supportive care in transplantation: A patient-centered care model to better support kidney transplant candidates and recipients

Kidney transplantation(KT),although the best treatment option for eligible patients,entails maintaining and adhering to a life-long treatment regimen of medications,lifestyle changes,self-care,and *** patients experience uncertain outcome trajectories increasing their vulnerability and symptom burden and generating complex care *** when transplants are successful,for some patients the adjustment to life post-transplant can be challenging and psychological difficulties,economic challenges and social isola-tion have been *** 50%of patients lose their transplant within 10 years and must return to dialysis or pursue another transplant or conservative *** paper documents the complicated journey patients undertake before and after KT and outlines some initiatives aimed at improving patient-centered care in transplantation.A more cohesive approach to care that borrows its philosophical approach from the established field of supportive oncology may improve patient experiences and *** propose the"supportive care in transplantation"care model to operationalize a patient-centered approach in *** model can build on other ongoing initiatives of other s.holars and researchers and can help advance patient-centered care through the entire care continuum of kidney transplant recipients and ***-dimensionality,multi-disciplinarity and evidence-based approaches are proposed as other key tenets of this care *** conclude by proposing the potential advantages of this approach to patients and healthcare *** Tip:Kidney transplant recipients and candidates face several uncertainties in their care journey and have several expressed unmet healthcare *** recommend a structured and comprehensive approach to transplant care across the entire continuum of a transplant patient’s journey similar to what has been developed in the field of *** supportive care in transplantation model can operational

New markers of fibrosis in hepatitis C:A step towards the holy Grail?

In the present issue of the World Journal of Hepatology,Ferrassi et al examine the problem of liver fibrosis staging in chronic hepatitis *** identify novel biomarkers in an effort to predict accurate fibrosis staging with the aid of the metabolome of Hepatitis C *** I think Ferrassi et al took a different approach in identifying fibrosis biomarkers,by looking at the patients’*** biomarkers clearly separate patients from *** can also separate out,patients with minimal fibrosis(F0-F1 stage)and patients with cirrhosis(F4 stage).Obviously,if these biomarkers were to be widely used,tests for all the important metabolites would need to be readily available for use in hospitals or outpatient setting and that may prove difficult and above all,***,this step could eventually lead to a metabolomic approach for novel biomarkers of ***,it would need to be validated,but could represent a step towards the holy Grail of Hepatology.

Objective:To scrutinize the definitions of minimal invasive surgical therapy(MIsT)and to investigate urologists’knowledge,attitudes,and practices for benign prostatic obstruction ***:A 36-item survey was developed with a Delphi *** on definitions of MIsT and attitudes and practices of benign prostatic obstruction surgeries were *** were invited globally to complete the online *** was achieved when more than or equal to 70%responses were“agree or strongly agree”and less than or equal to 15%responses were“disagree or strongly disagree”(consensus agree),or when more than or equal to 70%responses were“disagree or strongly disagree”and less than or equal to 15%responses were“agree or strongly agree”(consensus disagree).Results:The top three qualities for defining MIsT were minimal blood loss(n=466,80.3%),fast post-operative recovery(n=431,74.3%),and short hospital stay(n=425,73.3%).The top three surgeries that were regarded as MIsT were Urolift®(n=361,62.2%),Rezum®(n=351,60.5%),and endoscopic enucleation of the prostate(EEP)(n=332,57.2%).Consensus in the knowledge section was achieved for the superiority of Urolift®,Rezum®,and iTIND®over transurethral resection of the prostate with regard to blood loss,recovery,day surgery feasibility,and post-operative *** in the attitudes section was achieved for the superiority of Urolift®,Rezum®,and iTIND®over transurethral resection of the prostate with regard to blood loss,recovery,and day surgery *** on both sections was achieved for EEP as the option with the better symptoms and flow improvement,lower retreatment rate,and better suitable for prostate more than 80 ***:Minimal blood loss,fast post-operative recovery,and short hospital stay were the most important qualities for defining ***®,Rezum®,and EEP were regarded as MIsT by most urologists.

KLnF_(4):Yb^(3+),Er^(3+)/ho^(3+)(Ln=Y,Yb,Gd)phos.hors were prepared by solid-state reaction *** each of the Yb^(3+)/Er^(3+)and Yb^(3+)/ho^(3+)co-doped phos.hors independently,phase confirmation and morphological examination were accomplished using the X-ray diffraction technique and scanning electron microscope analysis.X-ray photoelectron spectroscopy analysis shows the charge state and existence of lanthanides and *** luminescence emissions of green and red colour were detected for the synthesized *** changes in the upconverted emission intensities and colour output pertaining to the change of the lanthanide host matrix along with dopants were investigated for the prepared *** the upconversion luminescence,erbium-doped samples exhibit a predominance of red emission while holmium-doped samples display a prevalence of green *** rare-earth Er/ho doped KYbF4shows strong red emission compared to other ***_(4):Yb,Er/ho exhibits relatively significant green upconve rsion emissions,while Yb,Er-doped KGdF_(4)host displays a 10-fold increase in green/red emission ratio with an increase in the input power of 20-780 *** photoluminescence studies and decay curve analysis were carried out using UV light excitation for the developed phos.hors.A higher lifetime of 2.2 ms was measured for the downshifting red emission in the ho doped KYbF_(4)*** study suggests a potential for colour tunability in the upconversion and downshifting emissions through host modifications to the fluoride phos.hor.

All solid-state batteries(AssBs)are the holy grails of rechargeable batteries,where extensive searches are ongoing in the pursuit of ideal solid-state ***,there is still a long way off to the satisfactorily high(enough)ionic conductivity,long-term stability and especially being able to form compatible interfaces with the solid ***,we have explored ionic transport behavior and high mobility in the sub-nano pore networks in the framework ***,the frameworked electrolyte behaves as a solid,and however in the(sub)-nano scales,the very limited number of solvent molecules in confinement makes them completely different from that in liquid *** from a liquid-electrolyte counterpart,the interactions between the mobile ions and surrounding molecules are subject to dramatic changes,leading to a high ionic conductivity at room temperature with a low activation ***+ions in the sub-nano cages of the network structure are highly mobile and diffuse rather independently,where the rate-limiting step of ions crossing cages is driven by the local concentration gradient and the electrostatic interactions between Li^(+)*** new class of frameworked electrolytes(FEs)with both high ionic conductivity and desirable interface with solid electrodes are demonstrated to work with Li-ion batteries,where the AssB with LiFePO_(4)shows a highly stable electrochemical performance of over 450 cycles at 2℃ at room temperature,with an almost negligible capacity fade of 0.03‰ each *** addition,the FE shows outstanding flexibility and anti-flammability,which are among the key requirements of large-scale applications.

The timing and mechanisms of the human occupation of the demanding high-altitude Tibetan Plateau environment are of great ***,we report on our reinvestigations and dating of the Nwya Devu site,located nearly 4600 meters above sea level on the central Tibetan Plateau.A new microblade techno-complex was identified on a lower lake shore at this site,distinct from the previously reported blade tool *** two lithic assemblages were dated to 45.6±2.6 and10.3±0.5 ka using optically stimulated luminescence and accelerator mass spectrometry^(14)C *** represent,respectively,the earliest known Paleolithic and microlithic sites on the interior Tibetan Plateau,indicating multiple occupation episodes of hunter-gatherers during the past 45 *** studies reveal that relatively stable depositional conditions and a paleoenvironment characterized by a comparatively warm climate facilitated these multiple occupations at Nwya *** contemporaneous occurrence of the Upper Paleolithic blade technology on the Tibetan Plateau and most of Eurasia between 50 and 40 ka indicates rapid,large-scale dispersals of humans that profoundly affected human demography on a large *** new archaeological evidence and previously reported genetic data,we conclude that the Tibetan Plateau provided a relatively stable habitat for Upper Paleolithic hunter-gatherers,which may have contributed to the complex and multiple-origin gene pool of present-day Tibetans.
