Epidemiological, diagnostic, Therapeutic and Progressive Aspects of Arterial Hypertension in Children at the University Hospital Center of Bouaké (Côte d’Ivoire)

Introduction: Arterial hypertension (AH) in children is under-diagnosed and often has a poor prognosis. The aim of this study was to describe the epidemiological, diagnostic, therapeutic and evolutionary aspects of hypertension in children at the University Hospital of Bouaké, with a view to improving the prognosis. Methods: This was a cross-sectional, analytical study carried out in the paediatrics department of Bouaké University Hospital. It concerned the medical records of children aged 4 to 15 hospitalised from 1 January 2017 to 31 december 2020 for hypertension. diagnosis was based on the simplified blood pressure guidelines of the Expert Consensus of the French Society of Hypertension. The variables studied were epidemiological, diagnostic, therapeutic and evolutionary. Quantitative variables were compared at the significance level p ≤ 0.05. Results: The hospital incidence of hypertension was 0.32% (69/21,642). The sex ratio was 0.72. 97.1% of the children were over five years of age. Oedema (49.3%) and breathing difficulties (20.2%) were the main reasons for consultation. Hypertension was classified as threatening (56.5%), confirmed (31.9%) and borderline (11.6%). The cause was renal in 66.7%, dominated by impure nephrotic syndrome (24.6%). Treatment for hypertension consisted mainly of a diuretic (79.7%) and a calcium channel blocker (47.8%). Outcome was favourable in 50.7% of cases. Mortality was 20.3%. No factor was significantly associated with death. Conclusion: Hypertension in children at Bouaké University Hospital is serious. The aetiology is mainly renal. Early diagnosis and management are key to improving prognosis.

MOdELLING THE HYdROdYNAMICS OF THE BOHAI SEA IN CHINA

The effects of tides, wind and river discharges on the flow patterns in Bohai Sea are investigated. The Finite Element Method is used to cast the vertically integrated, long wave equations into a system of nonlinear ordinary equations in time. For simplicity we adopted linear triangular elements which are easily integrated exactly and lead to matrices of relatively small bandwidth. The system of ordinary differential equations is solved using a simple finite difference *** of the small contribution from three partial tides S2, K1 and O1, we take only the M2 tide into account. In this study the computations show Liaodong Bay in the north to be the locale of both the maximum flood and ebb strengths; tidal currents in Laizhou Bay in the south are of less importance in the tidal circulation of Bohai Sea. Extraterrestrial Impacts and Geomagnetic Reversals

The existing data on the confirmed extraterrestrial impacts and geomagnetic reversals are examined. differential rotation of the inner core of the Earth relative to the rest of the planet has been observed through seismic records. differential rotation between the inner and outer core is thought to be related to the geomagnetic field of the Earth. Extraterrestrial impacts with the surface of the Earth have been shown to cause changes in this differential rotation. It is shown in this paper that there is a strong correlation between the dates of major impacts and geomagnetic reversals. This strongly supports the possibility of major extraterrestrial impacts being one of the causes of geomagnetic reversals as has been suggested previously.

The effect of the equatorially symmetric zonal winds of Saturn on its gravitational field

The penetration depth of Saturn’s cloud-level winds into its interior is unknown.A possible way of estimating the depth is through measurement of the effect of the winds on the planet’s gravitational *** use a self-consistent perturbation approach to study how the equatorially symmetric zonal winds of Saturn contribute to its gravitational *** important advantage of this approach is that the variation of its gravitational field solely caused by the winds can be isolated and identified because the leading-order problem accounts exactly for rotational distortion,thereby determining the irregular shape and internal structure of the hydrostatic *** assume that（i）the zonal winds are maintained by thermal convection in the form of non-axisymmetric columnar rolls and（ii）the internal structure of the winds,because of the Taylor-Proundman theorem,can be uniquely determined by the observed cloud-level *** calculate both the variation △J_n,n=2,4,6...of the axisymmetric gravitational coefficients J_n caused by the zonal winds and the non-axisymmetric gravitational coefficients △J_（nm） produced by the columnar rolls,where m is the azimuthal wavenumber of the *** consider three different cases characterized by the penetration depth 0.36 R_S,0.2 R_S and 0.1 R_S,where R_S is the equatorial radius of Saturn at the 1-bar pressure *** find that the high-degree gravitational coefficient （ J_（12）＋△J_（12）） is dominated,in all the three cases,by the effect of the zonal flow with ｜△J_（12）/J_（12）｜〉100%and that the size of the non-axisymmetric coefficientsdirectly reflects the depth and scale of the flow taking place in the Saturnian interior.

Sustained virologic response following HCV eradication in two brothers with X-linked agammaglobulinaemia

X-linked agammaglobulinaemia (XLA) is a humoral immunodeficiency syndrome characterized from childhood by the absence of circulating B lymphocytes, absent or reduced levels of serum immunoglobulin and recurrent bacterial infections. For many affected patients, regular treatment with immunoglobulin is life saving. Hepatitis C viral (HCV) infection acquired through contaminated blood products is widely described in this patient cohort. The natural history of HCV infection in patients with XLA tends to follow a more rapid and aggressive course compared to immunocompetent individuals. Furthermore, standard anti-viral therapy appears to be less efficacious in this patient cohort. Here we report the cases of two brothers with XLA who contracted HCV through contaminated blood products. They were treated with a six month course of Interferon alpha-2b and Ribavirin. We report a sustained virologic response five years after completing treatment.

Saturn＇s gravitational field induced by its equatorially antisymmetric zonal winds

The cloud-level zonal winds of Saturn are marked by a substantial equatorially antisymmetric component with a speed of about 50 m s^-1 which, if they are sufficiently deep, can produce measurable odd zonal gravitational coefficients △J2 k＋1, k = 1, 2, 3, 4. This study, based on solutions of the thermal-gravitational wind equation, provides a theoretical basis for interpreting the odd gravitational coefficients of Saturn in terms of its equatorially antisymmetric zonal flow. We adopt a Saturnian model comprising an ice-rock core, a metallic dynamo region and an outer molecular envelope. We use an equatorially antisymmetric zonal flow that is parameterized, confined in the molecular envelope and satisfies the solvability condition required for the thermal-gravitational wind equation. The structure and amplitude of the zonal flow at the cloud level are chosen to be consistent with observations of *** calculate the odd zonal gravitational coefficients △J2k＋1, k = 1, 2, 3, 4 by regarding the depth of the equatorially antisymmetric winds as a parameter. It is found that △J3 is-4.197 × 10^-8 if the zonal winds extend about 13 000 km downward from the cloud tops while it is-0.765 × 10^-8 if the depth is about 4000 km. The depth/profile of the equatorially antisymmetric zonal winds can eventually be estimated when the high-precision measurements of the Cassini Grand Finale become available.

Investigation of active vibration drilling using acoustic emission and cutting size analysis

This paper describes an investigation of active bit vibration on the penetration mechanisms and bit-rock interaction for drilling with a diamond impregnated coring bit. A series of drill-off tests(dOTs) were conducted where the drilling rate-of-penetration(ROP) was measured at a series of step-wise increasing static bit thrusts or weight-on-bits(WOBs). Two active dOTs were conducted by applying 60 Hz axial vibration at the bit-rock interface using an electromagnetic vibrating table mounted underneath the drilling samples, and a passive dOT was conducted where the bit was allowed to vibrate naturally with lower amplitude due to the compliance of the drilling sample mountings. during drilling, an acoustic emission(AE) system was used to record the AE signals generated by the diamond cutter penetration and the cuttings were collected for grain size analysis. The instrumented drilling system recorded the dynamic motions of the bit-rock interface using a laser displacement sensor, a load cell, and an LVdT(linear variable differential transformer) recorded the dynamic WOB and the ROP, respectively. Calibration with the drilling system showed that rotary speed was approximately the same at any given WOB, facilitating comparison of the results at the same WOB. Analysis of the experimental results shows that the ROP of the bit at any given WOB increased with higher amplitude of axial bit-rock vibration, and the drill cuttings increased in size with a higher ROP. Spectral analysis of the AEs indicated that the higher ROP and larger cutting size were correlated with a higher AE energy and a lower AE frequency. This indicated that larger fractures were being created to generate larger cutting size. Overall, these results indicate that a greater magnitude of axial bit-rock vibration produces larger fractures and generates larger cuttings which, at the same rotary speed, results in a higher ROP.
