Gastroduodenal Crohn’s disease (CD) is rare and the response to standard medical therapy is often poor. Anti-tumor necrosis factor therapy has revolutionised the treatment of CD. We present a patient with pyloric stenosis associated with CD which improved with Adalimumab therapy. We recommend considering antitumor necrosis factor therapy in symptomatic gastroduodenal CD.

We document the first record of Borrelia americana in Canada. This Borrelia was detected in an avian coast tick, Ixodes auritulus (Acari: Ixodidae), collected from a Varied Thrush, Ixoreus naevius, along coastal British Columbia. Using real-time PCR and DNA sequencing of the flagellin gene, we determined that the borrelial amplicon from the I. auritulus nymph was 99% homologous with B. americana type strain SCW-41. Because patients infected with B. americana can be seronegative for Lyme disease, medical professionals should be willing to pursue molecular analyses and consider treatment for patients with Lyme disease-like symptoms.

Infection Prevalence of <i>Borrelia burgdorferi</i>in Ticks Collected from Songbirds in Far-Western Canada

Worldwide, wild birds play a vital role in the dispersal of ticks that harbour tick-borne pathogens, including Borrelia burgdorferi, the Lyme disease bacterium. Using PCR testing, we found 124 (31%) of 405 ticks (4 species), which were collected from 21 species of birds in far-western Canada, to be infected with B. burgdorferi. Transstadial transmission of B. burgdorferi occurred from larva to nymph, plus nymph to adult, in the avian coastal tick, Ixodes auritulus, collected from songbirds in British Columbia (B.C). Collectively, all 3 motile life stages (larva, nymph, adult) of this tick had an infection prevalence of 31% for B. burgdorferi, which suggests vector competency. A Pacific Wren was highly infested with I. auritulus immatures, and 20 (44%) of 45 ticks (2 nymphs, 43 larvae) were infected with B. burgdorferi. This heavy infestation shows the high potential to initiate a new population of ticks and to disseminate Lyme spirochetes. Epidemiologically, B. burgdorferi-infected I. auritulus larvae collected from the Spotted Towhee, Swainson’s Thrush, Pacific Wren, and Fox Sparrow suggest that these avian hosts act as reservoirs for B. burgdorferi. In this study, the western blacklegged tick, Ixodes pacificus, and Ixodes spinipalpis played a limited role in the enzootic transmission cycle of B. burgdorferi along coastal B.C. We document the first record of I. spinipalpis on a bird in Alberta. Because songbirds widely disperse Lyme disease vector ticks, primary health providers and the general public must be vigilant that outdoors people may be bitten by B. burgdorferi-infected ticks throughout far-western Canada.

AIM: To study the significance of cap-fitted colonoscopy in improving cecal intubation time and polyp detection rate. METHODS: This study was a prospective randomized controlled trial conducted from March 2008 to February 2009 in a tertiary referral hospital at Sydney. The primary end point was cecal intubation time and the secondary endpoint was polyp detection rate. Consecutive cases of total colonoscopy over a 1-year period were recruited. Randomization into either standard colonoscopy (SC) or cap-assisted colonoscopy (CAC) was performed after consent was obtained. For cases randomized to CAC, one of the three sizes of cap was used: D-201-15004 (with a diameter of 15.3 mm), D-201-14304 (14.6 mm) and D-201-12704 (13.0 mm). All of these caps were produced by Olympus Medical Systems, Japan. Independent predictors for faster cecal time and better polyp detection rate were also determined from this study. RESULTS: There were 200 cases in each group. There was no signif icant difference in terms of demographic characteristics between the two groups. CAC, when compared to the SC group, had no signif icant difference in terms of cecal intubation rate (96.0% vs 97.0%, P = 0.40) and time (9.94 ± 7.05 min vs 10.34 ± 6.82 min, P = 0.21), or polyp detection rate (32.8% vs 31.3%, P = 0.75). On the subgroup analysis, there was no significant difference in terms of cecal intubation time by trainees (88.1% vs 84.8%, P = 0.40), ileal intubation rate (82.5% vs 79.0%, P = 0.38) or total colonoscopy time (23.24 ± 13.95 min vs 22.56 ± 9.94 min, P = 0.88). On multivariate analysis, the independent determinants of faster cecal time were consultant-performed procedures (P < 0.001), male patients (P < 0.001), non-usage of hyoscine (P < 0.001) and better bowel preparation (P = 0.01). The determinants of better polyp detection rate were older age (P < 0.001), no history of previous abdominal surgery (P = 0.04), patients not having esophagogastroduodenoscopy in the same setting (P = 0.003),

The percentage of overweight and obese patients(OPs) waiting for a liver transplant continues to increase. Despite the significant advances occurred in bariatric medicine, obesity is still considered a relative contraindication to liver transplantation(LT). The main aim of this review is to appraise the literature on the outcomes of OPs undergoing LT, treatments that might reduce their weight before, during or after surgery, and discuss some of the controversies and limitations of the current knowledge with the intent of highlighting areas where future research is needed.

Physiological and Morphological Responses of Susceptible and Resistant Barleys to Bird Cherry-Oat Aphid Feeding

The bird cherry-oat aphid (Rhopalosiphum padi [Linnaeus, 1758]) is considered a key pest of cereal crops worldwide, causing direct damage through sap feeding and by acting as a vector for viral diseases. Managing aphids is challenging because of their biology and potential resistance to insecticides. Developing resistant barley genotypes is a sustainable strategy for managing BCOA. In this study, we assessed responses of susceptible “Morex” and resistant “BCO R001” barley, Hordeum vulgare L. genotypes to different initial BCOA densities (0, 50, 100 or 200 ***-1). Physiological and morphological parameters were measured weekly for four weeks after infestation. Chlorophyll content, photosynthetic rate, plant aerial fresh and dry weight were greater for the resistant cultivar at lower aphid abundances and up to three weeks after infestation. Carbon assimilation curves (A/Ci) of infested “BCO R001” were similar to controls 15 days post infestation, differing from Morex. However, BCOA infestation of 50 ***-1 for two weeks negatively impacted the fitness of both genotypes. Initial resistance by BCO R001 to BCOA infestation can allow growers and natural enemies more time contributing to more effective and sustainable management of BCOA infestations.
