The management of water consumption in healthcare centres can have positive impacts on both the environmental performance and profitability of health *** tools assist in the decision-making process of managing the operation and maintenance of healthcare *** research aimed to integrate the empirical knowledge of experts in Healthcare Engineering and the historical data from 66 healthcare centres in a Fuzzy Cognitive *** outputs of the predictive model included water consumption,water cost,and CO_(2) emissions in healthcare facilities,along with eleven variables to discover the causes and consequences of water consumption in healthcare centres.A healthcare centre with about 12350 users,located in a city that experiences an average of 1100 heating degree days,whose facilities be moderately energy-efficient contributing over 50%with renewable energies is expected to consume 8.4 dam^(3) of water with 32.1 k€of cost,and contribute realising 30.8 ton CO_(2)eq *** use of Fuzzy Cognitive Maps for prediction can provide a high level of effectiveness in identifying the factors that contribute to water consumption and in designing key performance indicators to manage the environmental performance of healthcare *** tool is extremely effective in enhancing the performance of the management division of health systems.

Objectives: The biologic behavior of the adenoid cystic carcinoma (ACC) and the factors predicting outcome for these tumors are still poorly understood. Our objective is to analyze the predicting factors and the value of different treatment possibilities, since none sole treatment has been standardized. Methods: A retrospective analysis of the epidemiologic, clinical and histologic aspects of ACC, as well as treatment options and other prognostic factors of all the cases of ACC of the head and neck treated at this Institution were analyzed. From 1974 until 2011, 152 patients were diagnosed with ACC and treated at the Portuguese Institute of Oncology (Porto Centre). Main outcome measures: overall survival, local recurrence and distant metastasis were calculated by the Kaplan-Meier method. Factors predictive of outcome were identified by univariate and multivariate analysis. Results: The mean age at diagnosis was 55.8 years (range, 19-83 years). Incidence was higher in the female population, with a female to male ratio of 1.7:1 respectively. The primary tumor location was hard palate and submandibular region in 56 cases, 28 in each location (24.6%), parotid gland (17 cases, 14.9%) and oral cavity excluding palate (16 cases, 14%).Distributions according to T stage were: T1 (29.8%);T2 (30.7%);T3 (17.5%);T4 (22%). The overall 1-year, 5-year and 10-year survival for all patients were 94.6%, 60.5%, 41.6%, respectively. Conclusions: Univariate survival analysis revealed that age older than 60 years (p = 0.002), solid histologic subtype (p = 0.042), advanced clinical stage (p p = 0.036) were correlated with a poor survival. Multivariate analysis confirmed that age and advanced clinical stage were worst independent predicators of overall survival as well as perineural invasion for local recurrence and distant metastasis. In our analysis, radiotherapy did not have a relevant impact on survival, except in cases of solid histologic subtype. To analyze distant metastatic capacity

Business simulation games are considered as effective tools for the empowerment and mediation of business content learning. They act as serious games which contribute to learning through a simulation of real-life situations and busi-ness environments. They are especially useful in the area of business management processes and business strategies. The blending of designed simulation technology and content curricula offers participants (players, students) a risk-free opportunity to test out a range of relevant strategies to drive business results. By customizing computer-based business simulations, participants can integrate key strategic and financial priorities. This paper provides a brief review of busi-ness simulations that serve learning purposes. The first part presents a short introduction and description of business games and their evaluation properties, and the second part provides a brief evaluation and analysis of selected business simulation games.

Introduction: Total laryngectomy/pharingo-laryngectomy is a potentially aggressive surgery for advanced laryngeal/hypopharyngeal carcinomas, which results in important physical and functional changes that compromise some of the most vital functions, including speech communication. For these patients, tracheoesophageal speech is considered to be the gold standard for voice rehabilitation. Objectives: The purpose of the present study was to determinate the success rate of voice prosthesis rehabilitation, voice prosthesis lifetime and the rate of complications, and its related clinicopathologic factors. Material and Methods: Retrospective review of 92 patients who undergone tracheoesophageal puncture (TEP) performed between January 2007 and December 2012 at the Francisco Gentil Portuguese Institute of Oncology of Oporto. Age, primary disease, staging, the extent of surgical resection, radiotherapy treatment, timing of TEP, surgical and prosthesis-related complications were noted. The impact of these clinicpathological factors on functional outcome, complications of TEP and lifetime of prosthetic valves was assessed in univariate analysis. Vocal rehabilitation efficacy with voice prosthesis was assessed with the multidimensional Harrison-Robillard Shultz (HRS) Rating Scale. Lifetime of voice prosthesis and early and late complications were recorded. Results: A total of 83 patients met the study criteria, predominantly males (94%) with a mean age of 63.7 years. 77% of the patients underwent primary and 23% secondary TEP. 68.7% of patients achieved functional tracheoesophageal speech (HRS score ≥ 10), 67.2% had performed primary TEP and 73.7% had performed secondary TEP. The mean device lifetime was 9.8 months for voice prosthesis. Prosthesis-related complications occurred in 81% of the patients and the most common issues were prosthesis leakage (76%) and displacement (22%). Xerophyticplants are well adapted to the semiarid conditions because they have a high potential for the use of rainwater. They can store it in their roots (xylopodiums). The Amburanacearensis, commonly called Cumaru, native of the Brazilian semiarid, is very important because of its multiple-use: good quality wood, active food ingredient, perfume and medicine production. However, its management is practically unknown. Aiming to know more about this culture, an experiment was conducted to quantify the Cumaruwater needs, evaluate its water-use efficiency and the capacity of roots to store water. Five levels of water (0%, 25%, 50%, 75% and 100% of evapotranspiration) and five-plant spacing (0.20 × 0.20 m, 0.20 × 0.10 m, 0.20 × 0.05 m, 0.10 × 0.10 m and 0.05 × 0.05 m) were tested, and the results obtained analyzed statistically throughout F test. No influence of soil water level and plant spacing in the moisture content of the whole plant, in the aerial part or in the xylopodiums was observed. However, there was an effect of the treatments on the water stored in the whole plant in the aerial part or in the roots, per unit area. More than 80% of the water was stored in the roots. The crop growing with the smaller spacing without irrigation was shown to be quite efficient in using the rainwater, compared with traditional cultures irrigated.

The influence of xenobiotic compounds on environment and on living organisms has been reported as an imminent public health problem. Among them we can list the contamination by Alkanes present in petroleum, hydrocarbons and organic contaminant substances from industrial effluents. Also, heavy metals are of particular interest because of their persistence in the environment contaminating the food webs. Among the innovative solutions for treatment of contaminated water and soil is the use of biological materials like living or dead microorganisms. Yeasts exhibit the ability to adapt to extreme condition such as temperature, pH and levels of organic and inorganic contaminants that make them a potential material to be used to remediate contaminated environment application. The goal of this work was to search for yeast isolates capable to use n-hexadecane (alkane hydrocarbon) as a primary carbon source and for those able to tolerate high concentration of lead (Pb) within a collection of 90 isolates obtained from the Sao Paulo Zoo composting system. The isolated yeast strains were identified by mass spectrometry (MALDI-TOF-MS) and by sequencing of the ribosomal DNA (18S and D1/D2) conserved regions. We found that the collection bares 23 isolates capable of utilizing n-hexadecane and one which is able to tolerate high concentration of lead (Pb) with a high biosorption index compared to the reference yeast strains (BY4742, PE-2, CAT-1 and BG-1). These results confirm the initial hypothesis that the Sao Paulo Zoo composting is the source for diverse yeasts species with biotechnological application potential.

Paracoccidioidomycosis is the most prevalent systemic mycosis in Latin America. It is a fungal infection caused by the dimorphic fungus Paracoccidioides brasiliensis. This microorganism can penetrate the human body through direct contact and can cause intense lymphadenopathy, ulcerated lesions in the oral cavity and in the skin among other alterations. In this study, we report a case of paracoccidioidomycosis with unusual clinical presentation, as it manifested in the female sex and although it did not present any relevant complaints of respiratory focus, it presented a single ulcerated lesion in the buccal mucosa. A biopsy of the lesion and a mycological examination revealed Paracoccidioidesbrasiliensis infection.

Quantification of theophylline or paracetamol in milk matrices by high-performance liquid chromatography

A simple, accurate and sensitive high-performance liquid chromatography （HPLC） method was developed, validated and applied to the determination of either theophylline or paracetamol in milk-based samples. The method allowed drug quantification in fresh and powdered milk with a relatively short run time of analysis and it was also successfully applied to the quantification of the drugs in solid dosage forms intended for pediatric use. Moreover, the main significant advantages over other published works are the simplicity of the sample preparation, reduced assay time and sample loss. The method meets the International Conference on Harmonization guideline for analytical methods validation regarding specificity, linearity, accuracy, precision, specificity and robustness as required by health authorities and applied by industry while designing and marketing new drug products. The technique encompasses the separation of the analytes with a reverse phase C18 column under isocrafic conditions and UV detection at 272 nm and 243 nm, respectively, for theophylline and paraeetamol. The lower limit of quantification for both drugs was determined as 0.2 μg/mL and the between-batch accuracy was 99.7%. This HPLC method allows quantification of theophylline and paracetamol in milk matrices and it can be applied in the design, development and production of milk-based pediatric dosage forms.

Studies have confirmed that bone marrow-derived mesenchymal stem cells （MSCs） can be used for treatment of several nervous system diseases. However, isolation of bone marrow-derived MSCs （BMSCs） is an invasive and painful process and the yield is very low. Therefore, there is a need to search for other alterative stem cell sources. Adipose-derived MSCs （ADSCs） have phenotypic and gene expression profiles similar to those of BMSCs. The production of ADSCs is greater than that of BMSCs, and ADSCs proliferate faster than BMSCs. To compare the effects of venous grafts containing BMSCs or ADSCs on sciatic nerve injury, in this study, rats were randomly divided into four groups： sham （only sciatic nerve exposed）, Matrigel （MG; sciatic nerve injury ＋ intravenous transplantation of MG vehicle）, ADSCs （sciatic nerve injury ＋ intravenous MG containing ADSCs）, and BMSCs （sciatic nerve injury ＋ intravenous MG containing BMSCs） groups. Sciatic functional index was calculated to evaluate the function of injured sciatic nerve. Morphologic characteristics of nerves distal to the lesion were observed by toluidine blue staining. Spinal motor neurons labeled with Fluoro-Gold were quantitatively assessed. Compared with sham-operated rats, sciatic functional index was lower, the density of small-diameter fibers was significantly increased, and the number of motor neurons significantly decreased in rats with sciatic nerve injury. Neither ADSCs nor BMSCs significantly improved the sciatic nerve function of rats with sciatic nerve injury,increased fiber density, fiber diameters, axonal diameters, myelin sheath thickness, and G ratios （axonal diameter/fiber diameter ratios） in the sciatic nerve distal to the lesion site. There was no significant difference in the number of spinal motor neurons among ADSCs, BMSCs and MG groups. These results suggest that neither BMSCs nor ADSCs provide satisfactory results for peripheral nerve repair when using MG as the conductor for engraftment.
