Systemic mastocytosis: A rare cause of non-cirrhotic portal hypertension

Mastocytosis is a clonal neoplastic disorder of the mast cells(MC) that can be limited to the skin(cutaneous mastocytosis) or involve one or more extracutaneous organs(systemic mastocytosis). The clinical manifestations of mastocytosis are heterogeneous ranging from indolent disease with a long-term survival to a highly aggressive neoplasm with survival of about 6 mo. Although liver involvement in aggressive systemic mastocytosis(ASM) is relatively common, the development of portal hypertension with or without cirrhosis is rare. We report a case of ASM without skin involvement in a 72-year-old caucasian male who presented with non-cirrhotic portal hypertension based on clinical, analytical, imagiological and endoscopic findings. Given the hematological picture, the correct diagnosis was established based on ancillary tests for MC using bone marrow aspirates and biopsy. Extensive involvement of the liver and gastrointestinal tract was histologically documented. The disease progressed rapidly and severe pancytopenia and recurrent upper gastrointestinal bleeding became the dominant problem. This case illustrates the challenge in establishing a diagnosis of ASM especially when the clinical picture is atypical and without skin involvement. Gastroenterologists should consider infiltrative disease, particularly systemic mastocytosis, as a differential diagnosis in a clinical case of portal hypertension of unknown etiology.

Effectiveness of sub-acromial injections in rotator cuff injuries:A systematic review and meta-analysis

BACKGROUND Sub-acromial injections are a therapeutic option for rotator cuff injuries;however,evidence regarding the most effective drug in this context is unclear,which needs to be *** To evaluate the effectiveness of various sub-acromial injections for rotator cuff *** We conducted a systematic review and pair-wise and network meta-analyses of randomized clinical trials(RCTs)comparing sub-acromial injections for rotator cuff *** interventions evaluated were hyaluronic acid(HA),platelet-rich plasma(PRP),prolotherapy,and *** outcomes of interest were pain and functional improvement,which were evaluated with standardized *** Risk of Bias 2 tool and the Grading of Recommendations,Assessment,Development and Evaluation methodology were used to assess data *** Twenty RCTs,comprising 1479 participants,were *** the short term,HA achieved the best outcomes[pain mean difference(MD)=-1.48,95%confidence interval(CI)-2.37 to-0.59;function MD=10.18,95%CI:4.96-15.41].In the medium term,HA,PRP,HA+PRP,and corticosteroids were not superior to placebo in improving *** on function,HA+PRP was superior to placebo,corticosteroids,and PRP(MD=26.72;95%CI:8.02-45.41).In the long term,HA,PRP,and corticosteroids were not superior to placebo in reducing ***,based on function,HA+PRP,PRP,and HA were superior to placebo,and HA+PRP had the best result(MD=36.64;95%CI:31.66-33.62).CONCLUSION HA provides satisfactory short-term results,while HA with PRP demonstrates functional improvement in the medium and long ***,no intervention maintained the pain-relief effect on>3-month follow-up.

Development of metal organic fromwork-199 immobilized zeolite foam for adsorption of common indoor VOCs

Reticulated foam shaped adsorbents are more efficient for the removal of volatile organic compounds（VOCs）,particularly from low VOC-concentration indoor air streams. In this study composite structure of zeolite and metal organic frameworks（MOFs）,referred as ZMF,has been fabricated by immobilization of fine MOF-199 powder on foam shaped Zeolite Socony Mobil-5（ZSM-5）Zeolitic structure,referred as ZF. The ZMF possess a uniform and well-dispersed coating of MOF-199 on the porous framework of ZF. It shows higher surface area,pore volume,and VOCs adsorption capacity,as compared to ***-fabrication changes in selective adsorption properties of ZMF were studied with three common indoor VOCs（benzene,n-hexane,and cyclohexane）,using gravimetric adsorption technique. The adsorption capacity of ZMF with different VOCs follow the order of benzene 〉 n-hexane 〉 cyclohexane. In comparison with MOF-199 and ZF,the composite structure ZMF shows improvement in selectivity for benzene from other two VOCs. Further,improvement in efficiency and stability of prepared ZMF was found to be associated with its high MOF loading capacity and unique morphological and structural properties. The developed composite structure with improved VOCs removal and recyclability could be a promising material for small to limited scale air pollution treatment units.

Intelligent negotiation model for ubiquitous group decision scenarios

Supporting group decision-making in ubiquitous contexts is a complex task that must deal with a large amount of factors to succeed. Here we propose an approach for an intelligent negotiation model to support the group decision-making process specifically designed for ubiquitous contexts. Our approach can be used by researchers that intend to include arguments, complex algorithms, and agents' modeling in a negotiation model. It uses a social networking logic due to the type of communication employed by the agents and it intends to support the ubiquitous group decision-making process in a similar way to the real process, which simultaneously preserves the amount and quality of intelligence generated in face-to-face meetings. We propose a new look into this problem by considering and defining strategies to deal with important points such as the type of attributes in the multicriterion problems, agents' reasoning, and intelligent dialogues.

TLBO with variable weights applied to shop scheduling problems

The teaching–learning-based optimisation (TLBO) algorithm is a population-based metaheuristic inspired on the teaching–learning process observed in a classroom. It has been successfully used in a wide range of applications. In this study, the authors present a variant version of TLBO. In the proposed version, different weights are assigned to students during the student phase, with higher weights being assigned to students with better solutions. Three different approaches to assign weights are investigated. Numerical experiments with benchmark instances of the flow-shop and the job-shop scheduling problems are carried out to investigate the performance of the proposed approaches. They compare the proposed approaches with the original TLBO algorithm and with two variants of TLBOs proposed in the literature in terms of solution quality, convergence speed and simulation time. The results obtained by the application of a Friedman statistical test showed that the proposed approaches outperformed the original version of TLBO in terms of convergence, with no significant losses in the average makespan. The additional simulation time required by the proposed approaches is small. The best performance was achieved with the approach of assigning a fixed weight to half the students with the best solutions and assigning zero to other students.

3D Progressive Damage Based Macro-Mechanical FE Simulation of Machining Unidirectional FRP Composite

Finite element(FE) simulation is a powerful tool for investigating the mechanism of machining fiber?reinforced polymer composite(FRP). However in existing FE machining simulation works,the two?dimensional(2 D) progressive damage models only describe material behavior in plane stress,while the three?dimensional(3 D) damage models always assume an instantaneous sti ness reduction pattern. So the chip formation mechanism of FRP under machin?ing is not fully analyzed in general stress state. A 3 D macro?mechanical based FE simulation model was developed for the machining of unidirectional glass fiber reinforced plastic. An energy based 3 D progressive damage model was proposed for damage evolution and continuous sti ness degradation. The damage model was implemented for the Hashin?type criterion and Maximum stress criterion. The influences of the failure criterion and fracture energy dissipa?tion on the simulation results were studied. The simulated chip shapes,cutting forces and sub?surface damages were verified by those obtained in the reference experiment. The simulation results also show consistency with previous 2 D FE models in the reference. The proposed research provides a model for simulating FRP material behavior and the machining process in 3 D stress state.
