背景早前曾将2-氯普鲁卡因和布比卡因（C／B）联合用于区域麻醉，但因效果不确定而放弃。在这项前瞻性、双盲、随机研究中，我们比较了C／B联合-9利多卡因／布比卡因（L／B）联合的肌间沟阻滞的起效。方法30例拟在肌间沟阻滞下行肩关节镜术患者被分为2组，每组15例。一组（C／B）接受含有碳酸氢钠和肾上腺素的3％2．氯普鲁卡因，随即接受含肾上腺素的0．5％布比卡因；另一组（L／B）接受2％利多卡因而不用3％2．氯普鲁卡因。每15秒评估一次运动和感觉阻滞。主要的观察终点是完全运动阻滞的起效时间。使用时间-事件（生存率）统计分析检验。结果L／B组1例阻滞失败，被排除。C／B和L／B运动阻滞的中位数时间分别为90秒（15—575秒）和180秒（15．3720秒）（P=0．0325），C／B和L／B的感觉阻滞中位数时间分别为90秒（30～600秒）和210秒（30—3900秒）（P=0．0185）。生存分析显示，在第5分钟，C／B组15例患者中13例产生了成功的运动阻滞，而L／B组14例患者中只有7例。在第10分钟，C／B组15例患者全部产生了成功的运动阻滞，而L／B组14例中只有10例。对L／B组阻滞成功或失败的评价需时60分钟。结论本研究证明，肌间沟阻滞C／B比L／B较快达到成功阻滞。

背景与输血相关的高钾性心跳骤停是快速输注红细胞时发生的一项严重并发症。本文对快速输注红细胞过程中发生高钾血症及心跳骤停患者的临床情况和预后进行研究。方法本研究对1988年11月1日至2006年12月31日间，m.yo临床麻醉资料库记载的手术中发生输血相关性高钾血症及心跳骤停的所有患者进行了回顾性分析。结果我们确认共有16例因输血发生高钾性心跳骤停的患者，其中成年患者11例，儿童患者5例。多数患者接受如下3种手术：肿瘤手术、大血管手术及创伤手术。心跳骤停时血清钾离子平均浓度为7．2±1．4m.q／L（范围5．9-9．2m.q/L）。心跳骤停前红细胞输注量为1（2．7kg的新生儿）～54单位。心跳骤停时，几乎所有患者都存在酸中毒、高血糖症、低钙血症及体温过低。14例（87．5％）患者从中心静脉输注红细胞。11例成人患者中8例（72．7％）采用了市售高速输注泵，其他3例患者使用加压输血袋和快速注射泵输注红细胞。平均复苏时间为32分钟（2～127分钟），院内存活率为12．5％。结论输血相关的高钾血症性心跳骤停涉及多种发病机制，心输出量低下、酸中毒、高血糖症、低钙血症及低体温均可加重红细胞输注时的血钾升高。大出血输注大量库存红细胞时，应高度警惕高钾血症的可能，并做好预防措施。

AIm. To investigate the frequency, typical and atypical locations and patterns of m.lanom. m.tastases identifiable by com.uted tom.graphy (CT) in the abdom.n and pelvis. m.THODS: We perform.d a retrospective review of index CT exam.nations of the abdom.n and pelvis in patients with m.lanom. and recorded all findings suggestive of m.tastatic disease. RESULTS: m.tastases were present on 36% (181/508) of the index exam.nations and m.st com.only involved the liver (47%) and pelvic lym.h nodes (27%). Lower extrem.ty prim.ries had the highest rate of m.tastasis (52%). Ocular and head and neck m.lanom.s have a predilection to m.tastasize to the liver (hepatic involvem.nt in 70% and 63%, respectively, of patients with m.tastatic disease) and m.tastases from.lower extrem.ty prim.ries m.st com.only involve pelvic lym.h nodes (54% of patients with m.tastatic disease). m.tastases to atypical locations were present in 14% of patients and m.st com.only occurred in the subcutaneous tissue and spleen. Prim.ry tum.rs of the lower extrem.ty, back and head and neck were m.st com.only associated with atypical m.tastases. Pelvic m.tastases are m.re com.on with lower extrem.ty prim.ries (accounting for 70% of cases with pelvic m.tastases) but 5% of patients with supraum.ilical prim.ries also had pelvic m.tastases. CONCLUSION: The distribution of m.tastatic m.lanom. in the abdom.n and pelvis that we have defined should help guide the interpretation of CT exam. in these patients.

目的：观察青光眼手术患者虹膜切除标本的组织病理学特征。比较拉坦前列腺素治疗组与非拉坦前列腺素治疗组（对照组）组织学差异。方法：虹膜切除标本及患者病史资料呈送拉坦前列腺素病理学中心。标本由3位眼科病理学家以盲法分别观察。评估内容包括：癌变。癌前改变及其他病变：色素沉着水平差异，细胞构成差异。炎症，血管异常情况及虹膜色素上皮改变。结果：所收集标本中，449例曾接受拉坦前列腺素治疗，142例无拉坦前列腺素或其衍生物治疗史。所有标本均未发现癌或癌前病变。拉坦前列腺素治疗组，虹膜痣发生率显著高于对照组（P=0．001）。基质成纤维细胞和黑色素细胞数为对照组2倍（P〈0．001）。采用免疫组化染色技术分析2002年6月收集的各亚组标本，结果显示：拉坦前列腺素治疗组与非拉坦前列腺素治疗组黑色素细胞均数无显著差异（P=0．35）。结论：本研究结果支持已有观点，即：拉坦前列腺素诱发的眼部颜色的改变是由于虹膜基质黑色素细胞内黑色素数量增多而引起，虹膜痣数量增加可能是此效应的局部表现。
