BACKGROUnD Metabolic dysfunction-associated steatotic liver disease(MASLD),characterised by hepatic lipid accumulation,causes inflammation and oxidative stress accompanied by cell damage and *** injury(LI)is also frequently reported in patients hospitalised with coronavirus disease 2019(COVID-19),while preexisting MASLD increases the risk of LI and the development of COVID-19-associated *** of injury at the cellular level remain unclear,but it may be significant that severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)which causes COVID-19,uses angiotensin-converting expression enzyme 2(ACE2),a key regulator of the‘anti-inflammatory’arm of the renin-angiotensin system,for viral attachment and host cell *** To determine if hepatic ACE2 levels are altered during progression of MASLD and in patients who died with severe *** ACE2 protein levels and localisation,and histological fibrosis and lipid droplet accumulation as markers of MASLD were determined in formalin-fixed liver tissue sections across the MASLD pathological spectrum(isolated hepatocellular steatosis,metabolic dysfunction-associated steatohepatitis(MASH)+/-fibrosis,end-stage cirrhosis)and in post-mortem tissues from patients who had died with severe COVID-19,using ACE2 immunohistochemistry and haematoxylin and eosin and picrosirius red staining of total collagen and lipid droplet areas,followed by quantification using machine learning-based image pixel *** ACE2 staining is primarily intracellular and concentrated in the cytoplasm of centrilobular hepatocytes and apical membranes of bile duct ***,ACE2 protein levels are elevated in non-fibrotic MASH compared to healthy controls but not in the progression to MASH with fibrosis and in ***2 protein levels and histological fibrosis are not associated,but ACE2 and liver lipid droplet content are significantly correlated across the MASLD *** A

Gastrointestinal and liver disease in patients with schizophrenia:A narrative review

Schizophrenia is a severe mental illness which can have a devastating impact onan individual’s quality of life. Comorbidities are high amongst patients and lifeexpectancy is approximately 15 years less than the general population. Despite thewell-known increased mortality, little is known about the impact of gastrointestinaland liver disease on patients with schizophrenia. We aimed to reviewthe literature and to make recommendations regarding future care. Literaturesearches were performed on PubMed to identify studies related to gastrointestinaland liver disease in patients with schizophrenia. High rates of chronic liverdisease were reported, with non-Alcoholic Fatty Liver Disease being of particularconcern;antipsychotics and metabolic syndrome were contributing factors. Ratesof acute liver failure were low but have been associated with antipsychotic useand paracetamol overdose. Coeliac disease has historically been linked to schizophrenia;however, recent research suggests that a causal link is yet to be *** is emerging regarding the relationships between schizophrenia andpeptic ulcer disease, inflammatory bowel disease and irritable bowel syndrome;clinical vigilance regarding these conditions should be high. Patients with schizophreniapoorly engage with bowel cancer screening programmes, leading to latediagnosis and increased mortality. Clozapine induced constipation is a significantissue for many patients and requires close monitoring. There is a significantburden of gastrointestinal and liver disease amongst patients with *** levels of support from all members of the medical team are essential toensure that appropriate, timely care is provided.

Human thrombin for the treatment of gastric and ectopic varices

AIM:To evaluate the efficacy of human thrombin in the treatment of bleeding gastric and ectopic ***:Retrospective observational study in a Tertiary Referral *** January 1999-October 2005,we identified 37 patients who were endoscopically treated with human thrombin injection therapy for bleeding gastric and ectopic *** details including age,gender and aetiology of liver disease/segmental portal hypertension were *** thrombin was obtained from the Scottish national Blood Transfusion Service and prepared to give a solution of 250 IU/mL which was injected via a standard injection *** patient case notes were reviewed and the total dose of thrombin given along with the number of endoscopy sessions was *** haemostasis rates,rebleeding rates and mortality were catalogued along with the incidence of any immediate complications which could be attributable to the thrombin *** duration of follow up was also *** study was conducted according to the United Kingdom research ethics ***:Thirty-seven patients were included.33 patients(89%) had thrombin(250 U/mL) for gastric varices,2(5.4%) for duodenal varices,1 for rectal varices and 1 for gastric and rectal varices.(1) Gastric varices,an average of 15.2 mL of thrombin was used per ***-bleeding occurred in 4 patients(10.8%),managed in 2 by a transjugular intrahepatic portosystemic shunt(TIPSS)(one unsuccessfully who died) and in other 2 by a distal splenorenal shunt;(2) Duodenal varices(or type 2 isolated gastric varices),an average of 12.5 mL was used per patient over 2-3 endoscopy ***-bleeding occurred in one patient,which was treated by TIPSS;and(3) Rectal varices,an average of 18.3 mL was used per patient over 3 endoscopy *** re-bleeding occurred in this ***:Human thrombin is a safe,easy to use and effective therapeutic option to control haemorrhage from gastric and ectopic varices.

The management of complications in liver disease is often complex and *** has undergone a period of rapid expansion with numerous novel and specialized endoscopic modalities that are of increasing value in the investigation and management of the patient with liver *** this review,relevant literature search and expert opinions have been used to provide a brief overview and update of the current endoscopic management of patients with liver disease and portal *** main areas covered are safety of endoscopy in patients with liver disease,the use of standard endoscopy for the treatment of varices and the role of new endoscopic modalities such as endoscopic ultrasound,esophageal capsule,argon plasma coagulation,spyglass and endomicroscopy in the investigation and treatment of liver-related gastrointestinal and biliary *** is clear that the role of the endoscopy in liver disease is well beyond that of just treating *** the technology in endoscopy expands,so does the role of the endoscopist in liver disease.

Development and characterization of 3D-printed electroconductive pHEMA-co-MAA nP-laden hydrogels for tissue engineering

Tissue engineering(TE)continues to be widely explored as a potential solution to meet critical clinical needs for diseased tissue replacement and tissue *** this study,we developed a poly(2-hydroxyethyl methacrylate-co-methacrylic acid)(pHEMA-co-MAA)based hydrogel loaded with newly synthesized conductive poly(3,4-ethylene-dioxythiophene)(PEDOT)and polypyrrole(PPy)nanoparticles(nPs),and subsequently processed these hydrogels into tissue engineered constructs via three-dimensional(3D)*** presence of the nPs was critical as they altered the rheological properties during ***,all samples exhibited suitable shear thinning properties,allowing for the development of an optimized processing window for 3D *** were 3D printed into pre-determined disk-shaped configurations of 2 and 10 mm in height and diameter,*** observed that the nPs disrupted the gel crosslinking efficiencies,leading to shorter degradation times and compressive mechanical properties ranging between 450 and 550 *** conductivity of the printed hydrogels increased along with the nP concentration to(5.10±0.37)×10^(−7)S/*** vitro studies with cortical astrocyte cell cultures demonstrated that exposure to the pHEMA-co-MAA nP hydrogels yielded high cellular viability and proliferation ***,hydrogel antimicrobial studies with staphylococcus epidermidis bacteria revealed that the developed hydrogels affected bacterial *** together,these materials show promise for various TE strategies.

Characteristics and Preliminary Causes of Tropical Cyclone Remote Precipitation over China

In this study,the characteristics and preliminary causes of tropical cyclone remote precipitation(TRP)over China during the period from 1979 to 2020 are *** indicated that approximately 72.42%of tropical cyclones(TCs)in the Western Pacific produce TRP over *** peak months for TRP are July and *** four key regions of TRP are the adjacent areas between the Sichuan and Shaanxi Provinces,the northern coast of the Bohai Sea,the coast of the Yellow Sea,and the southern coast *** typical distance between the station with TRP and the TC center ranges from 1500 to 2500 *** of these stations are situated north to 60˚west of north of the *** south–west water vapor transportation on the west side of the TC is crucial to *** has a decreasing trend because of the decrease in the number of TCs that generate *** the perspective of large-scale environmental conditions,a decrease in the integrated horizontal water vapor transport in China's Mainland,the weakening of upward motion at approximately 25˚–35˚n,which is inconducive to convection,and an increase in low-level vertical wind shear,which is unfavorable for the development of TC in areas with high frequencies of TRP-related TCs,are the factors that result in the decreasing trend of TRP.

Robust Machine Learning Mapping of sEMG Signals to Future Actuator Commands in Biomechatronic Devices

A machine learning model for regression of interrupted Surface Electromyography(sEMG)signals to future control-oriented signals(e.g.,robot’s joint angle and assistive torque)of an active biomechatronic device for high-level myoelectric-based hierarchical control is proposed.A Recurrent neural network(Rnn)was trained using output data,initially obtained from offline optimization of the biomechatronic(human–robot)device and shifted by the prediction *** input of the Rnn consisted of interrupted sEMG signals(to mimic signal disconnections)and previous kinematic signals of the assistive *** Rnn with a 0.1-s prediction horizon could predict the control-oriented joint angle and assistive torque with 92%and 86.5%regression accuracy,respectively,for the test *** proposed approach permits a fast,predictive,and direct estimation of control-oriented signals instead of an iterative process that optimizes assistive torque in the inverse dynamic simulation of a multibody human–robot *** with these interrupted input signals significantly improves the regression accuracy in the case of sEMG signal *** Robust Predictive Control-oriented Machine Learning(Robust-MusclenET)model can support volitional high-level myoelectric-based control of biomechatronic devices,such as exoskeletons,prostheses,and assistive/resistive *** work should study the application to prosthesis control as well as the repeatability of the high-level controller with electrode *** low-level hierarchical controller that manages the human–robot interaction,the assistance/resistance strategy,and the actuator coordination should also be studied.

Damage evolution of rock-encased-backfill structure under stepwise cyclic triaxial loading

Rock-encased-backfill(RB)structures are common in underground mining,for example in the cut-andfill and stoping *** understand the effects of cyclic excavation and blasting activities on the damage of these RB structures,a series of triaxial stepwise-increasing-amplitude cyclic loading experiments was conducted with cylindrical RB specimens(rock on outside,backfill on inside)with different volume fractions of rock(VF=0.48,0.61,0.73,and 0.84),confining pressures(0,6,9,and 12 MPa),and cyclic loading rates(200,300,400,and 500 n/s).The damage evolution and meso-crack formation during the cyclic tests were analyzed with results from stress-strain hysteresis loops,acoustic emission events,and post-failure X-ray 3D fracture *** results showed significant differences between cyclic and monotonic loadings of RB specimens,particularly with regard to the generation of shear microcracks,the development of stress memory and strain hardening,and the contact forces and associated friction that develops along the rock-backfill *** important finding is that as a function of the number of cycles,the elastic strain increases linearly and the dissipated energy increases ***,compared with monotonic loading,the cyclic strain hardening characteristics are more sensitive to rising confining pressures during the initial compaction *** finding is that compared with monotonic loading,more shear microcracks are generated during every reloading stage,but these microcracks tend to be dispersed and lessen the likelihood of large shear fracture *** transition from elastic to plastic behavior varies depending on the parameters of each test(confinement,volume fraction,and cyclic rate),and an interesting finding was that the transformation to plastic behavior is significantly lower under the conditions of 0.73 rock volume fraction,400 n/s cyclic loading rate,and 9 MPa *** the findings have important practical implications on t
