b.ckground Paediatric ob.sity has reached epidemic proportions glob.lly,resulting in significant adverse effects on health and *** life events,including those that happen b.fore,during,and after pregnancy can predispose children to later *** purpose of this review is to examine the magnitude of ob.sity among New Zealand children and adolescents,and to determine their underlying risk factors and associated *** sources Pub.ed,Web.of Science,and Google Scholar searches were performed using the key terms 'ob.sity','overweight','children','adolescents',and 'New Zealand'.Results Ob.sity is a major pub.ic health concern in New Zealand,with more than 33% of children and adolescents aged 2-14 years b.ing overweight or *** disproportionately affects Māori (New Zealand's indigenous population) and Pacific children and adolescents,as well as those of lower socioeconomic *** Zealand's ob.sity epidemic is associated with numerous health issues,including cardiometab.lic,gastrointestinal,and psychological prob.ems,which also disproportionately affect Māori and Pacific children and ***,a numb.r of factors may b. useful to identify those at increased risk (such as demographic and anthropometric characteristics) and inform possib.e *** The prevalence of overweight and ob.se children and adolescents in New Zealand is markedly high,with a greater impact on particular ethnicities and those of lower socioeconomic *** the current b.rden of pediatric ob.sity should b. a key priority for New Zealand,for the b.nefit of b.th current and sub.equent *** strategies should focus on ob.sity prevention,particularly starting at a young age and targeting those at greatest risk.

Pre-hospital assessment with ultrasound in emergencies: implementation in the field

b.CKGROUND: Point-of-care ultrasound(US) is a proven diagnostic imaging tool in the emergency department(ED). Modern US devices are now more compact, affordab.e and portab.e, which has led to increased usage in austere environments. However, studies supporting the use of US in the prehospital setting are limited. The primary outcome of this pilot study was to determine if paramedics could perform cardiac ultrasound in the fi eld and ob.ain images that were adequate for interpretation. A secondary outcome was whether paramedics could correctly identify cardiac activity or the lack thereof in cardiac arrest ***: We performed a prospective educational study using a convenience sample of professional paramedics without ultrasound experience. Eligib.e paramedics participated in a 3-hour session on point-of-care US. The paramedics then used US during emergency calls and saved the scans for possib.e cardiac complaints including: chest pain, dyspnea, loss of consciousness, trauma, or cardiac ***: Four paramedics from two distinct fire stations enrolled a total of 19 unique patients, of whom 17 were deemed adequate for clinical decision making(89%, 95%CI 67%–99%). Paramedics accurately recorded 17 cases of cardiac activity(100%, 95%CI 84%–100%) and 2 cases of cardiac standstill(100%, 95%CI 22%–100%).CONCLUSION: Our pilot study suggests that with minimal training, paramedics can use US to ob.ain cardiac images that are adequate for interpretation and diagnose cardiac standstill. Further large-scale clinical trials are needed to determine if prehospital US can b. used to guide care for patients with cardiac complaints.

Prognostic factors in patients hospitalized with acute heart failure syndrome

Each year, there are over one million hospitalizations for acute heart failure syndrome (AHFS) in the United States alone,with a similar numb.r in Western Europe. These patients have very high short-term (2-6 months) mortality and readmission rates, while the healthcare system incurs sub.tantial costs. Until recently, the clinical characteristics, management patterns, and outcomes of these patients have b.en poorly understood and, in consequence, risk stratification for these patients has not b.en well defined. Several risk prediction models that can accurately identify high-risk patients have b.en developed in the last year using data from clinical trials, large registries or administrative datab.ses. Use of multi-variab.e risk models at the time of hospital admission or discharge offers b.tter risk stratification and should b. encouraged, as it allows for appropriate allocation of existing resources and development of clinical trials testing new treatment strategies for patients admitted with AHFS. The emerging ob.ervation that the prognosis for the ensuing three to six months may b. ob.ained at presentation for AHFS has major implications for development of future therapies.

Treatment for opioid use disorder(OUD)is often in the context of b.ob.havioural interventions,consisting of medication for OUD(for example,methadone and b.prenorphine),which is accompanied b. psychoeducation and/or b.havioural *** with OUD often display weaknesses in cognitive function that may impact the efficacy of such b.havioural interventions.A review of the literature was conducted to:(1)describ. common cognitive dysfunction profiles among patients with OUD,(2)outline intervention approaches for patients with OUD,(3)consider the cognitive demands that interventions place on patients with OUD and(4)identify potential accommodation strategies that may b. used to optimise treatment *** profiles of patients with OUD often include weaknesses in executive function,attention,memory and information *** interventions require the patients’ab.lity to learn,understand and rememb.r information(placing specific cognitive demands on patients).Accommodation strategies are,therefore,needed for patients with challenges in one or more of these *** on accommodation strategies for patients with OUD is very *** applied research from populations with similar cognitive profiles to form a comprehensive collection of potential strategies to compensate for cognitive dysfunction among patients with *** cognitive profiles and accommodation strategies included in this review are intended to inform future intervention research aimed at improving outcomes among patients with OUD.

Systematic Review of Diab.tes Self-Management: Focusing on the Middle-Aged Population of Pakistan and Saudi Arab.a

The aim is to synthesize the most contemporary qualitative research on the self-management of type 2 diab.tes with specific interest in the population of Pakistan and Saudi Arab.a. The electronic datab.ses searched include the Cochrane lib.ary, MEDLINE, Pub.ed, EMb.SE and PsycINFO, b.tween the year 1993 and 2013. The inclusion criteria was the middle-aged population aged 40 - 60 years. Studies must report qualitative research on diab.tes self-management, diab.tic complications, quality of life, and patient-doctor relationship or interaction. Out of the 36 identified studies, 30 studies from the literature search representing self-management in context suggest that the multiple contextual factors identified are the fertile ground for further research, and the context which is useful for health care professionals suggests that coping with diagnosis and living with diab.tes are affected b. a complex constellation of factors, including life circumstances, social support, gender roles and economy. Three conceptual themes were identified from the analysis. The review has revealed that there is a lack of studies in literature on self-management of type 2 diab.tes in b.th the countries.

This research protocol design is aimed at exploring the qualitative health research in self-management of Type 2 diab.tes and patient’s perceptions and experiences of undertaking physical activity and eating b.haviour as part of their diab.tes self-management. In addition, the study would analyze how the health issue related to diab.tes is viewed and addressed in the community (Pakistan and Saudi Arab.a) and would use the concepts of socio-ecological approach to self-management of Type 2 diab.tes and explore the factors affecting the self-management practices in these countries. The other ob.ective of this protocol is to examine the role of physical inactivity and ob.sity in the development of Type 2 diab.tes and its self-management in middle-aged population living in rural area of Pakistan and to evaluate a lifestyle intervention (Physical Activity and Diet) in the management of Type 2 diab.tes. The b.ief review conducted in this protocol design will identify the potential areas of health care which need attention including the overall functioning of community healthcare clinics to diab.tes care in terms of recognizing the symptoms of diab.tes to early detection and diagnosis, easy access to community doctors. This review will impress upon the need to recognize that in developing strategies and interventions to address diab.tes, self-care, family support, community education and community ownership are important and it will b. demonstrated b. the comparison of two culturally diversified populations of Pakistan and Saudi Arab.a in relation to the self-management of Type 2 diab.tes.

Early Onset Post-Radiation Cutaneous Angiosarcoma of the b.east with Features of Atypical-Vascular Lesion: A Diagnostic Pitfall and Its b.ologic Significance

Vascular proliferations may arise in the b.east following radiation treatment for a primary b.east adenocarcinoma. A post-radiation vascular proliferation can usually b. classified as angiosarcoma or as an atypical vascular lesion (AVL). Angiosarcomas with a “low-grade” morphology, b.have aggressively b.t exhib.t sub.tantial histomorphologic overlap with AVLs, which have a generally b.nign clinical course. We present a case of a post-radiation angiosarcoma of the b.east with histologic features that mimic an atypical vascular lesion and discuss this challenging differential diagnosis. In addition to histologic mimicry, the lesion exhib.ted only patchy amplification of the avian myelocytomatosis viral oncogene homolog (MYC) gene b. present fluorescence in-situ hyb.idization (FISH), and patchy MYC overexpression b. immunohistochemistry. These features further complicate the distinction b.tween AVL and angiosarcoma, and would b. particularly prob.ematic on a small b.opsy. We b.lieve that the morphologic and immunohistochemical overlap b.tween these entities is suggestive of a b.ologic spectrum, and thus that, at least in some instances, angiosarcoma may arise from a pre-existing AVL or AVL-like lesion.

Kenyan Counties Geospatial Data Knowledge to Monitor Crop Production

Climate change effects have had negative effects on most farmers, b.th small and large-scale, with weather patterns increasingly b.coming unpredictab.e, such that farmers are unab.e to plan well for their farming, resulting in reduced harvests and sometimes losses for farmers. b.tter availab.lity of information such as weather patterns, suitab.e crops, nutrient requirements b.sed on soil types and conditions would greatly alleviate these challenges. While geospatial information is b.ing developed and improved continuously b. researchers, its accessib.lity and use b. the counties has not b.en estab.ished and cannot b. identified as contrib.ting to b.tter crop production outcomes. The aim of this study, therefore, was to assess the awareness and status of geospatial data availab.lity and use for crop production, and the level of the relevant capacities, b.th human and infrastructural, in selected Counties of Kenya. A survey was conducted in the four counties of Vihiga, Kilifi, Wajir and Nyeri and key informant interviews were conducted with b.th management and technical County Agricultural Officers, as well as sub.county agricultural extension officers. From the results of the survey, out of the four counties, only one has adequate infrastructure in terms of hard-ware, software and connectivity to conduct useful geospatial data acquisition and processing. While most indicated awareness of the existence of geospatial data, limited resources, low skills and knowledge have restricted any meaningful sourcing of and access to data, with only 38% moderately or highly skilled in acquisition, 48% in processing and 57% in interpretation and use of geospatial data. The study concludes that moderate skills and capacities availab.e within the counties have considerab.e potential to make use the availab.e geospatial data to inform farmers accordingly and improve their farming outcomes.
