Objectives: To describe our technique for the implantation of the Thoraflex Hybrid prosthesis for replacement of the aortic arch in a safe and reproducible way. materials: Thoraflex™ Hybrid Plexus Device (Terumo Aortic). Design: Drawing on our own experience over the past 4 years in the management of acute type A aortic dissection, we have distilled the essentials of our “Frozen Elephant Trunk” technique which have led us through our own learning curve to the improved management of this taxing condition. method/ Results: Small extension of the median sternotomy incision along the medial border of sternocleidomastoid muscle. End to side graft anastomosis near the origin of the left subclavian artery during cooling on bypass towards 20 degrees. Attention to cardiac protection and maintenance of cerebral perfusion during the shortened corporeal arrest period. Excellent results in 24 consecutive AAAD patients with just one hospital mortality. Conclusions: We believe we are entering a new phase in the treatment of AAAD, facilitated by the availability of a hybrid prosthesis which combines expanding stent technology with familiar surgical graft material. Our particular management of the left subclavian artery and of the cerebral circulation during implantation has contributed to an expeditious and reproducible method of treating dissection within the arch of the aorta and beyond.

Small and micro unmanned aircraft are the focus of scientific interest due to their wide range of *** often operate in a highly unstable flight environment where the application of new morphing wing technologies offers the opportunity to improve flight *** investigated concept comprises port and starboard adjustable wings,and an adaptive elastoflexible membrane serves as the lifting *** focus is on the benefits of the deforming membrane during the impact of a one-minus-cosine type *** a low Reynolds number of Re=264000,the morphing wing model is investigated numerically by unsteady fluid-structure interaction ***,the numerical results are validated by experimental data from force and moment,flow field,and deformation ***,with the rigid wing as the baseline,the flexible case is investigated,focusing on the advantages of the elastic *** all configurations studied,the maximum amplitude of the lift coefficient under gust load shows good agreement between the experimental and numerical *** the decay of the gust,they differ more the higher the aspect ratio of the *** considering the flow field,the main differences are due to the separation behavior on the upper side of the *** flow reattaches earlier in the experiments than in the simulations,which explains the higher lift values observed in the *** at one intermediate configuration does the lift amplitude of the rigid configuration exceeds that of the flexible by about 12%,with the elastic membrane resulting in a smaller and more uniform peak load,which is also evident in the wing loading and hence in the root bending moment.

Hierarchically ordered macro-mesoporous TiO2 films (Ti-ma-me) were fabricated on fluorine-doped tin oxide (FTO) substrates through the confinement self-assembly method. The prepared Ti-ma-me possesses periodically ordered structure and a large specific surface area, which was applied as an interfacial layer between the nanocrystalline TiO2 film (P25-TiO2) and FTO electrode in the dye-sensitized solar cell (DSSC). The introduction of a Ti-ma-me interfacial layer increased the shortcircuit current density (Jsc) from 7.49 to 10.65 mA/cm2 and the open-circuit voltage (Voc) from 0.65 to 0.70 V as the result of its improved light harvesting efficiency by allowing for the high roughness factor and enhanced multiple internal reflection or scattering as well as reducing the back-transport reaction by blocking direct contact between the electrolyte and FTO electrode. Therefore, the photovoltaic conversion efficiency (η) was improved by 83% from 3.04% to 5.55%, as compared to a device using a bare P25 TiO2 photoanode.

Colorectal cancer(CRC)is a heterogeneous disease,arising from many possible etiological *** heterogeneity can have important implications for CRC prognosis and clinical *** studies of CRC risk and prognosis—as well as clinical trials for the treatment of CRC—must therefore be sensitive to the molecular phenotype of colorectal tumors in patients under *** this review,we describe four tumor markers that have been widely studied as reflections of CRC heterogeneity:(i)microsatellite instability(mSI)or DNA mismatch repair(mmR)deficiency,(ii)the CpG island methylator phenotype(CImP),and somatic mutations in(iii)BRAF and(iv)*** tumor markers have been used to better characterize CRC epidemiology and,increasingly,may be used to guide clinical *** beyond these traditional tumor markers,we also briefly review some more novel markers likely to be of clinical ***,recognizing that none of these individual tumor markers are isolated attributes but,rather,a reflection of broader tumor phenotypes,we review some of the hypothesized etiological pathways of CRC development and their associated clinical differences.

We develop a generalization of Nesterov’s accelerated gradient descent method which is designed to deal with orthogonality *** demonstrate the effectiveness of our method,we perform numerical experiments which demonstrate that the number of iterations scales with the square root of the condition number,and also compare with existing state-of-the-art quasi-Newton methods on the Stiefel *** experiments show that our method outperforms existing state-of-the-art quasi-Newton methods on some large,ill-conditioned problems.
