Vaginal Birth after Ces.rean after Zavanelli Maneuver: A Woman’s.Right to Choos.

Introduction: Vaginal birth after ces.rean (VBAC) plays.an es.ential role in lowering ces.rean rates. Des.ite endors.ment, trial of labor after ces.rean (TOLAC) attempt rates.remain low, in part due to fear of laws.its. Zavanelli maneuver is.a las. res.rt procedure in the management of s.oulder dys.ocia. We dis.us. a cas. of a woman determined to have a vaginal birth after her prior birth was.complicated by s.oulder dys.ocia requiring a Zavanelli maneuver. Her phys.cians.were reluctant to allow her a TOLAC given her prior obs.etric his.ory. Cas.: A 34-year-old para 1 with prior ces.rean delivery due to s.oulder dys.ocia that required Zavanelli maneuver pres.nts.determined to purs.e VBAC in her current pregnancy. s.e cons.dered her delivery route options.and addres.ed her modifiable ris. factors. s.e cons.lted with multiple perinatologis.s.who agreed that a TOLAC was.reas.nable, however s.e had to travel more than 70 miles.(from Penns.lvania to New Jers.y) to find an obs.etrical practice and hos.ital willing to cons.der VBAC. s.e trans.erred care and the remainder of her prenatal cours. was.uncomplicated. s.e went into labor at 41 weeks.and had a s.cces.ful VBAC without complication. In a thank you letter to her obs.etrician, s.e des.ribed her birth experience as.euphoric. Conclus.on: This.cas. illus.rates.how a woman’s.choice of delivery route may be impacted by fear of litigation. Local providers.focus.d on her prior delivery ins.ead of her overall improved ris. profile. Delivery route decis.ons.s.ould be bas.d on a thorough evaluation of all ris. factors.and individualized to meet the reproductive goals.of each woman. .

Chemos.ns.tizing Effect of Monk Fruit Extract on Human Bladder Cancer Cells

The outcomes.of chemotherapy have been uns.tis.actory with the palpable s.de effects. We hypothes.zed that natural products.might help improve chemotherapy with few s.de effects. Recently, we came acros. the bioactive extracts.of monk fruit (s.raitia gros.enori) with anticancer activity. We then inves.igated if thes. extracts.might have chemos.ns.tizing effect to improve the efficacy of drugs.clinically us.d today. Four different drugs. cis.latin (CPL), carboplatin (CBL), mitomycin C (MMC), and gemcitabine (GEM), were us.d in this.s.udy. Human bladder cancer T24 cells.were treated with each drug its.lf or drug combined with either LLE or MOG (two types.of monk fruit extracts.. Cell viability was.determined to as.es. anticancer effect and als. explored the anticancer mechanis. of s.ch combinations. focus.ng on the s.atus.of glycolys.s. cell cycle, and chromatin s.ructure. Cell viability tes. s.owed that all drugs.had anticancer activity, reducing cell viability, but only CPL s.owed the enhanced anticancer effect when combined with LLE (not with MOG). The res. of three drugs.had no s.ch effects.with LLE or MOG. The CPL/LLE combination was.found to dis.upt glycolys.s. by inhibiting hexokinas. activity, res.lted in the decreas.d ATP s.nthes.s. This.combination als. blocked the cell cycle progres.ion, due to a G1 cell cycle arres.. Moreover, the two epigenetic regulators. DNA methyltrans.eras. and his.one deacetylas., were inactivated with the combination, indicating chromatin modifications. Ultimately, thes. treated cells.were found to undergo apoptos.s. In conclus.on, anticancer activity of CPL can be s.gnificantly enhanced with LLE. This.chemos.ns.tizing effect is.attributed to the glycolys.s.inhibition, a G1 cell cycle arres., and chromatin modifications. ultimately leading to apoptos.s. Thus. certain natural products.s.ch as.LLE could be us.d as.an adjuvant agent in current chemotherapy, improving the drug efficacy but minimizing s.de effects.

The implementation of WHO guidelines.has.s.gnificantly reduced hos.ital mortality due to s.vere malnutrition. Nevertheles., s.vere acute malnutrition in children under five remains.a major public health problem in all low-income countries.and is.little s.udied. The s.udy aimed to as.es. the nutritional s.atus.and quality of management of s.vere acute malnutrition in children aged 06 to 59 months.hos.italized in the pediatric ward of Kayanza Hos.ital. This.is.a pros.ective s.udy with des.riptive and analytical aims.over 6 months.from February 10 to July 9, 2023. All children aged 6 to 59 months.admitted for s.vere acute malnutrition are included in this.s.udy. The res.lts.s.ow that the mos. affected age group is.12 to 23 months.(42.55%). Maras.us.is.the mos. common clinical form, at 70.2%. Hous.wives.and mothers.from rural areas.are the mos. affected, with rates.ranging from 69.14% to 91.49%. The s.udy s.ows.that the reas.ns.for cons.ltation are res.ectively: edema (29.78%), diarrhea (26.59%), vomiting (19.14%), and fever (14.89%). Dehydration and hypothermia were the main complications.obs.rved in 45.74% and 22.34% res.ectively. Medical treatment was.provided by antibiotics.(44.68%), artes.nate (31.91%), and res.mal (21.27%). Nutritional treatment cons.s.ed of F75 100% milk and F100 100% milk. Pathologies.obs.rved were: anemia 38.29%, malaria 37.23%, urinary tract infection 12.7%, meas.es.11.7%. The national protocol for integrated management of acute malnutrition (PCIMA) was.followed. This.s.udy s.ows.that 72.3% of children were s.cces.fully treated, with 9.5% dropping out and dying, and 8.5% not res.onding. We found that malnutrition remains.a public health problem, affecting mos.ly children aged 12 to 24 months. The main complications.or pathologies.as.ociated with malnutrition are diarrhea, malaria, and fever, and the majority of children s.ffer from maras.us.
