Radiotherapy(RT)is the backbone of multimodality treatment of more than half of cancer *** new modern RT techniques,late complications may occur such as radiation proctitis(RP).The natural history of RP is *** symptoms may resolve spontaneously or require conservative *** the other hand,for similar and uncomplicated clinical contexts,symptoms may persist and can even be refractory to the progressive increase in treatment *** the last decades,an enormous therapeutic armamentarium has been considered in RP,including hyperbaric oxygen therapy(HBOT).Currently,the evidence regarding the impact of HBOT on RP and its benefits is *** prospective and randomised studies are necessary to validate HBOT’s effectiveness in the‘real world’clinical *** article reviewed the relevant literature on pathophysiology,clinical presentation,different classifications and discuss RP management including a proposal for a therapeutic algorithm with a focus on HBOT.

The road to sustainable use and waste management of plastics in Portugal

As a European Union(EU)member,Portugal must comply with reductions in plastic *** Portugal,the 330 items/100 m of beach litter,comprising up to 3.9 million pieces and of which 88%is plastic,is higher than the EU median(149 items/100 m)and must be reduced to 20 items/100 m(94%).Integrative measures are needed to reduce littering and improve plastics’use and disposal under the circular *** this 414 kt of plastic packaging waste,163 kt were declared plastic packaging,140 kt subjected to recycling,and 94 kt to energy *** current recycling rate of plastic packaging(34%)should be improved to reach EU recycling averages(42%)and goals and to provide widespread benefits,considering revenues of 167€/*** a net importer of waste,Portugal could benefit from the valorization of imported *** increased recycling,pyrolysis and gasification could provide short-term alternatives for producing value-added substances from plastic waste,such as hydrogen,consistent with the National Plan of Hydrogen and improving ongoing regulations on single-use *** manuscript provides an integrative view of plastics in Portugal,from use to disposal,providing specific recommendations under the circular economy.

Trauma to the peripheral nervous system often results in loss of motor and sensory functions of the affected area of the body,leading to a series of functional impairments(Allodi et al.,2012).Injuries to peripheral nerves initiate a series of complex events,known as Wallerian degeneration,which allows the injured axons to regenerate and reinnervate their targets(Allodi et al.,2012).Damage to a nerve induces multiple alterations,which include:axonal degeneration,breakdown of myelin sheath,Schwann cells(SC)proliferation and conversion to a repair phenotype which express cytokines and chemokines that allow the infiltration and activation of macrophages(Mietto et al.,2015;Jessen and Mirsky,2016).

Injuries to the spinal cord result in permanent disabilities that limit daily life *** main reasons for these poor outcomes are the limited regenerative capacity of central neurons and the inhibitory milieu that is established upon traumatic *** decades of research,there is still no efficient treatment for spinal cord *** strategies are tested in preclinical studies that focus on ameliorating the functional outcomes after spinal cord *** these,molecular compounds are currently being used for neurological recovery,with promising *** molecules target the axon collapsed growth cone,the inhibitory microenvironment,the survival of neurons and glial cells,and the re-establishment of lost *** this review we focused on molecules that are being used,either in preclinical or clinical studies,to treat spinal cord injuries,such as drugs,growth and neurotrophic factors,enzymes,and *** mechanisms of action of these molecules are discussed,considering traumatic spinal cord injury in rodents and humans.

Capsule enteroscopy is useful for the therapeutic management of Crohn's disease

AIM: To analyze therapeutic changes in Crohn's disease(CD) patients following video capsule endoscopy(VCE) and to assess the usefulness of Lewis score and the Patency ***: Patency Capsule was performed in every patient that had indication for VCE, and those with negative patency did not undergo VCE. Patients with established CD that underwent VCE between January 2011 and February 2014 were selected for this study; those with suspected CD were excluded, independent of VCE results, since our purpose was to address differences in therapeutic regimen in CD patients before and after VCE. Patients with inconclusive VCE were also excluded. Patients had to be free of non-steroidal anti-inflammatories for at least 1 mo. Those patients who met these criteria were allocated into one of three groups: Staging group(asymptomatic CD patients that underwent VCE for staging of CD), Flare group(patients with active CD), or Post-op group(CD patients evaluated for post-operative recurrence). Lewis score was calculated for every VCE procedure. Statisticalanalysis was performed to address the impact of VCE findings on the therapeutic management of CD patients and to evaluate the utility of the Lewis ***: From a total of 542 VCEs, 135 were performed in patients with CD. Patency capsule excluded nearly 25% of the patients who were supposed to undergo VCE. No videocapsule retention during VCE was reported. From these 135 patients, 29 were excluded because CD diagnosis was not established at the time of VCE. Therefore, a total of 106 patients were included in the final analysis. From these, the majority were in the Staging group(n = 73, 69%), and the remaining were in the Flare(n = 23, 22%) or Post-op(n = 10, 9%) group. Median time between diagnosis and VCE was 5.5 years. Overall, VCE determined changes in the treatment of 40% of patients: only 21% remained free of immunosuppressors after VCE compared to 44% before VCE(P < 0.001). The antibacterial potential of silver nanoparticles（AgNPs） resulted in their increasing incorporation into consumer,industrial and biomedical ***,human and environmental exposure to AgNPs（either as an engineered product or a contaminant）supports the emergent research on the features conferring them different toxicity *** this study,30 ran AgNPs coated with citrate or poly（ethylene glycol）（PEG） were used to assess the influence of coating on the effects produced on a human hepatoma cell line（HepG2）,namely in terms of viability,apoptosis,apoptotic related genes,cell cycle and cyclins gene *** types of coated AgNPs decreased cell proliferation and viability with a similar toxicity *** the concentrations used（11 and 5 μg/mL corresponding to IC50 and-IC10 levels,respectively） the amount of cells undergoing apoptosis was not significant and the apoptotic related genes BCL2（anti-apoptotic gene）and BAX（pro-apoptotic gene） were both ***,both AgNPs affected HepG2 cell cycle progression at the higher concentration（11 μg/mL） by increasing the percentage of cells in S（synthesis phase） and G2（Gap 2 phase） *** the cell-cycle related genes,the expression of cyclin B1 and cyclin E1 genes were ***,this work has shown that citrate- and PEG-coated AgNPs impact on HepG2 apoptotic gene expression,cell cycle dynamics and cyclin regulation in a similar *** research is needed to determine the properties that confer AgNPs at lower toxicity,since their use has proved helpful in several industrial and biomedical contexts.

The influence of disturbance on driving carbon stocks and tree dynamics of riparian forests in Cerrado

Aims Riparian forests in the Brazilian Cerrado,also known as gallery forests,are very heterogeneous in structure,species composition and ecological features due to strong and abrupt variations of soil,hydrological and topographic ***,what are the variables driving forest carbon stock and productivity,mortality and recruitment in disturbed gallery forests?Methods We used 36 permanent plots data from a gallery forest in the Brazilian *** investigated how tree community dynam-ics vary in a gallery forest on two contrasting disturbance levels-logged and non-logged-across a topographic gradient intrinsically related to differences in moisture conditions,edge effects,as well as soil fertility and *** Findings Soil variables were reduced into principal components and we used structural equation modelling to disentangle covarying *** also included carbon stocks as a determinant variable of dynamics *** forest had 50%higher productivity than non-logged forest and streamside forest had aboveground carbon stocks 70%higher than the forest *** logging and natural disturbance drove variation in the carbon stocks which contributed to shaping productivity and recruitment *** from the river also drove mortality and carbon stock *** with high-carbon stocks favoured higher competition and lessened produc-tivity and recruitment *** soil fertility and texture are considered crucial components shaping forest dynamics,there was no clear influence of those variables on the present forest,probably because the strong effects of soil moisture,forest edge and distur-bance disrupted the correlation between soil and forest dynamics.

Needle chlorosis(NC)in Pinus taeda *** in Brazil becomes more frequent after second and third harvest rotation *** a study to identify factors contributing to yellowing needle chorosis(YNC),trees were grown in soils originating from contrasting parent materials,and soils and needles(whole,green and chlorotic portions)from 1-and 2-year-old branches and the first and second needle flush release at four sites with YNC on *** were analyzed for various elements and *** soils had very low base levels(Ca^(2+),Mg^(2+)and K^(+))and P,suggesting a possible lack of multiple *** symptoms started at needle tips,then extended toward the needle base with *** flush needles had longer portions with YNC than second flush needles *** from the lower crown also had more symptoms along their length than those higher in the *** were similar to those reported for *** chlorotic portions,Mg and Ca concentrations were well below critical values;in particular,Mg levels were only one third of the critical value of 0.3 g kg^(-1).Collectively,results suggest that Mg deficiency is the primary reason for NC of *** in various parent soils in Brazil.

Emerging role of liquid biopsy in rat sarcoma virus mutated metastatic colorectal cancer:A case report

BACKGROUND In patients with metastatic colorectal cancer(mCRC),the treatment options are limited and have been proved to be affected by rat sarcoma virus(RAS)mutational *** RAS wild-type(wt)patients,the combination of antiepidermal growth factor receptor(EGFR)monoclonal antibodies with chemotherapy(CT)is more effective than CT *** the other hand,RAS-mutated patients are not eligible for treatment with anti-EGFR *** SUMMARY Eleven patients with initially RAS-mutated mCRC were followed from diagnosis to May *** the time of cell-free DNA determination,five patients had undergone one CT line,five patients had undergone two CT lines,and one patient had undergone three CT lines(all in combination with bevacizumab).At the second and third treatment lines[second line(2L),third line(3L)],patients with neo-RAS wt received a combination of CT and *** neo-RAS wt patients treated with anti-EGFR,our findings indicated an increase in progression-free survival for both 2L and 3L(14.5 mo,P=0.119 and 3.9 mo,P=0.882,respectively).Regarding 2L overall survival,we registered a slight increase in neo-RAS wt patients treated with anti-EGFR(33.6 mo vs 32.4 mo,P=0.385).At data cut-off,two patients were still alive:A RAS-mutated patient undergoing 3L treatment and a neo-RAS wt patient who received 2L treatment with anti-EGFR(ongoing).CONCLUSION Our case series demonstrated that monitoring RAS mutations in mCRC by liquid biopsy may provide an additional treatment line for neo-RAS wt patients.

目的旨在通过系统综述,确定针对复杂慢性病患者增强自我管理的干预措施,以改善慢性呼吸困难,为护理人员促进该类人群的自我管理提供依据。方法2023年12月,系统检索Medline、Scopus、Web of Science、CINAHL、Cochrane图书馆和乔安娜布里格斯研究所(Joanna Briggs Institute,JBI)循证实践数据库。对纳入研究进行数据提取和综合,研究人员采用Rayyan文献管理软件筛选标题及摘要,并依据JBI研究质量评估工具进行方法学质量评估。结果共纳入18项研究。这些研究探讨了针对慢性呼吸困难的各种干预措施,包括药物干预(氧气疗法和吸入疗法)和非药物方法(教育计划、呼吸练习、液体摄入管理、身体感知技术、同伴支持、情商培训和网络应用程序的使用)。这些干预措施减轻了患者情绪困扰,可以使患者在自我管理方面变得独立自主。结论促进复杂慢性病患者对慢性呼吸困难的自我管理需要采取综合方法,但目前研究中没有充分考虑到针对患者行为改变的干预措施,需要进一步开展相关研究完善干预措施。
