Sphincter of Oddi dysfunction:Psychosocial distress correlates with manometric dyskinesia but not stenosis

AIM:To compare postcholecystectomy patients with Sphincter of Oddi(SO)dyskinesia and those with normal SO motility to determine the psychosocial distress,gender and objective clinical correlates of dyskinesia,and contrast these findings with comparisons between SO stenosis and normal SO ***:Within a cohort of seventy-two consecutive postcholecystectomy patients with suspected SO dysfunction,manometric assessment identified subgroups with SO dyskinesia(n=33),SO stenosis(n=18)and normal SO motility(n=21).each patient was categorized in terms of Milwaukee Type,sociodemographic status and the severity of stress-coping ***:Logistic regression revealed that in combination certain psychological,sociodemographic and clinical variables significantly differentiated SO dyskinesia,but not SO stenosis,from normal SO *** of psychosocial stress and of coping with this stress(*** suppressed more frequently and the use of significantly more psychological coping strategies)were highest among patients with SO dyskinesia,especially *** levels of neuroticism(the tendency to stressproneness)further increased the likelihood of SO ***:A motility disturbance related to psychosocial distress may help to explain the finding of SO dyskinesia in some postcholecystectomy patients.

AIM:To compare the prevalence of Functional gastrointestinal disorders(FGIDs)using ROMeⅢand ROMeⅡand to describe predictors of FGIDs among eating disorder(eD)***:Two similar cohorts of female eD inpatients,aged 17-50 years,with no organic gastrointestinal or systemic disorders,completed either the ROMeⅢ(n=100)or the ROMeⅡ(n=160)questionnaire on admission for eD *** two ROMe cohorts were compared on continuous demographic variables(e.g.,age,BMI)using Student’s t-tests,and on categorical variables(e.g.,eD diagnosis)usingχ*** relationship between eD diagnostic subtypes and FGID categories was explored usingχ***,BMI,and psychological and behavioural predictors of the common(prevalence greater than 20%)ROMeⅢFGIDs were tested using logistic regression ***:The criteria for at least one FGID were fulfilled by 83%of the ROMeⅢcohort,and 94%of the ROMeⅡ*** were no significant differences in age,BMI,lowest ever BMI,eD diagnostic subtypes or eD-related quality of life(QOL)scores between ROMeⅡand ROMeⅢ*** most prevalent FGIDs using ROMeⅢwere postprandial distress syndrome(PDS)(45%)and irritable bowel syndrome(IBS)(41%),followed by unspecified functional bowel disorders(U-FBD)(24%),and functional heartburn(FH)(22%).There was a 29%or 46%increase(depending on presence or absence of cyclic vomiting)in functional gastroduodenal disorders because of the introduction of PDS in ROMeⅢcompared to ROMeⅡ.There was a 35%decrease in functional bowel disorders(FBD)in RomeⅢ(excluding U-FBD)compared to ROMeⅡ.The most significant predictor of PDS was starvation(P=0.008).The predictor of FH(P=0.021)and U-FBD(P=0.007)was somatisation,and of IBS laxative use(P=0.025).Age and BMI were not significant *** addition of the 6-mo duration of symptoms requirement for a diagnosis in ROMeⅢadded precision to many ***:ROMeⅢconfers higher precision in diagnosing FGIDs but self-induced vomiting should be excluded from the

Development of Light Weight Structures to Provide Blast-Resistant Designs

In the past, blast-resistant designs for structures were often constructed with massive type structural systems, which relied more on brute strength than on finesse to achieve the required blast resistance. However, structures composed of COLD-FORMeD steel components, such as sheet metal and metal studs, have shown great promise in providing blast resistance with the added benefits of low cost and ease of construction. Some examples of using such structures to provide containment for package handling facilities (PHF) are described in the paper for situations where blast containment is needed, such as a potential package bomb being discovered during the package vetting process. Results from tests and analytic data are used to illustrate aspects of design peculiar to such types of applications. Designs for specific capacities of PHF are described.

Cased explosives generate highly energetic fragments as their casing breaks up. Due to the complexity of casing fragment related behavior such as embedment, perforation and ricochet, it may be insufficient to use equivalent triangular pressure loading in fragment impact simulations. This simplified method may over- or under-predict the target response. Recently, a procedure using contact techniques has been proposed to overcome such difficulties. It has been shown that the new method has the inherent capability in modeling the multi-piece and multi-hit fragment impact problems in a more realistic way. To investigate the applicability of the proposed method to simulations involving multi-layer penetration, the selected problems of fragment impact on multi-layer targets are described in this paper. It is demonstrated that this method is capable of predicting the complicated multi-layer structural response caused by fragment impact and penetration. Modeling procedures and some technical issues are also discussed.

This paper has focused on applying mathematical techniques to address fundamental question in therapy planning when to switch, and how to sequence therapies. We consider switching and sequencing available therapies so as to maximize a patient＇s expected total lifetime. We assume knowledge only about the lifetime distributions induced by the therapies. We discuss a specialization of this model that is tailored to a frequently reoccurring type of management problem, where our goal is to determine the best timing for testing and treatment decisions for patients with ischemic heart disease. Typically, decisions are made with an overall, goal of maximizing the patient＇s expected lifetime or quality-adjusted lifetime.

The last five years have been an exciting time in the study of esophageal motor disorders due to the recent advances in esophageal function testing. New technologies have emerged, such as intraluminal impedance, while conventional techniques, such as manometry, have enjoyed many improvements due to advances in transducer technology, computerization and graphic data presentation. While these techniques provide more detailed information regarding esophageal function, our understanding of whether they can improve our ability to diagnose and treat patients more effectively is evolving. These techniques are also excellent research tools and they have added substantially to our understanding of esophageal motor function in dysphagia. This review describes the potential benefits that these new technologies may have over conventional techniques for the evaluation of dysphagia.

In the present work, an alternative numerical methodology is developed for a fast and effective simulation and analysis of the complex flow and energy conversion in Pelton impulse hydro turbines. The algorithm is based on the Lagrangian approach and the unsteady free-surface flow during the jet-bucket interaction is simulated by tracking the trajectories of representative fluid particles at very low computer cost. Modern regression tools are implemented in a new parameterization technique of the inner bucket surface. Key-feature of the model is the introduction of additional terms into the particle motion equations to account for various hydraulic losses and the flow spreading, which are regulated and evaluated with the aid of experimental data in a Laboratory Pelton turbine. The model is applied to study the jet-runner interaction in various operation conditions and then to perform numerical design optimization of the bucket shape, using a stochastic optimizer based on evolutionary algorithms. The obtained optimum runner attains remarkably higher hydraulic efficiency in the entire load range. Finally, a new small Pelton turbine （150 kW） is designed, manufactured and tested in the Laboratory, and its performance and efficiency verify the model predictions.

Surgery is required in the vast majority of patients with Crohn’s disease (CD) and in approximately one-third of patients with ulcerative colitis (UC). Similar to medical treatments for IBD, significant advances have occurred in surgery. Advances in CD include an emphasis upon conservatism as exemplified by more limited resections, strictureplasties, and laparoscopic resections. The use of probiotics in selected patients has improved the outcome in patients with pouchitis following restorative proctocolectomy for UC. It is anticipated that ongoing discoveries in the molecular basis of IBD will in turn identify those patients who will best respond to surgery.
