A retrospective analysis of pain and opioid usage in head and neck free flap reconstruction

Rigidity(or stiffness)of materials and extracellular matrix has proven to be one of the most significant extracellular physicochemical cues that can control diverse cell behaviors,such as contractility,motility,and spreading,and the resultant pathophysiological *** 2D materials engineered with tunable rigidity have enabled researchers to elucidate the roles of matrix biophysical cues in diverse cellular events,including migration,lineage specification,and mechanical ***,the recent findings accumulated under 3D environments with viscoelastic and remodeling properties pointed to the importance of dynamically changing rigidity in cell fate control,tissue repair,and disease ***,here we aim to highlight the works related with material/matrix-rigidity-mediated cell and tissue behaviors,with a brief outlook into the studies on the effects of material/matrix rigidity on cell behaviors in 2D systems,further discussion of the events and considerations in tissue-mimicking 3D conditions,and then examination of the in vivo findings that concern material/matrix *** current discussion will help understand the material/matrix-rigidity-mediated biological phenomena and further leverage the concepts to find therapeutic targets and to design implantable materials for the treatment of damaged and diseased tissues.

The elevational distributions of bacterial communities in natural mountain forests,especially along large elevational gradients,have been studied for many ***,the distributional patterns that underlie variations in soil bacterial communities along small-scale elevational gradients in urban ecosystems are not yet well *** Illumina MiSeq DNA sequencing,we surveyed soil bacterial communities at three elevations on Zijin Mountain in Nanjing City:the hilltop(300 m a.s.l.),the hillside(150 m a.s.l.),and the foot of the hill(0 m a.s.l.).The results showed that edaphic properties differed significantly with *** community composition,rather than alpha diversity,strongly differed among the three elevations(Adonis:R2=0.12,P<0.01).Adonis and DistLM analyses demonstrated that bacterial community composition was highly correlated with soil pH,elevation,total nitrogen(TN),and dissolved organic carbon(DOC).The degree scores,betweenness centralities,and composition of keystone species were distinct among the *** results demonstrate strong elevational partitioning in the distributions of soil bacterial communities along the gradient on Zijin *** pH and elevation together drove the smallscale elevational distribution of soil bacterial *** study broadens our understanding of distribution patterns and biotic co-occurrence associations of soil bacterial communities from large elevational gradients to short elevational gradients.

Purpose To determine the amount of familiarization sessions required by breast cancer survivors to achieve a reliable meas-urement of muscle function assessed using isokinetic *** Twenty-six breast cancer survivors performed three isokinetic knee extension tests separated by,at least,48 *** isokinetic testing protocol included one warm-up set of 10 submaximal knee extensions at 120°/s,followed by two sets of four maximal knee extensions at 60°/s,with 2-min rest interval between *** torque(PT),time to peak torque(TPT),angle of peak torque(APT),and average power(AP)of each trial was used for the assessment of testing *** change in the mean,typical error,coefficient of variation and intraclass correlation coefficients(ICC2.1)were calculated to determine test-retest *** For PT,change in mean was lower between trials 2 and 3 than between trials 1 and 2(4.18% and 13.18%,respec-tively),and ICC was greater between trials 2 and 3 than between trials 1 and 2(0.962 and 0.818,respectively).For TPT and APT,ICC was clinically acceptable only between trials 2 and 3(0.757 and 0.803,respectively).For AP,change in mean was clinically acceptable between trials 2 and 3(9.84%),while ICC met acceptable reliability between both,trials 1 and 2 and,trials 2 and 3(0.756 and 0.891,respectively).Conclusion At least one familiarization session is adequate to achieve reliable measurements of muscle function using isokinetic dynamometry,while avoiding the impact of learning effect of the measurements in breast cancer survivors.

MRI Classification of Subtalar Joint Osteoarthritis Using a Novel Scoring System

Background: Plain radiography usual method to detect degeneration in the subtalar and talonavicluar joints. MRI is a better way to fully characterise non-ossified structures, such as articular cartilage, marrow tissue and synovial fluid and therefore detect changes of arthritis. The motivation behind this study was to develop a quantitative way to score arthritic changes to the subtalar and talonavicular joints using MRI. The developed system will then be used as a research tool and in the close assessment and monitoring of patients with hindfoot degenerative disease. Methods: The MRI scans of thirty consecutive subjects with foot and ankle pain were retrospectively evaluated. Images were interpreted independently by three musculoskeletal radiologists in order to determine intra-observer reliability as well as the inter-observer reliability of the score. Five features of osteoarthritis were scored in the Subtalar joint and the Talonavicular joint. These were cartilage morphology, subarticular marrow, subarticular cyst, marginal osteophytes and synovitis. Results: For the 30 MRI scans the mean score for the Subtalar joint ranged from 11.7 to 14.4 and for the Talonavicular joint ranged from 3.7 to 5.6. The inter-observer correlation for the Subtalar joint between the three readers ranged between 0.53 and 0.83 for the individual features but overall was excellent at 0.76. For the Talonavicular joint the total correlation was good at 0.67. The inter-observer ICC for the total score was 0.75 which showed excellent agreement between the three readers. The total intra-observer correlation was excellent. Conclusions: The current work has shown excellent reliability for the scoring system. It will be a useful tool to diagnose and monitor disease progression of the Subtalar and Talonavicluar joints.

＜正＞ The novel mechanism of functional selectivity （differential activation of pathways linked to a single receptor） has been shown for dopamine hD2L receptor regulated endpoints such as GIRK. MAPK. ACase. etc. The current work explored the hypothesis that functionally selective ligands induce unusual receptor conforma-tional states that lead to differential activation. We docked the propylnorapo-morphine enantiomers [ RNPA ＆ SNPA] to the D2R receptor to identify potential residues of importance, and then made and expressed selected mutant receptors. WT D2R binding confirmed that RNPA forms p - OH H bonds with both S5.46 and T3.37 whereas the m OH interacts with S5.42, while SNPA H- bonds to S5.42 （p ＆ m OH）. When mutated, differential effects were seen on functional endpoints （e.g.. S5.42A caused loss of D2R mediated effects on ACase. but not MAPK or AA release）. These data demonstrate that single point receptor mutations can make a "normal" ligand become functionally selective, or change the character of a functionally selective compound. We hypothesize that ligandspecific residue interactions contribute to the conformational changes needed to result in activation of specific heterotrimeric complexes.

A fast, green and readily reproducible microwave-based method for the production of high quality silver nanoparticles （AgNPs） in high yield is presented. Starch is used as a stabilizing agent with few pentose different reducing carbohydrates as D-ribose, D-arabinose and L-arabinose. From the UV-vis peak profile spectra of the solutions of the silver nanoparticles, the authors have investigated the size of the NPs together with the average diameter, shape, and aggregation state of the colloidal AgNPs. TEM measurements and EDX analysis have confirmed the morphology of our AgNPs.
