Effect of robot-assisted gait training on the biomechanical properties of burn scars:a single-blind,randomized controlled trial

Background:Robot-assisted gait training(RAGT)is more effective in the range of motion(ROM)and isometric strength in patients with burns than conventional ***,concerns have been raised about whether RAGT might negatively affect the scars of patients with ***,we investigated the effects of RAGT-induced mechanical load on the biomechanical properties of burn ***:This was a single-blind,randomized clinical trial conducted on inpatients admitted to the Department of Rehabilitation Medicine between September 2020 and August *** was conducted for 30 min per day,five days a week for 12 weeks and the control group received conventional gait training for 12 *** pre-training ROM of lower extremity joints was evaluated and the levels of melanin,erythema,trans-epidermal water loss,scar distensibility and elasticity were assessed before training and at 4 and 12 weeks after ***,19 patients in the gait assistance robot(GAR)group and 20 patients in the control group completed the 12-week trial and all ***:There were no significant differences in the epidemiologic characteristics,pre-training ROM of joints and pre-training biomechanical properties of the burn scar between the groups(p>0.05 for all).None of the patients experienced skin abrasion around the burn scar where the fastening belts were applied or musculoskeletal or cardiovascular adverse events during the *** thickness significantly increased in both groups(p=0.037 and p=0.019)and scar distensibility significantly decreased in the control group(p=0.011)during the *** was significantly decreased in the GAR group during the training(p=0.038).The GAR and control groups showed significant difference in the change in the values of hysteresis between pre-training and 12 weeks after training(p=0.441 and p=0.049).Conclusions:RAGT significantly decreased hysteresis in hypertrophic burn scars and did not cause a significant decrease

Nickel-iron nanocrystalline alloys with different compositions and grain sizes were fabricated by electroplating for MEMS devices. The iron content of the deposits was changed by varying the nickel/iron ion ratio in the electrolyte. X-ray diffraction (XRD) analysis was applied for measuring the strength of the texture and grain size of the deposits. The nickel/iron atom ratio of the deposits was analyzed by EDS. The hardness of the alloys was evaluated by Vickers hardness indenter. The internal stress of the deposits was measured by thin film stress measurement using Stoney's formula. Surface morphology and roughness were investigated by scanning electron microscopy (SEM) and atomic force microscopy (AFM). Hardness and internal stress mechanism of the electroplated structure as a function of Fe ion content and current density were revealed. With increasing the iron content, the hardness and internal stress of the deposits increase. An excellent correlation between the increase in the internal stress and the loss of (200) texture were found.

For real parameters αand β such that 0≤α 〈 1 〈β, we denote by S（α,β） the class of normalized analytic functions which satisfy the following two-sided inequality：where U denotes the open unit disk. We find a sufficient condition for functions to be in the class S（α,β） and solve several radius problems related to other well-known function classes.

Systematic review on the surgical treatment for T1 gallbladder cancer

AIM:To evaluate the efficacy of simple and extended cholecystectomy for mucosa(T1a) or muscularis(T1b) gallbladder(GB) cancer. METHODS:Original studies on simple and extended cholecystectomy for T1a or T1b GB cancer were searched from MEDLINE(PubMed) ,Cochrane Library,EMBase,and CancerLit using the search terms of GB,cancer/carcinoma/tumor/neoplasm. RESULTS:Twenty-nine out of the 2312 potentially relevant publications met the eligibility *** the 1266 patients with GB cancer included in the publications,706(55.8%) and 560(44.2%) had T1a and T1b GB cancer,*** cholecystectomy for T1a and T1b GB cancer was performed in 590(83.6%) and 375(67.0%) patients,respectively(Pchoice was simple cholecystectomy for T1a GB cancer patients with a 5-year survival rate of 100%.Lymph node metastasis was detected in 10.9% of the T1b GB cancer patients and in 1.8%of the T1a GB cancer patients,respectively(Pcholecystectomy represents the adequate treatment of T1a GB *** is no definite evidence that extended cholecystectomy is advantageous over simple cholecystectomy for T1b GB cancer.

Bispectral index monitoring as an adjunct to nurse-administered combined sedation during endoscopic retrograde cholangiopancreatography

AIM:To determine whether bispectral index (BIS) monitoring is useful for propofol administration for deep sedation during endoscopic retrograde cholangiopancreatography (ERCP). METHODS:Fifty-nine consecutive patients with a variety of reasons for ERCP who underwent the procedure at least twice between 1 July 2010 and 30 November 2010. This was a randomized cross-over study, in which each patient underwent ERCP twice, once with BIS monitoring and once with control monitoring. Whether BIS monitoring was done during the first or second ERCP procedure was random. Patients were intermittently administered a mixed regimen including midazolam, pethidine, and propofol by trained nurses. The nurse used a routine practice to monitor sedation using the Modified Observer's Assessment of Alertness/Sedation (MOAA/S) scale or the BIS monitoring. The total amount of midazolam and propofol used and serious side effects were compared between the BIS and control groups. RESULTS:The mean total propofol dose administered was 53.1 ± 32.2 mg in the BIS group and 54.9 ± 30.8 mg in the control group (P = 0.673). The individual propofol dose received per minute during the ERCP procedure was 2.90 ± 1.83 mg/min in the BIS group and 3.44 ± 2.04 mg in the control group (P = 0.103). The median value of the MOAA/S score during the maintenance phase of sedation was comparable for the two groups. The mean BIS values throughout the procedure (from insertion to removal of the endoscope) were 76.5 ± 8.7 for all 59 patients in using the BIS monitor. No significant differences in the frequency of < 80% oxygen saturation, hypotension (< 80 mmHg), or bradycardia (< 50 beats/min) were observed between the two study groups. Four cases of poor cooperation occurred, in which the procedure should be stopped to add the propofol dose. Factors associated with use of gastric cancer screening services in Korea

AIM: To identify the factors associated with participation in gastric cancer screening programs. METHODS: Using data from the Korea National Health and Nutrition Examination Survey 2005 (KNHANES Ⅲ), a nationwide health-related surveyin Korea, a cross-sectional study was performed to investigate the multiple factors associated with gastric cancer screening attendance among persons aged at least 40 years. The study population included 4593 individuals who completed a gastric cancer screening questionnaire and had no previous cancer history. Four groups ofindividual-level or environmental level covariates were considered as potential associated ***: Using KNHANES Ⅲdata, an estimated31.71% of Korean individuals aged at least 40 years adhered to gastric cancer screening recommendations. Subjects who graduated from elementary school[adjusted odds ratio (aOR), 1.66; 95% CI: 1.21-2.26], middle/high school (aOR, 1.38; 95% CI: 1.01-1.89), and university or higher (aOR, 1.64; 95% CI: 1.13-2.37) were more likely to undergo gastric cancer screening than those who received no formal education at all. The population with the highest income tertile had more attendance at gastric screening compared to those with the lowest income tertile (aOR, 1.36; 95% CI: 1.06-1.73). Gastric screening was also negatively associated with excessive alcohol consumption (aOR, 0.71; 95% CI: 0.53-0.96). A positive attitude to preventive medical evaluation was significantly associated with better participation in gastric cancer screening programs (aOR, 5.26; 95% CI: 4.35-6.35). CONCLUSION: Targeted interventions for vulnerable populations and public campaigns about preventive medical evaluation are needed to increase gastric cancer screening participation and reduce gastric cancer mortality.

Standard vs magnifying narrow-band imaging endoscopy for diagnosis of Helicobacter pylori infection and gastric precancerous conditions

BACKGROUND Advances in endoscopic imaging enable the identification of patients at high risk of gastric ***,there are no comparative data on the utility of standard and magnifying narrow-band imaging(M-NBI)endoscopy for diagnosing Helicobacter pylori(***)infection,gastric atrophy,and intestinal *** To compare the diagnostic performance of standard and M-NBI endoscopy for *** gastritis and precancerous *** In 254 patients,standard endoscopy findings were classified into mosaic-like appearance(type A),diffuse homogenous redness(type B),and irregular redness with groove(type C).Gastric mucosal patterns visualized by M-NBI were classified as regular round pits with polygonal sulci(type Z-1),more dilated and linear pits without sulci(type Z-2),and loss of gastric pits with coiled vessels(type Z-3).RESULTS The diagnostic accuracy of standard and M-NBI endoscopy for *** gastritis was 93.3%and 96.1%,*** gastric precancerous conditions,the accuracy of standard and M-NBI endoscopy was 72.0%vs 72.6%for moderate to severe atrophy,and 61.7%vs.61.1%for intestinal metaplasia in the corpus,*** to type A and Z-1,types B+C and Z-2+Z-3 were significantly associated with moderate to severe atrophy[odds ratio(OR)=5.56 and 8.67]and serum pepsinogen I/II ratio of≤3(OR=4.48 and 5.69).CONCLUSION Close observation of the gastric mucosa by standard and M-NBI endoscopy is useful for the diagnosis of *** gastritis and precancerous conditions.
