The Use of Triancinolone for the Treatment of Keloid Scars

Scars, when in good evolution, result in a smooth, thin and discreet tissue. Keloid scars, however, are a type of abnormal and exacerbated repair response to tissue injury, whether in surgical interventions or in various injuries, which pre.e.t in a prominent and gross way. In this context, there is an excess of collagen deposition in the tissue repair process, which can lead to the formation of keloids. The diagnosis of the condition pre.e.ted is made by the medical professional or by the patient himself after the surgical intervention or skin injury. Under this analysis, protruding, rough and bad-looking scars are identified. In addition, we highlight the existence of keloids similar to large tumors, described as jorge Lobo disease. The treatment encompasses massages, compressions, corticosteroids, chemotherapy, collagenase and cryotherapy. At first, we used corticosteroid-based massages, and then we started using compressive dressings until we started intrakeloid infiltrations with injectable triamcinolone. Triamcinolone 10 mg injectable—1/10—in 0.9% saline, with syringe and fixed needle 0.3 mm × 8 mm, intralesional infiltrate, in this context, proved to be effective for its treatment when applied sequentially and linearly. In cases where the medication was applied, there was an improvement after 21 days of application and a definitive improvement 2 months after the injury. In comparison, on the other hand, patients who were not subjected to the application of the medication may improve after 4 months of the injury or worsen compared to the initial case. We have come to the conclusion that this procedure may be one of the chosen ones for the treatment of keloid scars, being one of the most recommended for cases of keloid already installed.

Anorexia nervosa(AN) is a complex disorder with a strong genetic component. Comorbidities are frequent and there is substantial overlap with other disorders. The lack of understanding of the molecular and neuroanatomical causes has made it difficult to develop effective treatments and it is often difficult to treat in clinical practice. re.e.t advances in genetics have changed our understanding of polygenic diseases,increasing the possibility of understanding better how molecular pathways are intertwined. This review synthetizes the current state of genetic re.e.rch providing an overview of genome-wide association studies(GWAS) findings in AN as well as overlap with other disorders, traits, pathways, and imaging results. This paper also discusses the different putative global pathways that are contributing to the disease including the evidence for metabolic and psychiatric origin of the disease.

Core promoter: A critical region where the hepatitis B virus makes decisions

The core promoter(CP) of the viral genome plays an important role for hepatitis B virus(HBV) replication as it directs initiation of transcription for the synthesis of both the precore and pre.e.omic(pg) RNAs. The CP consists of the upper regulatory region and the basa core promoter(BCP). The CP overlaps with the 3'-end of the X open reading frames and the 5'-end of the precore region,and contains cis-acting elements that can independently direct transcription of the precore mRNA and pgRNA. Its transcription regulation is under strict control of viral and cellular factors. even though this regulatory region exhibits high sequence conservation,when variations appear,they may contribute to the persistence of HBV within the host,leading to chronic infection and cirrhosis,and eventually,hepatocellular carcinoma. Among CP sequence variations,those occurring at BCP may dysregulate viral gene expression with emphasis in the hepatitis B e antigen,and contribute to disease progression. In this review these molecular aspects and pathologic topics of core promoter are deeply evaluated.

Objective： To evaluate the treatment effect of acupuncture on patients with idiopathic trigeminal neuralgia（ITN） by case-control longitudinal blinded study. Methods： Sixty ITN patients and 30 healthy subjects were included. The ITN patients were randomly assigned to acupuncture group（15 cases）, sham-acupuncture group（15 cases） and carbamazepine group（30 cases）, respectively. Clinical orofacial evaluation（including pain intensity and medication doses）, re.e.rch diagnostic criteria for temporomandibular disorders（RDC/TMD） and Helkimo indexes（for functional evaluation of the masticatory system）, and quantitative sensory testing for sensory thresholds（gustative, olfactory, cold, warm, touch, vibration and superficial and deep pain） were.e.aluated before treatment, immediately after treatment, and 6 months after treatment. results： The mean pain intensity by the Visual Analogue Scale only decreased in the acupuncture group at the last evaluation（P=0.012）. Patients in the sham-acupuncture group had an increase in carbamazepine doses according to the prescriptions（P〈0.01）. There was a reduction in secondary myofascial pain and mandibular limitations at the acupuncture and sham-acupuncture groups, however only the acupuncture group kept the changes after 6 months（P〈0.01, P=0.023）. There was a decrease in mechanical thresholds in the acupuncture group（tactile, P〈0.01; vibration, P=0.027） and an increase in deep pain thresholds in both acupuncture and sham-acupuncture groups（P=0.013）. Conclusion： Acupuncture can be an option in the treatment of ITN due to its analgesic effect in both ITN and secondary myofascial pain associated with it.

Influence of flooding and vegetation on carbon,nitrogen,and phosphorus dynamics in the pore water of a Spartina alterniflora salt marsh

Four sites were selected in a salt marsh in the Bahia Blanca estuary （Argentina）：（1） low marsh （flooded by the tide twice daily） vegetated by S. alterniflora; （2） non-vegetated low marsh; （3） high marsh （flooded only in spring tides） vegetated by S. alterniflora; （4） non-vegetated high marsh. The pH and eh were measured in sediments, while dissolved nutrients （ammonium, nitrate, nitrite and phosphate） and particulate organic matter （POM） were determined in pore water, pH （6.2-8.7） was only affected by vegetation in low areas. eh （from -300 to 250 mV） was lower at low sites than at high ones; in the latter, the values were higher in the non-vegetated sediments. The POM concentration was greater in the high marsh than in the low marsh, with no effect of vegetation. Ammonium was the most abundant nitrogen nutrient species in pore water, except in the non-vegetated high marsh where nitrate concentration was higher. All nitrogen nutrients were affected by both flooding and vegetation. Phosphate was always pre.e.t in pore water at all sites throughout the year and its concentration varied within narrow limits, with no effect of flooding and greater values always at non-vegetated sites. Our results showed that the variability of the pore water composition within the marsh is greater than the temporal variation, meaning that both tidal flooding and vegetation are important in the dynamics of nutrients and organic matter in the sediment pore water.

One of the main tools for monitoring the condition of high voltage equipment is the measure.e.t of partial discharges(PD).The electromagnetic(eM)radiation originated from this degradation phenomenon can be captured by various types of ultra‐high fre-quency(UHF)antennas carefully designed and optimised for specific frequency ***,the pre.e.ce of environmental noise may limit the use of this *** types of monopole antennas normally used in UHF PD measure.e.t have been evaluated in order to validate the performance of a novel separation technique of eM ***,a new separation technique based on the energy ratio of the captured signals was developed,considering noise *** results re.e.led that the new technique allows an adequate separation,even when three sources act simultaneously.

echocardiography in centenarians： characteristics, utility and follow-up

Background echocardiography has been shown to be a valuable resource in the diagnosis of many cardiac conditions, and can be used in all age groups, from the fetus to the oldest old. In the context of an increasingly aging population, the impact and utility of echocar- diography in centenarians is largely unknown. This study is to determine whether echocardiography in centenarians aids in making clinical patient management decisions. Methods A retrospective review of echocardiograms from 1986 to 2014, at two affiliated tertiary centers, in individuals who were 100 years or older at the time of the examination. Patient and echocardiogram characteristics, management decisions based on echocardiography, and mortality were documented. results 114 centenarians had echocardiograms, with ages ranging from 100 to 107 years （101 - 1.4 years）. In 82 of the centenarians evaluated （72%）, no changes in management occurred as a consequence of the echocardiogram. From all management changes directly related to the echocardiogram, 81% （n = 26） of these corresponded to medication adjustments; interventional or surgical procedures followed the echocardiogram only in 4% （n = 5） of the total number of centenarians. echocardiogram-based changes in management were only significant in patients that were re.e.red for congestive heart failure （P = 0.02）. After the echocardiogram was performed, 1-month and 1-year mortality were 15% and 47%, respectively. The median survival after the echocardiogram was obtained was 13 months （range 0.03 to 145 months）, with no difference if there was a change or no change in manage- ment （P = 0.21）. Conclusions Among centenarians undergoing echocardiography, despite additional diagnostic information, echocardio- grams in centenarians influence management in a minority of cases, most commonly in the form of medication changes for treatment of heart failure. Landslides induced by prolonged rainfalls are frequent mass movements along the northeastern portion of the Sierra Madre Oriental in Mexico,causing significant damage to *** this work,we have studied the connection between rainfall and landslides in the Santa Rosa Canyon,a catchment located in the northeastern Mexico.A landslide database triggered by major storms and hurricanes that have hit the region over the past 30 years was analyzed.A total of 92 rainfall events in the Santa Rosa Canyon were studied to determine the amount of precipitation needed to trigger shallow *** each event the duration(D,in hours)and the cumulated rainfall event(e,in mm)were determined by using historical rainfall data from weather stations located near the study *** have proposed an eD threshold for rainfall-induced landslides with durations 0.5ress the conditions that trigger these events in the study *** analyzing the obtained threshold,it has been established that almost 60 mm of a daily rainfall accumulation is required to trigger shallow landslides in the study *** estimation is consistent with other calculations made for areas close to the Santa Rosa ***,we validated the predictive capability of the threshold with a different set of rainfall data that did not result in landslides performing a straightforward re.e.ver operating characteristic analysis.A good approach was obtained,especially for rainfall events with daily *** could be used as input information in the design of a landslide early warning system for the northeastern Mexico,and replicated for other landslide prone areas in the region.
