The histopathological diagnosis of gastric mucosal biopsy and endoscopic mucosal resection/endoscopic submucosal dissection specimens is important,but the diagnostic criteria,terminology,and grading system are not the same in the East and West.A structurally invasive focus is necessary to diagnose carcinoma for most Western pathologists,but Japanese pathologists make a diagnosis of cancer based on severe dysplastic cytologic atypia irrespective of the presence of *** the Vienna classification was introduced to reduce diagnostic discrepancies,it has been difficult to adopt due to different concepts for gastric epithelial neoplastic *** pathologists experience much difficulty making a diagnosis because we are influenced by Japanese pathologists as well as Western *** is geographically close to Korea,and academic exchanges are ***,Korean doctors are familiar with Western style medical *** a result,the terminology,definitions,and diagnostic criteria for gastric intraepithelial neoplasia are very heterogeneous in *** solve this problem,the Gastrointestinal Pathology Study Group of the Korean Society of Pathologists has made an effort and has suggested guidelines for differential diagnosis:(1) a diagnosis of carcinoma is based on invasion;(2) the most important characteristic of low grade dysplasia is the architectural pattern such as regular distribution of crypts without severe branching,budding,or marked glandular crowding;(3) if nuclear pseudostratification occupies more than the basal half of the cryptal cells in three or more adjacent crypts,the lesion is considered high grade dysplasia;(4) if severe cytologic atypia is present,careful inspection for invasive foci is necessary,because the risk for invasion is very high;and(5) other structural or nuclear atypia should be evaluated to make a final decision such as cribriform pattern,papillae,ridges,vesicular nuclei,high nuclear/cytoplasmic ratio,loss

Dear Editor, During CO2 assimilation, the reductive pentose phos- phate cycle generates triose phosphates （triose-P） at the expense of ATP and NADPH generated by photosynthetic light reactions. Triose-P is retained within chloroplasts to either regenerate the CO2 acceptor ribulose-l,5-bi- sphosphate or to feed into transitory starch synthesis. Alternatively, triose-P can be exported into the cytosol to produce sucrose. To maintain optimum rates of photo- synthesis, the rate of triose-P export from the chloroplast stroma to the cytosol is necessarily adjusted to the demands for photoassimilate export. The triose-P/phosphate （pi） translocator （TPT） mediates the export of stromal triose-P into the cytosol in a counter exchange for cytosolic pi.

目的利用乳腺癌患者连续PET/CT显像和MRI所得的参数评价肿瘤的分化程度。方法回顾性研究78例乳腺癌患者,患者均行连续的PET/CT显像和MRI[注射;F-脱氧葡萄糖（FDG）后1 h和2 h分别行PET/CT显像,随后立刻行MRI检查]。PET/CT显像后计算1 h和2 h FDG的最大标准摄取值(SUV;1和SUV;2)、代谢肿瘤体积（MTV）和总病灶糖酵解（TLG）。动态对比增强MRI参数用动态增强曲线进行分析。对有关表观弥散系数（ADC）值的弥散加权成像（DWI）进行评价。组织学分级为1级和2级的乳腺癌归为低等级肿瘤,3级的归为高等级肿瘤。结果 78例乳腺癌患者的病灶组织学分级属3级,2级和1级的分别为45、26和7个病灶。高等级肿瘤患者的平均ADC值（0.99±0.19）低于低等级肿瘤患者的平均ADC值（1.12±0.32;P=0.007）。高等级肿瘤患者的SUV;1、MTV2.5[平均标准摄取值(SUV;)取2.5时的MTV]和TLG2.5(SUV;取2.5时的TLG)值要高于低等级肿瘤患者[SUV;1:7.92±4.5和6.19±3.05, P=0. 09, MTV2.5:7.90±9.32和4.38±5.10,P=0.095;TLG2.5:40.83±59.17和19.66±26.08, P=0.082]。但高、低等级肿瘤患者间的其他参数差异无统计学意义。受试者工作特征（ROC）曲线分析发现高等级肿瘤的平均ADC值曲线下面积最大,达到0.681（95%CI:0.566～0.782）。结论在乳腺癌PET显像和MRI的多种功能性参数中,较低的平均ADC值也许能预测肿瘤较差的分化程度。

Outcome of transarterial chemoembolization-based multi-modal treatment in patients with unresectable hepatocellular carcinoma

AIM:To investigate the efficacy and safety of transarterial chemoembolization(TACE)-based multimodal treatment in patients with large hepatocellular carcinoma(HCC).METHODS:A total of 146 consecutive patients were included in the analysis,and their medical records and radiological data were reviewed ***:In total,119 patients received TACE-based multi-modal treatments,and the remaining 27 received conservative *** survival(P<0.001)and objective tumor response(P=0.003)were significantly better in the treatment group than in the conservative *** subgroup analysis,survival benefits were observed not only in the multi-modal treatment group compared with the TACE-only group(P=0.002)but also in the surgical treatment group compared with the loco-regional treatment-only group(P<0.001).Multivariate analysis identified tumor stage(P<0.001)and tumor type(P=0.009)as two independent pre-treatment factors for *** adjusting for significant pre-treatment prognostic factors,objective response(P<0.001),surgical treatment(P=0.009),and multi-modal treatment(P=0.002)were identified as independent post-treatment prognostic ***:TACE-based multi-modal treatments were safe and more beneficial than conservative *** surgery after successful downstaging resultedin long-term survival in patients with large,unresectable HCC.

Strategies to overcome resistance to epidermal growth factor receptor monoclonal antibody therapy in metastatic colorectal cancer

Administration of monoclonal antibodies(mAbs)against epidermal growth factor receptor(EGFR)such as cetuximab and panitumumab in combination with conventional chemotherapy substantially prolongs survival of patients with metastatic colorectal cancer(mCRC).However,the efficacy of these mAbs is limited due to genetic variation among patients,in particular K-ras *** discovery of K-ras mutation as a predictor of non-responsiveness to EGFR mAb therapy has caused a major change in the treatment of *** that inhibit transformation caused by oncogenic alterations of Ras and its downstream components such as BRAF,MEK and AKT seem to be promising cancer therapeutics as single agents or when given with EGFR *** multiple therapeutic strategies to overcome EGFR mAb-resistance are under investigation,our understanding of their mode of action is *** drug development based on stringent preclinical data,biomarker validation,and proper selection of patients is of paramount importance in the treatment of *** this review,we will discuss diverse approaches to overcome the problem of resistance to existing anti-EGFR therapies and potential future directions for cancer therapies related to the mutational status of genes associated with EGFRRas-ERK and pi3K signalings.

Flexural Behavior of FRP-Reinforced Concrete Composite Member

Concrete Filled FRP (Reinforced Polymeric Plastic) Tubes (CFFT) and Reinforced Concrete Filled FRP Tubes (RCFFT) are known to have the capability to enhance structural performance in terms of structural stability, ductility, as well as chemical resistance when compared with conventional concrete members. In this study, the authors evaluate the structural performance of the CFFT and the RCFFT through flexural tests for the purpose of applying the members as flexural ones. Moreover, the compressive behavior of the CFFT and the RCFFT members was investigated to examine their confinement effects. Based on the experimental and analytical results of the compressive behavior of the members, equations for estimating the ultimate compressive strengths of the CFFT and the RCFFT were proposed. In addition, the degree of improvement on the flexural performance of the RCFFT member strengthened by the FRP was analyzed from the flexural tests.

Gallbladder lymphangioma:A case report and review of the literature

Lymphangiomas are rare, benign tumors of the lymphatic system, usually present in children aged 5 years and younger. Because they are asymptomatic until the mass enlarges to cause symptoms, most lymphangiomas are diagnosed at adulthood incidentally. We experienced a case of a 60-year-old man diagnosed with a cystic lymphangioma of the gallbladder, which was successfully resected without any complication. Magnetic resonance imaging and magnetic resonance cholangiopancreatography were very helpful for the diagnosis of the cystic lesion around the gallbladder as were ultrasonography and computed tomography scan. These showed a multi-lobulated cystic mass with intact cystic duct and bile duct in the gallbladder fossa. The patient underwent an open cholecystectomy and the histological findings were consistent with a cystic lymphangioma of the gallbladder. We here report the case of cystic lymphangioma of the gallbladder with a review of the literature.

Trans-10,cis-12, not cis-9,trans-11, conjugated linoleic acid decreases ErbB3 expression in HT-29 human colon cancer cells
