Surface Modification on Ti-30Ta Alloy for Biomedical Application

Titanium and titanium alloys are currently being used for clinical biomedical applications due to their high strength, corrosion resistance and elastic modulus. The Ti-30Ta alloy has gotten extensive application as the important biomedical materials. The substrate surface of the Ti-30Ta alloy was altered either by chemical or topographical surface modification. The biocompatibility of an implant is closely related to its surface properties. Thus surface modification is one of effective methods for improving the biocompatibility of implants. The development status of biomedical materials has been summarized firstly, the biomedical application. In this study Ti-30Ta alloy surface was investigate as-casting (Group 1) modified with alkaline and heat-treatments in NaOH with 1.5M at 60°C for 24 hrs (Group 2), alkaline and heat-treatments with SBF-coatings by immersion in NaOH and SBFX5 for 24hrs (Group 3), anodization process was performed in an electrolyte solution containing HF (48%) and H2SO4 (98%) with the addition of 5% dimethyl sulfoxide (DMSO) 35V for 40 min (Group 4) and ion beam etching with 1200 eV ions with a beam current of 200 mA for a 3 hrs etch (Group 5). SEM was used to investigate the topography, EDS the chemical composition, and surface energy was evaluate with water contact angle measurement. SEM results show different structure on the surface for each group. EDS spectra identified similarity on Group 1, 4 and 5. The results indicate for group 2 an amorphous sodium tantalate hydrogel layer on the substrate surface and for group 3 the apatite nucleation on substrate surface. The Group 4 shows unorganized and vertically nanotubes and Group 5 shows a little alteration in the topography on the substrate surfaces. Overall the contact angle shows Group 5 the most hydrophobic and Group 4 the most hydrophilic. The study indicates Group 3 and 4 with potential for biomedical application. Yield of capsule endoscopy in obscure gastrointestinal bleeding:A comparative study between premenopausal and menopausal women

AIM To evaluate differences in capsule endoscopy(CE) performed in the setting of obscure gastrointestinal bleeding(OGIB) among premenopausal women(pMW) and menopausal women(MW).METHODS Retrospective, single-center study, including female patients submitted to CE in the setting of OGIB between May 2011 and December 2016. patients were divided into 2 groups according to age, considering fertile age as ≤ 55 years and postmenopausal age as > 55 years. The diagnostic yield(DY), the rebleeding rate and the time to rebleed were evaluated and compared between groups. Rebleeding was defined as a drop of Hb > 2 g/dL or need for transfusional support or presence of melena/*** A hundred and eighty three female patients underwent CE for OGIB, of whom 30.6%(n = 56) were pMW and 69.4%(n = 127) were MW. The DY was 30.4% in pMW and 63.8% in MW. The most common findings were angiodysplasias in both groups(pMW: 21.4%, MW: 44.9%)(p = 0.003). In pMW, only 1.8% required therapeutic endoscopy. In 17.3% of MW, CE findingsled to additional endoscopic treatment. Rebleeding at 1, 3 and 5 years in pMW was 3.6%, 10.2%, 10.2% and 22.0%, 32.3% and 34.2% in MW. postmenopausal status was significantly associated with higher DY(p p = 0.003), rebleeding(p = 0.031) and lower time to rebleed(p = 0.001).CONCLUSION pMW with suspected OGIB are less likely to have significant findings in CE. In MW DY, need for endoscopic treatment and rebleeding were significantly higher while time to rebleed was lower.

AIM: To evaluate the effect of promoter region polymorphisms of toll-like receptor(TLR)2-196 to-174 del and TLR4-1607T/C(rs10759932) on m RNA and protein expression in tumor tissue and of TLR4+896A/G(rs4986790) on colorectal cancer(CRC) ***: The TLR2-196 to-174 del polymorphism was investigated using allele-specific polymerase chain reaction(pCR) and the TLR4-1607T/C and TLR4+896A/G by pCR-restriction fragment length p o l y m o r p h i s m( R F L p). W e g e n o t y p e d 4 3 4 D N A samples from 194 CRC patients and 240 healthy individuals. The m RNA relative quantification(RQ) was performed in 40 tumor tissue samples by quantitative pCR Taq Man assay, using specific probes for TLR2 and TLR4 genes, and ACTB and GApDH reference geneswere used as endogenous controls. protein expression was analyzed by immunohistochemistry with specific primary ***: No association was found for TLR4-1607T/C and TLR4+896A/G by three statistical models(logadditive, dominant and recessive). However, based on dominant and log-additive models, the polymorphic variant TLR2-196 to-174 del was associated with increased CRC risk [dominant: odds ratio(OR) = 1.72, 95%CI: 1.03-2.89; p = 0.038 and log-additive: OR =1.59, 95%CI: 1.02-2.48; p = 0.039]. TLR2 m RNA expression was increased in tumor tissue(RQ = 2.36) when compared to adjacent normal tissue(RQ = 1; p pression(RQ = 0.74 vs RQ = 1, p = 0.452). Immunohistochemistry analysis of TLR2 and TLR4 protein expression was concordant with the findings of m RNA expression. In addition, the TLR2-196 to-174 del variant carriers showed m RNA relative expression 2.19 times higher than wild-genotype carriers. The TLR2 protein expression was also higher for the TLR2-196 to-174 del variant carriers [117 ± 10 arbitrary unit(a.u.) vs 95 ± 4 a.u., p = 0.03]. AIM To evaluate the association between polymorphismsin glutathione S transferases(GSTs) and the risk of sporadic colorectal cancer(SCRC), tumor progression and the survival of *** A case-control study of 970 individuals from the Brazilian population was conducted(232 individuals from the case group with colorectal cancer and 738 individuals from the control group without a history of cancer). pCR multiplex and pCR-RFLp techniques were used to genotype the GST polymorphisms. The tumors were categorized according to the TNM classification: tumor extension(T), affected lymph nodes(N), and presence of metastasis(M). Logistic regression, multiple logistic regression and survival analysis were used to analyze the data. The results are presented in terms of odds ratio(OR) and 95% confidence interval(CI). The level of significance was set at 5%(p ≤ 0.05).RESULTS Age equal to or over 62 years(OR = 8.79; 95%CI: 5.90-13.09, p p polymorphisms revealed an association between the GSTM1 polymorphisms and a risk of SCRC(OR = 1.45; 95%CI: 1.06-2.00; p = 0.02), as well as between GSTT1 and a reduced risk of the disease(OR = 0.65; 95%CI: 0.43-0.98; p = 0.04). An interaction between the presence of the wild-type allele of GSTp1 Ile105 Val polymorphism and tobacco consumption on risk of SCRC(OR = 2.33; 95%CI: 1.34-4.05; p = 0.05) was observed. There was an association between the GSTM1 null genotype and the presence of advanced tumors(OR = 2.33; 95%CI: 1.23-4.41; p = 0.009), as well as increased risk of SCRC in the presence of a combination of GSTT1 non-null/GSTM1 null genotypes(OR = 1.50; 95%CI: 1.03-2.19; p = 0.03) and GSTT1 non-null/GSTM1 null/GSTp1 Val~*(OR = 1.85; 95%CI: 1.01-3.36, p = 0.04). Impact of the timing of capsule endoscopy in overt obscure gastrointestinal bleeding on yield and rebleeding rate-is sooner than 14 d advisable?

AIM To evaluate the impact of the timing of capsule endoscopy(CE) in overt-obscure gastrointestinal bleeding(OGIB). METHODS Retrospective, single-center study, including patients submitted to CE in the setting of overt-OGIB between January 2005 and August 2017. patients were divided into 3 groups according to the timing of CE(≤ 48 h; 48 h-14 d; ≥ 14 d). The diagnostic and therapeutic yield(DY and TY), the rebleeding rate and the time to rebleed were calculated and compared between groups. The outcomes of patients in whom CE was performed before(≤ 48 h) and after 48 h(> 48 h), and before(patients underwent CE for overt-OGIB. The DY was 80%, TY-46.1% and rebleeding rate-32.2%. At 1 year 17.8% of the patients had rebled. 33.9% of the patients performed CE in the first 48 h, 30.4% between 48 h-14 d and 35.7% after 14 d. The DY was similar between the 3 groups(p = 0.37). In the ≤ 48 h group, the TY was the highest(66.7% vs 40% vs 31.7%, p = 0.005) and the rebleeding rate was the lowest(15.4% vs 34.3% vs 46.3% p = 0.007). The time to rebleed was longer in the ≤ 48 h group when compared to the > 48 h groups(p = 0.03).CONCLUSION performing CE within 48 h from overt-OGIB is associated to a higher TY and a lower rebleeding rate and longer time to rebleed.

Alpha Defensins Genes and Vulvovaginal Candidiasis:A Study of Cases

Objective: To evaluate the alpha-defensin (α-DF) genes polymorphism in women with vulvovaginal candidiasis and recurrence. Methods: This observational study included clinical vaginal secretion samples collected over four years from 88 women, ranging in age from 18 to 65 years, from medical centers of Sao paulo and Mogi das Cruzes, Brazil. Thirty-six of these women were asymptomatic (control group) and 52 presented clinical condition compatible with vulvovaginitis (38 primary or episodic as non-recurrent forms, and 14 recurrent vulvovaginal candidiasis). A portion of each sample was plated on Sabouraud dextrose agar with chloramphenicol and grown on CHROMagar Candida for presumptive characterization. The identification of the species was obtained by sequencing of the ITS1 region of rDNA. α-DF genes were amplified for subsequent evaluation of polymorphisms by endonuclease restriction assay. Results: From 88 samples were isolated 60 Candida albicans and 28 non-albicans Candida spp. Resistant C. albicans strains and non-albicans Candida spp. were more prevalent in recurrence. In all groups, the number of resistant non-albicans Candida spp. was most high than susceptible strains. α-DF1, α-DF3 and α-DF1/α-DF3 genotypes were found in 32 (36.4%), 17 (19.3%), 6 (6.8%) vaginal samples, respectively. About 33 samples were not amplified. Recurrence and severe disease were more observed in homozygous population. Conclusions: Non-albicans Candida spp. and homozygotic α-DF genotipes (α-DF1 and α-DF3) were more related with severe clinical signs and recurrence. Further studies about vulvovaginal candidiasis and α-DF genes are necessary to access the more comprehensive role of defensins in clinical manifestations.

Methadone Maintenance Treatment program in portuguese Community pharmacies

MMT （Methadone maintenance treatment） is an effective way to treat opioid dependence. In portugal, an MMT program has been available in pharmacies since January 1998, when a formal agreement was signed by the IDT, I.p. （Institute on Drugs and Drug Addiction）, the ppS （portuguese pharmaceutical Society）, and the ANF （National Association of pharmacies）. In January 2004, the INFARMED （National Authority of Medicine and Health products） became a partner of the program. The possibility of continuing the treatment at a community pharmacy was restricted to patients receiving a stabilized dose of methadone at IDT, I.p. treatment centers. pharmacists joining the MMT were given mandatory training. patients on the MMT program received daily doses of methadone solution under directly observed therapy, and were followed up by trained pharmacists. From January 1998 to January 2013, 3,090 patients underwent MMT in portuguese community pharmacies. The delivery of MMT at community pharmacy level is feasible. This strategy improves access and adherence to methadone treatment, thus helping to reduce the use of illegal opioids. Community pharmacists fully demonstrated their ability to perform extended roles in public health and harm-reduction strategies. Interaction and close cooperation between the different health professionals and organizations involved were crucial to achieve adequate support to the patients.
