinus cembroides,Pinus engelmannii,and Quercus grisea *** images were acquired with UAV and information on meteorological variables was obtained from NASA/POWER *** ANOVA explored possible differences in NDVI among the three *** correlation was performed to identify the linear relationship between NDVI and meteorological *** differences in NDVI values were found at the genus level(Pinus and Quercus),possibly related to the physiological features of the species and their *** grisea had the lowest NDVI values throughout the year which may be attributed to its sensitivity to relative humidity and *** the use of UAV with a multispectral sensor for NDVI monitoring allowed genera differentiation,in more complex forest analyses hyperspectral and liDAR sensors should be integrated,as well other vegetation indexes be considered.
