Esophagogastric junction distensibility assessed using the functional lumen imaging probe

AIM To assess reference values in the literature for esophageal distensibility and cross-sectional area in healthy and diseased subjects measured by the functional lumen imaging probe(FLIP). METHoDS Systematic search and review of articles in Medline and Embase pertaining to the use of FLIP in the esophagus was conducted in accordance with the PRISMA guidelines. Cross-sectional area and distensibility at the esophagogastric junction(EGJ) were abstracted for normal subjects, achalasia, and gastroesophageal reflux disease(GERD) patients, stratified by balloon length and volume of *** Six achalasia studies(n = 154), 3 GERD(n = 52), and 5 studies including healthy controls(n = 98) were included in the systematic review. Normative data varied widely amongst studies of healthy volunteers. In contrast, studies in achalasia patients uniformly demonstrated low point estimates in distensibility ≤ 1.6 mm2/mmH g prior to treatment that increased to ≥ 3.4 mm2/mmH g following treatment at 40 mL bag volume. In GERD patients, distensibility fell to the range of untreated achalasia(≤ 2.85 mm2/mm Hg) following *** FLIP may be a useful tool in assessment of treatment efficacy in achalasia. The drastic drop in EGJ distensibility after fundoplication suggests that FLIP measurements need to be interpreted in the context of esophageal body motility and highlights the importance of pre-operative screening for dysmotility. Future studies using standardized FLIP protocol and balloon size are needed.

The properties of hydrogen atoms in a nano-diamond grain surrounded by an amorphous carbon shell are studied with Tight Binding computer *** samples model nano-diamond grains,of a few nanometers in size,that nucleate within an amorphous carbon matrix,as observed in deposition from a hydrocarbon rich *** calculations show that the average hydrogen interstitial formation energy in the amorphous region is lower than in the nano-diamond core,therefore hydrogen interstitial sites in the in the amorphous region are more stable than in the nano-diamond *** formation energy difference is the driving force for the diffusion of hydrogen atoms from nano-diamond grains into amorphous carbon *** energy well was observed on the amorphous side of the nano-diamond amorphous carbon interface:hydrogen atoms are expected to be trapped *** scenario agrees with experimental results which show that hydrogen retention of diamond films increases with decreasing grain size,and suggest that hydrogen is bonded and trapped in nano-diamond grain boundaries and on internal grain surfaces.

We conducted a systematic review of studies using non-invasive brain stimulation（NIBS： repetitive transcranial magnetic stimulation（r TMS） and transcranial direct current stimulation（t DCS）） as a research and clinical tool aimed at improving motor and functional recovery or spasticity in patients following spinal cord injury（SCI） under the assumption that if the residual corticospinal circuits could be stimulated appropriately, the changes might be accompanied by functional recovery or an improvement in spasticity. This review summarizes the literature on the changes induced by NIBS in the motor and functional recovery and spasticity control of the upper and lower extremities following SCI.

BACKGRoUND: Hypoxic preconditioning can protect hepatocytes against hypoxic injury, but its mechanism has not been elucidated. The aim of this study was to profile gene expression patterns involved in hypoxic preconditioning and probable mechanism at the level of gene expression. METHoDS: Hepatocytes were divided into 2 groups: control group and hypoxic preconditioning group. Biotinlabeled cRNA from the control group and the hypoxic preconditioning group was hybridized by oligonucleotide microarray. Genes that were significantly associated with hypoxic preconditioning were filtered, and validated at the level of transcript expression. RESULTS: Forty-three genes with significantly altered expression patterns were discovered and most of them had not been previously reported. Among these genes,genes encoding superoxide dismutase 2 (SoD2)and interleukin 10 (IL-10) in the hypoxic preconditioning group were confirmed to be up-regulated with real-time quantitative PCR. CoNCLUSIoNS: Many cytokines are involved in hypoxic preconditioning and protect hepatocytes from hypoxiareoxygenation injury, and the increase of oxygen freeradical scavengers and anti-inflammatory factors may play a key role in this phenomenon. Diverse signal pathways are probably involved.
