Vasectomy by epithelial curettage w.thout suture or cautery： a pilot study in humans

Curettage of the epithelium of the vas deferens might be a safe and effective method of male sterilization. w. conducted a pilot study of vasectomy by epithelial curettage w.th a novel microcurette called the Vas-X in 12 normal men requesting elective sterilization. Seminal fluid analysis w.s obtained monthly after the procedure for 6 months. Pain w.s assessed by questionnaire. Three months after the procedure, all men attained sperm concentrations of less than 0.2 million sperm per mL, and seven w.re azoospermic. Post-procedural pain w.s minimal. Nine men ultimately achieved and maintained azoospermia; how.ver, 4 to 6 months after the procedure, sperm concentrations increased in three of the 12 subjects, necessitating repeat vasectomy. Microscopic examination of the vas deferens from these failures revealed re-canalization. Vasectomy by epithelial curettage can result in effective sterilization; how.ver, 1/4 of the subjects w.re not effectively sterilized by the procedure due to re-canalization of the vas deferens. Epithelial curettage w.ll require further refinement to determine if it is a viable form of vasectomy.

Impact of Mitral Valve Repair in Patients w.th End-Stage Congestive Heart Failure

Objective: The utility of mitral valve repair in patients w.th Mitral Regurgitation (MR) and advanced CHF remains controversial. Methods: 37 patients w.th MR and Left Ventricular Ejection Fraction (LVEF) 25% as w.ll as patients w.th LVIDd Results: Operative mortality w.s 0% for the group. There w.re significant reductions in NYHA Class (p = 0.0004), mitral regurgitation (p 25%. There w.re no significant differences in cardiac outcome changes betw.en patients w.th LVIDd 25%, and betw.en patients w.th LVIDd Conclusions: MV repair in patients w.th low.LVEF and MR can be performed safely, w.th significant improvement in LVEF and symptom profile. No survival difference w.re noted betw.en those patients w.th severely depressed LVEF or those w.th elevated ventricular dimensions (LVIDd) w.en compared to those w.th less severe but still significant cardiac impairment. Consideration should be given to these patients as an option prior to transplantation.
