epstein-Barr vi.us (EBV) mucocutaneous ulcerati.n i. a rare compli.ati.n of i.munosuppressi.n that results i. pai.ful ulcerati.n of the tongue or gi.gi.a, and less commonly, refractory ulcerati.n of the gastroi.testi.al tract. Hi.h cli.i.al suspi.i.n i. requi.ed, as fai.ure to di.gnose EBV mucocutaneous ulcerati.n may result i. si.ni.i.ant morbi.i.y and mortali.y. We report the case of a 64-year-old female renal transplant reci.i.nt requi.i.g admi.si.n to hospi.al for management of severe oral and epi.astri. pai.. Exami.ati.n revealed a large, superfi.i.l, well-ci.cumscri.ed ulcer at the base of the tongue. Blood tests suggested a secondary i.munodefi.i.ncy characteri.ed by mi.d leucopeni., hypogammaglobuli.aemi., and low memory B-cells wi.h normal i.munophenotype. Endoscopy revealed four, cratered ulcers i. the pre-pylori. regi.n of the stomach. A core bi.psy of the tongue ulcer confi.med EBV mucocutaneous ulcerati.n. The pati.nt’s i.munosuppressi.n was opti.i.ed, whi.h resulted i. complete resoluti.n of the tongue and gastri. ulcers at ei.ht and twelve weeks of follow-up respecti.ely.
