Few patients with slow-transit constipation refractory to conservative treatment can benefit with a subtotal colectomy with ileorectal anastomosis with the preservation of the superior rectal *** this letter to the editor some important issues were ***,the study did not include a comparison ***,they did not present the functional results in the short or long term related to the bowel function of these patients after ***,the authors showed that this surgical procedure was safe,and no cases of leakage were found.

AIM： To analyze the risk of cardiovascular complications in patients with indication for surgical treatment of Chagasic esophageal achalasia and to correlate the surgical risks with the degree of esophageal dilation, thereby proposing a risk scale index. METHODS： One hundred and twenty-four patients with Chagasic esophageal achalasia, who received surgical treatment at the Hospital das Clinicas of the Federal University of Goiás, were included in this study. The patients were mostly related to the postoperative complications due to the cardiovascular system. All the patients were submitted to：（1） clinical history to define the cardiac functional class （New York Heart Association）; （2） conventional 12-lead electrocardiogram at rest; and （3） contrast imaging of the esophagus to determine esophageal dilatation according to Rezende＇s classification of Chagasic megaesophagus. RESULTS： An assessment of the functional classification （FC） of heart failure during the preoperative period determined that 67 patients （54.03%） were assigned functional class Ⅰ （FC Ⅰ）, 46 patients （37.09%） were assigned functional class Ⅱ （FC Ⅱ）, and 11 patients （8.87%） were assigned functional class Ⅲ （FC Ⅲ）. None of the patients were assigned to functional class Ⅳ （FC Ⅳ）. There was a positive correlation between the functional class and the postoperative complications （FC Ⅰ×FC Ⅱ： P〈0.001; FC Ⅰ×FC Ⅲ： P〈0.001）. The ECG was normal in 44 patients （35.48%） and presented abnormalities in 80 patients （64.52%）. There was a significant statistical correlation between abnormal ECG （arrhythmias and primary change in ventricular repolarization） and postoperative complications （P〈0.001）. With regard to the classification of the Chagasic esophageal achalasia, the following distribution was observed： group Ⅱ, 53 patients （42.74%）; group Ⅲ, 37 patients （29.83%）; and group Ⅳ, 34 patients （27.41%）. Metamaterial one-dimensional periodic structures are composed of split-ring resonators, which can display electric permittivity and magnetic permeability simultaneously negative, are studied experimentally. In the present study, each resonator is made up of two concentric circular copper rings patterned on a substrate of kapton, with slits diametrically opposite each other and with the line of the splits along the longitudinal direction of the periodic array containing seven split rings evenly spaced. The experiments consist in inserting the metamaterial slab into a square waveguide of side length 6 mm, corresponding to a cutoff frequency of 25 GHz. Transmission bands due to magnetic and electrical responses are identified for slits with aperture widths of 1 mm and 2 mm, centered at 5.67 and 6.12 GHz frequencies, respectively, values well below the 25 GHz frequency cutoff, so characterizing a medium with negative permeability and permittivity.

Macauba as Promising Substrate for Crude Oil and Biodiesel Production

The preparation of macauba fruits （Acrocomia aculeate （Jacq.） Lodd. ex Martius） both for oil extraction by continuous pressing and biodiesel production were investigated. The fruits and its pulp and kernel were characterized for their resulting oils. Experimental design was carried out to study the transesterification of each oil to obtain the concentrations of ethanol and catalyst that would provide the best biodiesel yield. A higher amount of biodiesel was produced from the best conditions to evaluate some characteristics as specified by ANP （National Agency of Petroleum, Natural Gas and Biofuels）. The results showed a variation in the fruits composition and different characteristics presented by the pulp and kernel oil. It was possible to define technological steps to obtain oil from the fruits of macauba and find the optimal conditions to biodiesel production, concentration of catalyst 4.42% and 5% and 13.3：1 and 9：1 ethanol：oil molar ratio to kernel oil and pulp oil, respectively.

Anemia as a Predictor of Mortality in Patients with Decompensated Heart Failure

Objective： To correlate file presence of anemia with mortality in hospitalized patients with decompensated HF （heart failure）. Methods： A total of 148 patients hospitalized for HF compensation were analyzed from April 1 to December 31, 2017 in a tertiary referral hospital for cardiovascular diseases. The diagnosis of anemia was established according to the Brazilian Ministry of Health criteria, considering a serum hemoglobin value of 〈 13 g/alL for men and 〈 12 g/dL for women. Data were collected through medical and physical attendance records. Statistical analysis was performed using the Epi Info program, version 7.0 for Windows, with Student＇ s t test for unpaired variables, Chi-square test and Fischer test. The significance ofp 〈 0.05 was considered. Results： Of the 148 selected patients, 63.5% were male, 60.1% were over 65 years old, 50.74% were enrolled in the first grade, and 93.2% were self-declared white. The prevalence of anemia observed in the study population was 72 （56.25%）, of which 49 （68%） were male and 23 （32%） were female. The mortality rate of the anemic patients was 19.4%, a total of 14 patients, whereas the non-anemic patients rate was 7.9%. It was observed that patients with worse outcome had hemoglobin 〈 12rag/alL （p = 0.045） and Functional Class （FC IV） by the New York Heart Association （NYHA）（p = 0.0001） at hospital admission. Conclusions： It is noticed that anemia is a frequent finding in patients with HF and that it interferes in the clinical manifestations, with worsening of the prognosis. Anemia treatment is not yet standarized in the HF approach, but patients without anemia appear to evolve with more favorable outcomes than anemia.

The search for diagnostic and prognostic markers in Alzheimer's disease(AD) has been an area of active research in the last decades. Biochemical markers are correlates of intracerebral changes that can be identified in biological fluids, namely: peripheral blood(total blood, red and white blood cells, platelets, plasma and serum), saliva, urine and cerebrospinal fluid. An important feature of a biomarker is that it can be measured objectively and evaluated as(1) an indicator of disease mechanisms(markers of core pathogenic processes or the expression of downstream effects of these processes), or(2) biochemical responses to pharmacological or therapeutic intervention, which can be indicative of disease modification. Platelets have been used in neuropharmacological models since the mid-fifties, as they share several homeostatic functions with neurons, such as accumulation and release of neurotransmitters, responsiveness to variations in calcium concentration, and expression of membrane-bound compounds. Recent studies have shown that platelets also express several components related to the pathogenesis of AD,in particular to the amyloid cascade and the regulation of oxidative stress: thus they can be used in the search for biomarkers of the disease process. For instance, platelets are the most important source of circulating forms of the amyloid precursor protein and other important proteins such as Tau and glycogen synthase kinase-3B. Moreover, platelets express enzymes involved in membrane homeostasis(e.g., phospholipase A2), and markers of the inflammatory process and oxidative stress. In this review we summarize the available literature and discuss evidence concerning the potential use of platelet markers in AD.
