Clinical Profiles at the Time of Diagnosis of SARS-CoV-2 Infection in Costa Rica During the Pre-vaccination Period Using a machine Learning Approach

The clinical manifestations of COVID-19,caused by the SARS-CoV-2,define a large spectrum of symptoms that are mainly dependent on the human host *** Costa Rica,more than 169,000 cases and 2185 deaths were reported during the year 2020,the pre-vaccination *** describe the clinical presentations at the time of diagnosis of SARS-CoV-2 infection in Costa Rica during the pre-vaccination period,we implemented a symptom-based clustering using machine learning to identify clusters or clinical profiles at the population level among 18,974 records of positive *** were compared based on symptoms,risk factors,viral load,and genomic features of the SARS-CoV-2 sequence.A total of 18 symptoms at time of diagnosis of SARS-CoV-2 infection were reported with a frequency>1%,and those were used to identify seven clinical profiles with a specific composition of clinical *** the comparison between clusters,a lower viral load was found for the asymptomatic group,while the risk factors and the SARS-CoV-2 genomic features were distributed among all the *** other distribution patterns were found for age,sex,vital status,and *** conclusion,during the pre-vaccination time in Costa Rica,the symptoms at the time of diagnosis of SARS-CoV-2 infection were described in clinical *** host co-morbidities and the SARS-CoV-2 genotypes are not specific of a particular profile,rather they are present in all the groups,including asymptomatic *** addition,this information can be used for decision-making by the local healthcare institutions(first point of contact with health professionals,case definition,or infrastructure).In further analyses,these results will be compared against the profiles of cases during the vaccination period.

The usage of renewable energies,including geothermal energy,is expanding rapidly *** low efficiency of geothermal cycles has consistently highlighted the importance of recovering heat loss for these *** paper proposes a combined power generation cycle(single flash geothermal cycle with trans-critical CO_(2) cycle)and simulates in the EES(Engineering Equation Solver)*** results show that the design parameters of the proposed system are significantly improved compared to the BASIC single flash ***,the proposed approach is optimized using the genetic algorithm and the Nelder-mead Simplex *** pressure,steam turbine output pressure,and CO_(2) turbine inlet pressure are three assumed variable parameters,and exergy efficiency is the target *** the default operating mode,the system exergy efficiency was 32%,increasing to 39%using the genetic algorithm and 37%using the Nelder-mead method.

Dynamics of lattice disorder in perovskite materials, polarization nanoclusters and ferroelectric domain wall structures

The nexus between classic ferroelectricity and the structure of perovskite materials hinges on the concept of lattice *** the ordered perovskites display short-range displacements of the central cations around their equilibrium points,the lattice disorder dynamically unfolds to generate a myriad of distorted rhombohedral lattices characterized by the hopping of the central cations across*** is discovered that the lattice disorder correlates with the emergence of minimum configuration energypathways for the central cations,resulting in spatially modulated ultrafast polarization nanocluster arrangements that are stabilized by the electric charge defects in the *** high-resolution phonon dispersion analyses encompassing molecular dynamics(mD)and density functional theory(DFT)simulations,we provide unequivocal evidence linking the hopping of central cations to the development of diffuse soft phonon modes observed throughout the phase transitions of the *** massive mD simulations,we unveil the impact of lattice disorder on the structures of domain walls at finite-temperature vis-à-vis collective activation and deactivation of***,our simulations demonstrate the development of hierarchical morphotropic phase boundary(mPB)nanostructures under the combined influence of externally applied pressure and stress relaxation,characterized by sudden emergence of zig-zagged monoclinic arrangements that involve dualshifts of the central *** findings have implications for tailoring mPBs in thin-film structures and for the light-induced mobilization of *** are finally uncovered to the exploration of lattice disorder through gradual shear strain application.

Benchtop tissue cultures have become increasingly complex in recent years,as more on-a-chip biological technologies,such as microphysiological systems(mPS),are developed to incorporate cellular constructs that more accurately represent their respective biological *** mPs have begun facilitating major breakthroughs in biological research and are poised to shape the field in the coming *** biological systems require integrated sensing modalities to procure complex,multiplexed datasets with unprecedented combinatorial biological *** this work,we expanded upon our polymer-metal biosensor approach by demonstrating a facile technology for compound biosensing that was characterized through custom modeling *** reported herein,we developed a compound chip with 3D microelectrodes,3D microfluidics,interdigitated electrodes(IDEs)and a *** chip was subsequently tested using the electrical/electrochemical characterization of 3D microelectrodes with 1 kHz impedance and phase recordings and IDE-based high-frequency(-1 mHz frequencies)impedimetric analysis of differential localized temperature recordings,both of which were modeled through equivalent electrical circuits for process parameter ***,a simplified antibody-conjugation strategy Was employed for a similar IDE-based analysis of the implications of a key analyte(L-glutamine)binding to the equivalent electrical ***,acute microfluidic perfusion modeling was performed to demonstrate the ease of microfluidics integration into such a polymer-metal biosensor platform for potential complimentary localized chemical ***,our work demonstrates the design,development,and characterization of an accessibly designed polymer-metal compound biosensor for electrogenic cellular constructs to facilitate comprehensive mPS data collection.

meningoencephalitis secondary to Listeria monocytogenes (L. monocytogenes) mainly affects newborns, the elderly and immunocompromised people;there are extremely rare cases in which said infection occurs in immunocompetent individuals. We present the case of a young adult patient without immunocompromise, who developed meningoencephalitis due to L. monocytogenes;This case is exceptional, since it occurred in an individual outside the classic age group, in addition to not having risk factors, which is why it should be considered an atypical causal agent.

Uremic Bleeding in 2 Patients with Dieulafoy’s Lesion: Case Report and Literature Review

Dieulafoy’s lesions are rare vascular malformations of the gastrointestinal tract. A Dieulafoy’s lesion is an aberrant vessel that does not reduce in caliber when it extends from the submucosa to the mucosa. Damage to this artery can cause severe and intermittent arterial bleeding from small vascular stumps that are difficult to visualize. Furthermore, these catastrophic bleeding episodes frequently result in hemodynamic instability and the need for transfusion of multiple blood products. Recently, uremic syndrome has been identified as a risk factor for gastric mucosal lesions. We present two clinical cases of acute digestive bleeding due to Dielafoy lesion with chronic kidney disease as the main cause, where two different therapies were performed endoscopically. We concluded with the results of our patients that the best therapy was the application of the hemostatic hemoclip on the injury vs the injection with adrenaline on the wound site. Uremia is identified as a risk factor for upper gastrointestinal bleeding in patients with pre-existing Dieulafoy’s lesion, as well as a higher incidence of new bleeding.

Formation potential and analysis of 32 regulated and unregulated disinfection by-products: Two new simplified methods

Water disinfection is an essential process that provides safe water by inactivating pathogens that cause waterborne diseases. However, disinfectants react with organic matter naturally present in water, leading to the formation of disinfection by-products(DBPs). multi-analyte methods based on mass spectrometry(mS) are preferred to quantify multiple DBP classes at once however, most require extensive sample pre-treatment and significant resources. In this study, two analytical methods were developed for the quantification of 32 regulated and unregulated DBPs. A purge and trap(P&T) coupled with gas chromatography mass spectrometry(GC-mS) method was optimized that automated sample pre-treatment and analyzed volatile and semi-volatile compounds, including trihalomethanes(THms), iodinated trihalomethanes(I-THms), haloacetonitriles(HANs), haloketones(HKTs) and halonitromethanes(HNms). LOQs were between 0.02-0.4 μg/L for most DBPs except for 8 analytes that were in the low μg/L range. A second method with liquid chromatography(LC) tandem mass spectrometry(mS/mS) was developed for the quantification of 10 haloacetic acids(HAAs) with a simple clean-up and direct injection. The LC-mS/mS direct injection method has the lowest detection limits reported(0.2-0.5 μg/L). Both methods have a simple sample pre-treatment, which make it possible for routine analysis. Hyperchlorination and uniform formation conditions(UFC) formation potential tests with chlorine were evaluated with water samples containing high and low TOC. Hyperchlorination formation potential test maximized THms and HAAs while UFC maximized HANs. Ascorbic acid was found to be an appropriate quencher for both analytical methods. Disinfected drinking water from four water utilities in Alberta, Canada were also evaluated.

Distal nucleotides affect the rate of stop codon read-through

Background:A key step in gene expression is the recognition of the stop codon to terminate translation at the correct ***,it has been observed that ribosomes can misinterpret the stop codon and continue the translation in the 3′UTR *** phenomenon is called stop codon read-through(SCR).It has been suggested that these events would occur on a programmed basis,but the underlying mechanisms are still not well ***:Here,we present a strategy for the comprehensive identification of SCR events in the Drosophila melanogaster transcriptome by evaluating the ribosomal density *** associated ribosomal leak rate was estimated for every event identified.A statistical characterization of the frequency of nucleotide use in the proximal region to the stop codon in the sequences associated to SCR events was ***:The results show that the nucleotide usage pattern in transcripts with the UGA codon is different from the pattern for those transcripts ending in the UAA codon,suggesting the existence of at least two mechanisms that could alter the translational termination ***,a linear regression models for each of the three stop codons was developed,and we show that the models using the nucleotides at informative positions outperforms those models that consider the entire sequence context to the stop ***:We report that distal nucleotides can affect the SCR rate in a stop-codon dependent manner.
