BACKGROUND: Recent findings on emergency department (ED) patient experience surveys and concerns for the low response rates challenge the quality and reliability of the survey reports. We assessed the consistency of an ED patient experience survey report and identifi ed the effects of patient demographics on ED patient experiences. METHODS: We conducted a prospective, cross-sectional study at a university-based ED from July to December 2017. We obtained ED patient experience scores from an institutional version (IS) survey and the Press Ganey Associates-distributed survey (PGA). We compared top box scores from the two reports using frequency analysis and performed multivariable logistic regressions to identify associations between IS patient demographics and scores. RESULTS: We obtained 289 PGA and 234 IS responses. The IS reported signifi cant, higher top box scores in doctor-specifi c patient questions compared to PGA (all four P-values < 0.01). Female, Christian and White patients were more likely to give top box scores (OR 3.07, OR 2.22 and OR 2.41, P-value < 0.05, respectively). CONCLUSION: We found significant differences in ED patient experience scores between the IS and PGA surveys. We recommend that healthcare providers consider patient demographic variables when interpreting ED experience score reports. Multiple survey techniques and distribution methods may be adopted to best capture ED patient experiences.

Objective:Determine the effects of hyoid myotomy and suspension (HMS) without concurrent palatal or tongue base sleep surgery for obstructive sleep apnea (OSA).Method:Patients with OSA treated with HMS were identified using CPT code (21685) at an academic and private sleep surgery *** who underwent concurrent palatal or tongue base sleep surgery were *** included simultaneous procedures,apnea-hypopnea index (AHI),lowest oxyhemoglobin saturation (LSAT),and Epworth Sleepiness Scale (ESS).Results:Nineteen patients with OSA underwent HMS without palatal or tongue base sleep *** average age at surgery was (55.3 ± 13.5) years with a majority of patients being male (71％).Concurrent procedures included the following:torus mandibularis excision (n =1),endoscopic sinus surgery (n =4),septoplasty (n =10),inferior turbinate reduction (n =12),and nasal valve repair (n =2).AHI improved signiflcantly from (39.7 ± 21.2) eventslh to (22.6 ± 22.7) events/h after HMS (P ＜ 0.01),which represented a 43％ *** significantly increased from (82.2％ ± 9.9％) to (86.6％ ± 6.2％),P ＜ *** was no improvement in ESS after surgery (8.2 ± 4.4) to (8.3 ± 5.2),P =0.904.A subset of patients with severe OSA (AHI ＞ 30 events/h) had an improvement in AHI from (49.9 ± 16.6) events/h to (29.1 ± 24.9) events/h,P ＜ ***:HMS without palatal or tongue base sleep surgery improves OSA *** can be considered as a valid option in the treatment of OSA in appropriately-selected patients.

A series of experimental studies of the innovative propulsor named Collective and Cyclic Pitch Propeller(CCPP) applied to an underwater vehicle were designed and performed at the Australian Maritime College, University of Tasmania. The bollard pull and captive model tests were conducted to investigate the characteristics of CCPP and to examine the effect of different parameter settings to its performance. The results show that the CCPP is able to generate effective manoeuvring forces in various operational condition. In addition, the obtained results in the form of force coefficients provide a useful empirical model for the simulation and control of an underwater vehicle equipped with this propulsor.

Objective:To evaluate the use of Gallium-68 prostate-specific membrane antigen positron emission tomography/computed tomography(^(68)Ga-PSMA PET/CT),compared with conventional CT abdomen/pelvis(CTAP)and whole body single photon emission CT bone scan(BS),for detection of local or distant metastasis following biochemical failure/recurrence in post-prostatectomy ***:We conducted a review of our prospectively maintained,institutional database to identify 384 patients with post-prostatectomy biochemical failure/recurrence who underwent PSMA PET/CT,CTAP and BS from February 2015 to August 2017 in Nepean Hospital,tertiary referral *** results of the three imaging modalities were analysed for their ability to detect local recurrence and distant *** PET/CT and CTAP imaging were separately performed on the same day and the BS was performed within several days(mostly in 24 h).Difference in detection rates was determined between the modalities and the Chi square test was used to determine ***:A total of 384 patients were identified with a median prostate-specific antigen(PSA)of 0.465 ng/mL(interquartile range =0.19-2.00 ng/mL).Overall,PSMA PET/CT was positive for 245(63.8%)patients whereas CTAP and BS were positive in 174 patients(45.3%).A total of 98 patients(25.5%)had local or distant metastasis detected on PSMA only,while 20 patients(5.2%)had recurrences detected on CTAP but not on PSMA PET/***:The use of PSMA PET/CT has a higher detection rate of predicted local or distant metastasis compared to CTAP and BS in the staging of patients with biochemical recurrences after radical prostatectomy.

Long-range and fast transport of coherent excitons is important for the development of high-speed excitonic circuits and quantum computing ***,most of these coherent excitons have only been observed in some low-dimensional semiconductors when coupled with cavities,as there are large inhomogeneous broadening and dephasing effects on the transport of excitons in their native states in ***,by confining coherent excitons at the 2D quantum limit,we first observed molecular aggregation-enabled‘supertransport’of excitons in atomically thin two-dimensional(2D)organic semiconductors between coherent states,with a measured high effective exciton diffusion coefficient of ~346.9 cm^(2)/s at room *** value is one to several orders of magnitude higher than the values reported for other organic molecular aggregates and low-dimensional inorganic *** coupling to any optical cavities,the monolayer pentacene sample,a very clean 2D quantum system(~1.2 nm thick)with high crystallinity(J-type aggregation)and minimal interfacial states,showed superradiant emission from Frenkel excitons,which was experimentally confirmed by the temperature-dependent photoluminescence(PL)emission,highly enhanced radiative decay rate,significantly narrowed PL peak width and strongly directional in-plane *** coherence in monolayer pentacene samples was observed to be delocalised over~135 molecules,which is significantly larger than the values(a few molecules)observed for other organic thin *** addition,the supertransport of excitons in monolayer pentacene samples showed highly anisotropic *** results pave the way for the development of future high-speed excitonic circuits,fast OLEDs,and other optoelectronic devices.

Purpose: To report a novel Y-suture transfixation technique to improve success rate of high-risk Descemets stripping automated endothelial keratoplasties (DSAEK). Design: Retrospective non-comparative case series. Participants: Twenty five high-risk patients undergoing DSAEK at one tertiary care institution. Methods: A retrospective evaluation of consecutively enrolled high-risk DSAEK cases performed by three surgeons at one institution during a 3-year period (2007-2010). Main outcome measures were graft adherence and suture-related complications. Results: Highrisk factors were defined as followed: prior incisional glaucoma surgery (58%), presence of vitreous in the anterior chamber (15%), anterior chamber intraocular lens (8%), graft dislocation (8%), previous DSAEK (8%), and iris trauma/aphakia (3%). Twenty-five high-risk patients underwent DSAEK with Y-suture transfixation. Postoperative follow-up ranged from 3 to 27 months. One patient had dislocation due to hypotony from prior trabeculectomy (4%), requiring repeat DSAEK. Neither primary graft failure nor suture-related complication was observed. Conclusion: The Y-suture transfixation technique is associated with improved success rate of high-risk DSAEK transplant.
