Background:Discordance in hepatocellular carcinoma(HCC)staging between pre-transplant imaging and explant pathology is associated with an increased risk of recurrence and *** aim was to evaluate variables that predicted concordance/discordance in the era of new generation locoregional therapies(LRT)and improved radiologic technology in ***:A single-center retrospective study was performed on patients who received a liver transplant for HCC between ***-and post-LT variables,including type of LRT,downstaging(DS),transplant time period,and radiologic response to LRT,were analyzed for concordance/***-Meier analysis was used to assess post-LT ***:Of 146 patients transplanted within Milan Criteria(MC),discordance rates(understaged)were 45%.Discordance was associated with≥3 HCC lesions at diagnosis but not newer generation LRT(transarterial radioembolization/stereotactic body radiation therapy),traditional LRT or *** differences in discordance were seen between transplant periods(2008-2013 vs.2014-2019),but those within MC in the earlier period had higher concordance rates.A trend was observed between DS and ***:HCC stage discordance remains common and poorly *** was associated with three or more HCC lesions at the time of *** within MC transplanted between 2008-2013 was associated with concordance,while a trend was noted between DS and *** other pre-or post-LT variables predicted discordance/*** was associated with decreased survival.

Therapeutic molecules for multiple human diseases identified from pigeon pea(Cajanus cajan ***.)through GC–MS and molecular docking

Molecular mechanism behind the therapeutic potential of pigeon pea over the human diseases such as rheumatoid arthritis,breast cancer,type II diabetes,malaria,measles and sickle cell disease were revealed through docking of GC–MS identified phyto-compound ligands with candidate disease *** the 242 ligands,three dimensional structures of 47 compounds had to be drawn using ChemSketch and the remaining structures were retrieved from PubChem and docked with the active sites of candidate *** molecules identified through docking were further subjected to ADMET analysis and promising drug candidates were identified for each *** paper presents a precise account of the chemoprofile of pigeon pea leaves,stems and seeds,interaction of these molecules with target proteins and suggests 26 highly potential molecules which are drug candidates for multiple human *** pea seeds are especially proven as invaluable source for therapeutic molecules.

Multiple Myeloma(MM)is a common haematological malignancy that is associated with a high rate of venous thromboembolism(vTE)with almost 10% of patients suffering thrombosis during their disease *** studies have shown that,despite current thromboprophylaxis strategies,vTE rates in MM remain disappointingly *** pathophysiology behind this consistently high rate of vTE is likely multifactorial.A number of factors such as anti-thrombin deficiency or raised coagulation Factor vIII levels may confer resistance to heparin in these patients,however,the optimal method of clinically evaluating this is unclear at present,though some groups have attempted its characterisation with thrombin generation testing(TGT).In addition to testing for heparin resistance,TGT in patients with MM has shown markedly varied abnormalities in both endogenous thrombin potential and serum thrombomodulin *** from these thrombin-mediated processes,other mechanisms potentially contributing to thromboprophylaxis failure include activated protein C resistance,endothelial toxicity secondary to chemotherapy agents,tissue factor abnormalities and the effect of immunoglobulins/“M-proteins”on both the endothelium and on fibrin fibre *** thus appears clear that there are a multitude of factors contributing to the prothrombotic milieu seen in MM and further work is necessitated to elucidate which factors may directly affect and inhibit response to anticoagulation and which factors are contributing in a broader fashion to the hypercoagulability phenotype observed in these patients so that effective thromboprophylaxis strategies can be employed.

The glenohumeral joint(GHJ)allows for a wide range of motion,but is also particularly vulnerable to episodes of *** GHJ instability is especially frequent among young,athletic populations during contact sporting *** first time dislocators can be managed non-operatively with a period of immobilization and rehabilitation,however certain patient populations are at higher risk for recurrent instability and may require surgical intervention for adequate *** of the optimal treatment strategy should be made on a case-by-case basis while weighing both patient specific factors and injury patterns(i.e.,bone loss).The purpose of this review is to describe the relevant anatomical stabilizers of the GHJ,risk factors for recurrent instability including bony lesions,indications for arthroscopic vs open surgical management,clinical history and physical examination techniques,imaging modalities,and pearls/pitfalls of arthroscopic soft-tissue stabilization for anterior glenohumeral instability.

Quercetin: A Promising Flavonoid with a Dynamic Ability to Treat various Diseases, Infections, and Cancers

Quercetin is a multifaceted dietary flavonoid with a multitude of biologic activities that can be used to treat various ailments. These include cancer, bacterial and viral infections, cardiovascular disease, and diabetes. A greater emphasis on cancer is discussed within this paper by highlighting some of the beneficial qualities of quercetin without including other related dietary flavonoids and quercetin analogs. In vitro and in vivo analysis are evaluated without making recommendations on dosage, dosing regiments, or administration since quercetin has not been subjected to rigorous clinical trials despite the significant amount of research that has been conducted with quercetin.

Post-Natal Spontaneous Resolution of a Congenital Pulmonary Airway Malformation in an Infant: Plain Radiographic and CT Manifestations

We present a case of a near-term infant who developed respiratory distress, shortly after birth. Chest X-ray showed a right sided tension pneumothorax (PTX) with a large air containing structure at the base of the right hemithorax. The pneumothorax was partially evacuated with needle aspiration at which time the patient became asymptomatic, despite a small residual pneumothorax. CT scan of the chest confirmed a multilocular air-filled structure within the right lower lobe, most likely consistent with a congenital pulmonary airway malformation (CPAM). Subsequently, there was spontaneous resolution of the PTX and CPAM prior to surgical intervention. This case illustrates post-natal spontaneous resolution of a CPAM, thus obviating the need for surgical removal.

The focus of environmental regulations has changed significantly since the introduction of the bioassay as a standard means of assessing environmental impact. Prominent in this change is an increasing emphasis on protecting the integrity of natural ecosystems, which incorporate community- and system-level properties as well as organismal and population processes. Consequently, support for the use of multispecies testing has widened to include not only ecologists in academia but environmental scientists in the regulatory and industrial sector as well. The reason for this trend is clear: the additional environmental realism gained from tests utilizing communities of organisms allows for greater insight into the potential hazard of chemicals and other forms of human activity to natural ecosystems that cannot be obtained from single species tests alone. Many of the problems cited for multispecies testing early in their evolution as a hazard assessment tool have been refuted or overcome. In particular, the use of natural microbial communities minimizes several shortcomings typically associated with multispecies toxicity testing. This discussion includes the utility of microcosm and mesocosm tests using aquatic microbial communities as hazard assessment tools in conjunction with accumulating information on their performance in toxicity testing protocols. An increasing body of experimental evidence supports an expansion in the use of these tests for a variety of regulatory and research purposes. A shift in research focus is needed, however, to answer remaining questions and further refine standard protocols for these valuable ecotoxicological tools.

Objective:Leptin(LEP)is an obesity-associated adipokine associated with tumor cell *** examined the relevance of genetic variants of LEP and leptin receptor(LEPR)to colorectal cancer(CRC)survival by using data from the Newfoundland Familial Colorectal Cancer ***:A total of 532 patients newly diagnosed with CRC between 1997 and 2003 were followed up until April *** on their demographics and lifestyles were collected via *** of blood samples was performed with the Illumina Human Omni-Quad Bead *** Cox models were used to assess the relationships of 35 tag single-nucleotide polymorphisms(SNPs)in LEP and LEPR with overall survival(OS),disease-free survival(DFS),and CRC-specific ***:At the gene level,LEP was associated with DFS(P=0.017),and LEPR was associated with both DFS(P=0.021)and CRCspecific survival(P=0.013)in patients with *** single-SNP analysis,LEP rs11763517,LEPR rs9436301,and LEPR rs7602 were associated with DFS after adjustment for multiple *** LEPR haplotypes G-C-T(rs7534511-rs9436301-rs1887285)and A-A-G(rs7602-rs970467-rs9436748)were associated with prolonged OS among patients with CRC overall(G-C-T:HR,0.63;95%CI,0.43–0.93;A-A-G:HR,0.59;95%CI,0.38–0.91)and those diagnosed with colon cancer(G-C-T:HR,0.54;95%CI,0.34–0.86;A-A-G:HR,0.49;95%CI,0.29–0.83).Similar results were observed for ***,significant interactions were found among LEPR rs7602(A vs.G),LEPR rs1171278(T vs.C),red meat intake,and BMI status:the associations between these variants and prolonged DFS were limited to patients with below-median red meat consumption and body mass index(BMI)variations in the LEP and LEPR genes were associated with survival of patients after CRC *** LEP/LEPR-CRC survival association was modified by participants’red meat intake and BMI.
