BACKGROUND infection of a transjugular intrahepatic portosystemic shunt(TiPS)stent is a rare and serious complication that most commonly occurs during TiPS creation and *** typically present with recurrent bacteremia due to shunt occlusion or *** date there are approximately 58 cases *** present a patient diagnosed with late polymicrobial TiPS infection five years following TiPS *** SUMMARY A 63-year-old female status-post liver transplant with recurrent cirrhosis and portal hypertension presented with sepsis and recurrent extended-spectrum betalactamase Escherichia coli *** tomography of the abdomen revealed an occluded TiPS with thrombus extension into the distal right portal vein,and focal thickening of the cecum and ascending *** revealed patchy ulcers in these areas with histopathology demonstrating ulcerated colonic mucosa with fibrinopurulent *** thrombectomy and revision revealed infected-appearing *** initially cleared her infection with antibacterial therapy and TiPS revision;however,soon after,she developed Enterobacter cloacae bacteremia and Candida glabrata and *** fungemia with recurrent TiPS *** remained on antifungal therapy indefinitely and later developed vancomycin-resistant Enterococcus faecium with recurrent TiPS *** option of liver re-transplant for removal of the infected TiPS was not offered given her critical illness and complex shunt *** patient became intolerant to linezolid and elected hospice *** Clinicians should be aware that TiPS superinfection may occur as long as five years following TiPS creation in an immunocompromised patient.

An analysis of the size and direction of the association between mental toughness and Olympic distance personal best triathlon times

Background: Athletes, coaches, sport psychology practitioners, and researchers suggest that mental toughness represents an important construct that is associated with athletic performance. Unfortunately, the absence of real-world performance as a dependent variable has limited our ability to substantiate this claim. The concern of a lack of ecologically valid measures of sports performance might be addressed by examining the relationship between mental toughness and sports performance using a standardized measure of personal best(PB) triathlon time and a validated unidimensional measure of mental ***: Three hundred and sixteen triathletes completed the 8-item mental toughness index(MTi), reported their age, and provided a PB Olympic distance triathlon time and the total number of triathlons they had completed to date. Given that males are typically quicker than females, a standardized time was calculated by dividing the PB by the current Olympic record for gender; we also hypothesized that more experienced triathletes would report quicker PB times because of greater frequency and duration of training. Once we had controlled for gender and experience,we predicted that mental toughness would be negatively associated with triathlon time and the size of the relationship would be of a moderate ***: Results revealed small-to moderate-sized negative relationships between both completed number of triathlons and mental toughness with standardized PB ***: The hierarchical regression analysis showed that mental toughness provided a unique contribution to the variability in standardized Olympic triathlon PB after controlling for the total number of triathlons completed.

Experimental Trypanosoma brucei infection at immediate post partum period:effects on dam and the offspring

interface-induced warping in hybrid two-dimensional materials

Two-dimensional hybrid materials consisting of heterogeneous domains have been of great interest. Using empirical molecular dynamical simulation, we show that the morphology of such hybrid 2D materials can extend into the third dimension via strong warping intrinsic to the interfaces between the domains. The interface warping stems from the compressive stress in the domain with a larger lattice constant and even penetrates into the stretched domain. Based on classic plate theory, we analytically quantify the amplitude, wave length and penetration depth of the interface warping as functions of the lattice mismatch, achieving good agreement with the simulations. Moreover, we propose that periodically placing pentagon-heptagon dislocations along the interface can eliminate the warping in the 2D material and such defective interface can be more favorable than the warped one over a critical domain size, which is consistent with recent experimental observations. Our results suggest that the interface warping in 2D hybrid materials should be considered in further exploring their promising properties.

The influence of overlap multi-pass friction stir processing on the microstructure and the mechanical properties, in particular, strength, ductility and hardness of die cast A1-7Si-3Cu aluminum alloy was investigated. it was observed that increase in the number of overlap passes friction stir processing resulted in significant refinement and redistribution of aluminum silicon eutectic phase and elimination of casting porosities. The microstructural refinement by the friction stir processing not only increases the ultimate tensile strength from 121 to 273 MPa, but also increases the ductility as observed by the increase in fracture strain from 1.8% to 10%. Analysis of the fractured surface reveals that the microstructural refinement obtained by friction stir processing plays a vital role in transforming the fracture mode from completely mixed mode to the ductile mode of the fracture with increasing number of passes. The change in the size, shape, morphology and distribution of eutectic silicon particles and elimination of the porosities are the main reasons for the increases in tensile strength and ductility due to friction stir processing.

Dear Editor, The patient is a 45-year-old male who presented to an outside clinic in 2014 for evaluation of painful ejaculation, infertility, and difficulty with urination for as long as he could remember. He reported no past significant medical or surgical history and no family history of infertility. He was given a trial of tamsulosin without improvement in his symptoms. initial semen analysis demonstrated azoospermia.

Burnout and mental distress among community health workers in low- and middle-income countries: a scoping review of studies during the COViD-19 pandemic

Community health workers(CHWs)represent the backbone of primary health systems,especially in many low-and middle-income countries(LMiCs).The coronavirus disease 2019(COViD-19)pandemic stretched health sys-tems and increased the workload for *** objective of this scoping review was to identify the mental health symptoms experienced among CHWs in LMiCs during the COViD-19 *** searched PubMed for published literature,from January 1,2020 to December 31,2022 that focused on documenting the experiences of burnout,distress,and mental health symptoms among CHWs in *** quality of included studies was assessed using the Joanna Briggs institute Critical Appraisal *** studies were grouped into the follow-ing broad thematic categories:(1)symptoms experienced;(2)drivers of different mental health symptoms;and(3)strategies for coping with different *** identified 10 cross-sectional,qualitative,and observational studies from 11 LMiCs in South and Southeast Asia,South America,and Eastern/Southern Africa that assessed the mental health burden CHWs faced during the *** studies identified disorders and symptoms such as depression,anxiety,fear,burnout,worsened stress,and *** factors included increased work-load,financial constraints,and an understaffed and underequipped *** reported using different adaptive responses like humor,support from family and colleagues,denial,and substance use,and asked for recommended regular mental health checkups and *** research and policies should be targeted to-wards promoting the mental wellbeing of CHWs to help ensure responsive and resilient health systems in LMiCs in the face of future emerging public health threats.

Objective:To evaluate the anti-inflammatory potential of aqueous extract of Pterocarpus santalinus *** using molecular docking and in vivo ***:An aqueous extract of Pterocarpus santalinus heartwood was prepared using a Soxhlet *** in the extract were tentatively identified using high-resolution mass *** docking experiments were carried out to evaluate the binding affinity of selected compounds,phloridzin to cyclooxygenase-1(COX-1),cyclooxygenase-2(COX-2),prostaglandin E synthase-1(PGES-1)and 5-lipoxygenase(5-LOX).Anti-inflammatory potential was evaluated by carageenan induced paw edema model in ***:The presence of major component phloridzin along with quercetin,parthenin,ginkgolide B,picrotoxinin,usnic acid,octopine,and epigallocatechin was detected in the *** docking study showed that phloridzin inhibited COX-1,COX-2,PGES-1 and 5-LOX with more affinity than ibuprofen and *** santalinus heartwood extract at 200 and 400 mg/kg BW showed significant reduction in carageenan-induced hind paw edema in a dose-dependent manner,but the effect was slow when compared with the standard ibuprofen(30 mg/kg p.o.).Conclusions:The study indicated that after clinical trials,the aqueous extract of Pterocarpus santalinus heartwood can be effectively used in phytotherapy to treat inflammation.
